Used to be the best place for acne treatments - the veteran ladies were the best! I felt as though they really cared for my skin and I was seeing improvements! They recently hired new people and unfortunately, I'm so disappointed. I question if the new esthetician is even well trained because she's so timid when performing the treatments. I'm hesitant to say anything because she's supposed to be the professional and I don't think it should be necessary considering I'm paying shitloads (pardon my French) but damn. CANNOT RECOMMEND TOTAL DERMATOLOGY FOR AGE SPOTTED HANDS... My experience with Total Dermatology wasn't "horrible" like some of the other reviewers, it was just completely disappointing! I found Dr. Pilest and his staff friendly. I went in for a consultation for sun/age spots on my hands. I paid the $100, which can be applied toward a treatment, which was not a concern for me because I had planned on getting a treatment done. Dr. Pilest greeted me warmly and took a look at my hands and suggested doing a few test spots with the Alex Laser to be sure I didn't have any adverse pigmentation problems. He really sold the Alex Laser to me and said that I should see a 90% improvement. I agreed to the test spots and was very happy to take a more conservative approach with my first session. I paid $300 for three spots of which they applied the $100 consult fee. I followed up with the Dr. 2 weeks later. I was able to view the pics that we had originally taken of both hands and I compared them and the spots did not look much different at all. Dr. Pilest said that it had only been 2 weeks and we should wait the full 4 weeks to see if any better. Two weeks passed and I wasn't getting much in the way of peeling or scabbing which is the typical experience from this laser as described in researching about it. I had done IPL and had the pigment lift and sort of turn to what they describe as "coffee grounds" or scabbing and then flake off. I went in for my follow-up which is now 4 weeks out, and the nurse asked if I wanted to go ahead with treatment IF he said that it looked good to him -- I said "yes". What I really should have said was please let me talk to him before I make that decision. I feel that by saying yes, it was the green light for the Doctor to say " It looks lighter" and lets continue with this course of treatment. I could have had a conversation about the intensity levels of the laser and what might be a good option for me, etc. At any rate, I went ahead and had both hands completely done with the Alex laser. It cost me $850 and he also suggested that I apply Cyspera cream after 1 week, which cost $210. At the time, I thought to myself that the Cyspera cream was his way of making sure I got some kind of lightening if the laser did not do the job! Anyway, I went along with the plan and came back for a 3 week follow-up this time. At 3 weeks, I still did not see a lot of lightening of the spots. Yes, they were visibly lighter than when I started this whole process, however, I did not consider them significantly lightened. At this visit, I confirmed with Dr. Pilest that he turned up the laser to the next higher level and he said he did. He then suggested that he could give me 1/2 off another treatment and that he would work with me. I appreciated that, however, it did not comfort me much because I wasn't getting results and why would I pay more money for not much in the way of results! He finally suggested that I use the Cyspera for a month and even keep it on all night if I wanted and then come back for a follow-up to see where I was at. I agreed. I did try the Cyspera for a month and only left it on for 1 full night. I did not like the very strong smell of it - even through my cotton gloves. I just used it for double the normal time -- 30 minutes a night and washed it off. It did lighten the spots a bit more. A month later my spots are still there. Yes, they are lighter, but not significantly -- not 90% -- no way!! I called Total Dermatology two days before my 1 month follow-up, I called and asked if the Dr. would be willing to do a couple of test spots for free at a much higher intensity to see if I responded to that and if so, then I would gladly pay for another treatment on all spots again -- I just wanted results -- is that so unreasonable? The front desk called me back with the answer -- Dr. Pilest would be able to do a couple of spots for $300 as we did before. Wow, I was a little surprised by that -- I mean he knew me pretty well by now and knew the trouble I was having getting any real results -- and he couldn't agree to this? Not a very good business strategy. I would have happily gone back to Dr. Pilest and Total Dermatology and spent my money on many other procedures they offer. My next plan was to get Radiesse injections for volume loss in the back of the hands. Goodbye Dr. Pilest :( read more

This doctor is a total hack. He will make drastic mistakes that may cause permanent damage to you. He will never admit his errors, and he will lie to you and use double-talk to try to convince you his mistakes were actually intentional. You won't get an admission that he made an error, and you won't get your money back. Run the other direction. He needs to be put out of business. read more

I asked my plastic surgeon who I've known for over 16 years about a service I wanted to have done at total dermatology and he said " it's Unethical Another Dr would even say yes to doing that service , he clearly is just wanting to take your money!" I researched dermatologists from LA all the way to Oc and through my intense research I decided that Dr pilest was going to be my new doctor. I'm very selective when it comes to anyone touching my body or face, so I didn't make this decision quickly, I REALLY did my homework. I stalked him online for 3 months, maybe more before taking the plunge. I decided that my skin and face needed some minor touch ups and I felt that with his experience as a dermatologist, as well as being a very high up injector directly for allergen (maker of Botox etc) he would know exactly what to do for my face and skin. I had my appointment today and wow, he really knew exactly what to do and was such a perfectionist, I really loved him and his entire wonderful staff. He listened to my concerns and he also measured my entire face using the "Golden ratio" because he wants his patients results to fit their face and make them look the most symmetrical as possible which is extremely important when you want to look beautiful. Once my minor swelling goes down I'll add another photo but here I am before my procedure numbing. Thank you dr pilest ! See you in a few weeks for my facial and very first laser next! Ps- if you're complaining about Dr pilests prices please hold that thought. He is an experienced doctor who has an office and staff to pay. This isn't some back alley Tijuana shop. This is your health and your face, and anything cosmetic is a LUXURY, NOT a necessity, so think about that before you walk in the door. He doesn't deserve some of the bad reviews I saw before walking in his door. Luckily I'm smart enough to see past the penny pinchers who think they are "entitled" to these luxury services at a safe high end place expecting Tijuana prices. Admittedly this is an odd review for me to write, as it pertains to an experience I had with Nissan Pilest as a 13 year old child (so 15 years ago). I've never forgotten his name because I was so traumatized by my experience and am horrified to see he is still practicing and offering laser hair removal treatments. My grandmother offer to gift me some light laser hair removal only between my eyebrows as I was "blessed" with a unibrow. Super attractive for a budding teen. I saw Nissan Pilest maybe only two or three times max for laser hair removal treatments, because on my last visit (I was supposed to have more but we promptly cut off all treatment with him), he made a huge mistake. He lasered off half of my right eyebrow. I wish I was exaggerating. It hurt like you-know-what because I wasn't numbed there (obviously, since it wasn't supposed to come off) and because the hair was so thick and dark there. My father was absolutely furious and nearly sued the pants off this guy. I was sobbing and inconsolable because that's the LAST thing an awkward tween needs...add it to braces and acne and holy crap thanks a lot. I spent the rest of that year trying to fill in my bald eyebrow as best as I could, but was teased like crazy and had zero makeup skills back then so I didn't know what I was doing. (I'm a makeup artist now, go figure.) To this day, that eyebrow has never been the same. Yes, some of it grew back because it was only one treatment. It is still much thinner than my other eyebrow, and since that day I've never let anyone else touch my eyebrows. I shape them myself and fill them in every single day so they look more even. Later this year I will be trying microblading for the first time (permanent tattooed individual hairs) to finally have a more permanent solution to what became a permanent problem. Do NOT give this man your business. And definitely don't take your daughters in for laser hair removal, especially anywhere near the eyes. At least he only took off half my eyebrow and didn't slip even further because he could have easily damaged eye or even blinded me permanently. I guarantee he remembers me and this incident since it was such a huge mistake and my father escalated the situation. By the way, he never admitted fault and actually tried to convince us he "did it on purpose" because my hair was thicker there and he wanted it to grow in thinner. Um, no. You don't remove half of a child's eyebrow without consent or without numbing them. Actually...you just don't remove half of anyone's eyebrow. There's zero percent chance it was on purpose and for a medical reason of any sort. It was a mistake and he knows it. read more

I came here for a facial/peel. However, instead of washing, doing extractions and cleansing my face. I walked in and sat in a chair upright as a medical assistance tried to apply the peel over face (which still had make up on it). I have been getting facials for over 10 years. I have tried many places, but this was the first time I was not treated by an esthetician. Also, I have never paid for a facial/peel where they did not cleanse my face prior. Despite this, I still paid the same price as any other medical spa. Personally, it was not worth the price. I saw no result. It really feels like this establishment only cares about money and providing adequate service to those that they want to, I was extremely uncomfortable my entire visit and I will never come back again. It's been several years since I last visited, but the sour taste remains in my mouth. Dr. Pilest had some wonderfully kind staff members, and the office was always welcoming, but every time I visited, it just happened that whatever new (expensive) treatment or product he had recently started selling was exactly the one I needed for my skin---what a coincidence!---and I left for good after he complained to me one day that "this office is too cold because all the women who work here are on their periods and they keep turning the thermostat down." Every time I come to see this doctor I feel that I walk out of there robbed. He never tries to get to the root of the problem by trying to run tests then working from there. What he tries to do is sell you all of his products and once you're walking out of his office $500 later then he tells you that he'll run blood test and what other options he feels he would recommend. Plus they don't accept any insurance. You either pay cash or you have to deal with billing and the insurance on your own. Good luck with that. read more

Wow! I just had a really lovely experience at Total Dermatology in Irvine! I was given Dr. Nissan Pilest's name from a different dermatologist. I am so glad I had this referral. From the start this office treated me with respect. When I called to make the appointment the front office staff was warm and friendly and did a great job explaining to me how the billing process went so I didn't have any surprises and I got an appointment right away. I love the decor in the office! All the colors are soothing and absolutely beautiful. The entire office, rooms and bathroom are super clean. This is a really big deal to me and I was so happy. The woman that brought me back to ask me questions about my needs and concerns is such a doll! Maria is so professional and kind. She was very caring about my health issues and very supportive. She never tried to sell me anything like other businesses try and push on you. No one made me feel pressured about any decisions nor did they try and sell me something I don't need or can't afford. In fact, Maria was incredibly understanding about my finances and worked with me to ensure I got what I needed for my budget and timing to get things taken care of. She treated me as compassionately as a family member. :) She also answered and addressed all the questions I through at her. Dr. Pilest is a very kind and gentle man. He has a warm demeanor, a great sense of style, very professional and obviously very good at his craft. I left his care feeling confident in the choices I made and his advice for aftercare for my procedure will be followed. I will be continuing to come to Total Dermatology for my future needs. I appreciated that Dr. Pilest made me feel comfortable in my own skin yet he was able to make my skin even more beautiful. If I would have known this from the start, I would have NEVER gotten ANY treatment with him, 'nor speak so highly of him to all my friends, family and clients. I got emails a week prior to my appointments saying that if I didn't give them 48 hour notice for cancellation, that I would get charged a $100 fee. What makes my time any different? Worst experience ever !!!!! I did laser hair removal and that Laura doctor who has been doing it forever ... BURNT me so bad it's been 2 months and I still have burn scars everywhere .. ( Nissan pilest looked at them and said no problem it will go away ) still here !!!!!!! Apparently Laura doctor had it on high heat by accident? How does she even have a license to laser? How do you make a mistake like that????????????!!!!!!! Not to mention they didn't even offer me a refund!! I had to ask for it ... I recently visited Dr. Pilest about an indented scar I have on my cheek and he suggested Juvederm Ultra and I was glad because I didn't want to do laser on my entire face when I only have a small issue I wanted addressed. I had my appt yesterday to get the filler and it was super quick staff is so welcoming and friendly, Dr. Pilest was so professional and the procedure was quick and painless and so far loving the results. Wow factor here! I was totally impressed with the Dr , his staff and his facility. Second visit earlier this afternoon and I'm happy to say that I'm just as satisfied! Made the appointment last week, which was surprisingly easy even though my schedule is all over the place, but the front desk was willing to stay patient with me and trying to find a time. Came in and I don't think I waited more than 5 minutes to be taken back to my room. Rachel was working again and she helped me sign in and figure out my account so that I could use my points for the best savings. This doctor is SO quick and painless! It really helps that they have another nurse in the room to rub your arm and distract from any discomfort. Again, he still wasn't pushy or made me feel insecure or as if I needed a ton of work done like other doctors I've seen in the past. I think he truly cares to improve, but not over-do it. HE DOES IT AGAIN! Dr. Pilest and his team ARE THE BEST THERE IS. shout out this time to Jessica for being by my superwoman cheerleader and Ana and Sandy for being amazing like always. I'm the BIGGEST baby with needles and I basically pass out every time I get a vaccine or piercing. Dr. Pilest always goes super slow and his nurses do absolutely EVERYTHING to make sure I'm comfortable and ok with whatever is going on. I got lip fillers for the first time yesterday and fainted for the first time during my injections. Jess applied topical numbing cream before Dr. Pilest started, and I could barely feel anything. Dr. Pilest was patient and went slow, but my silly brain worked itself up and I passed out so embarrassing! They woke me up, got me some water and a cookie and Jess even got a little fan to blow on my face! Dr. Pilest was super understanding and gave me all the time I needed to recover, and then finished my injections BEAUTIFULLY. I love this place. I love this team and I love this doctor. I'm shipping myself off to Hawaii in October, so I scheduled some much needed facial maintenance - hence I'm back to see Dr. Pilest and Sandy. Today was Juvederm XC, Restylane Lyft, and a wee bit of botox for my marionette lines - the results are immediate, and Dr.Pilest fixed my problem areas in one near pain-free sitting. No bruising, bleeding, or noticeable swelling; my face was left with several miniscule pin pricks where the needle went in, and a rejuvenated younger looking mug. I'll be going back in 3 weeks for maybe a slight touch-up since I had some filler leftover, but we'll play it by ear and see what the he says. His word is the equivalent of the law as far as I'm concerned. We all know how I feel about him, he leaves me in awe. Why am I writing yet another review for TD? I wanted to give a shout-out to my girl crush/nurse extraordinaire, Sandy. Sandy is truly a thoughtful, beautiful individual. I see her 3 to 6 months at a time, but when I there, she puts me at total ease. She's super informative and patient with my questions, generously numbs and ice me, gives me the kind of support I would get from a legit friend I've known for years. She's genuine, a bundle of vivacious vibes, and being around her makes you feel good about yourself. All this, and I didn't even touch on her extensive medical experience. Please clone this woman already. Since I come in regularly, they suggested I sign up for this "Brilliant Distinctions" Program. Apparently I can earn points for my future visits for Juviderm or botox, so naturally I signed up once I got home.
