If I could give zero stars I would. My father was placed here to recover from a broken leg and pneumonia. During his stay, his leg was never elevated and he was always laying flat on his back. They took him in a wheel chair naked to weigh him in the activity room where other residents were and he had no privacy. He said people would scream at night "get me out of here!". He was doing okay the first few days but after that he was a heavily sedated on pain meds. They were giving him pain meds every 4 hours plus a sleeping pill at night. He was on so much pain medication that he forgot how to use his phone. He wasn't making any sense and stopped eating. He even lost his dentures. The nurses will Leave you sitting in a bed full of urine all day before they change the sheets. They don't clean you up well and tear the skin when wiping you. My father ended up being discharged back to the hospital because he developed double pneumonia plus and intestinal disorder called ileus (which was most likely caused by the opioids). My father passed away a few days later at Palomar Hospital and I believe this rehab contributed to his death. Please do not send you loved ones here. They are short staffed, the place is a dump and your loved one deserves better. read more

This hospital is a Joke and unsafe. They let a young man stay there who has mental issues and I was in the room with him and he broke my phone and stole from me And he attacked 2 disabled residents and was allowed to stay there. I didn't feel safe and hurried up and called my insurance to get me out of there. Showers ever 4 days and if you buy food staff is not allowed to hear it up to you but can cook popcorn for you. They treat you like animals and the food was always cold. And doesn't go by dietary instructions. If you can't eat the food mostly cold sandwiches are offered. Being a diabetic they have no late night snacks or ice because the kitchen is closed. And don't let them take your clothes to laundry you'll end up with so one one else's clothes or they would be kept for weeks without being returned. Some staff are very mean some are nice. If you hit the call button maybe because you have a accident or if they take you to use the restroom they rush you and when they clean you they leave you WET AND STINKY RUSH JOB. They gave me the wrong medicine without my permission or explaining to me why they gave it to me. Now I'm fighting low blood pressure. They are understaffed and some of the nurses who have been there longer are just there for the pay check. If you try and ask questions they don't have time to answer your questions in which could be bad for your health I LOST 1 WE POUNDS MY FIRST WEEK BECAUSE OF THE MISDIAGNOSIS AND THE WRONG MEDICATION. I almost lost my place of residence because it took staff 3 weeks to send documents after telling me that they had. I had 2 strokes and I be STRESSED OUT. I HAD TO BUY ME A NEW PHONE AT THE TIME MY FAMILY COULDN'T GET IN TOUCH WITH ME MORE STRESS. I WOULDN'T SAY THAT THE PLACE IS ALL BAD BUT IT WOULD BE THE LAST PLACE I WOULD SEND MY FAMILY TO. Family member Was sent there for Rehabilitation expecting a facility resembling a rehab. This place has a mix of everyone. It's more of a skilled nursing home for older people. People yelling and screaming often throughout the day and night. Impossible to nap or sleep at night. Get woke up multiple times per night. 2 people share a room divided by a curtain and also share a bathroom not only with themselves but also with the adjoining room. So 4 people share 1 bathroom. That alone is a nightmare especially when you have the opposite gender using the same bathroom as you. Service is horrible. Don't expect anything immediate. It usually takes anywhere from 20-45 minutes for a nurse to come in and help. By the time you get your pain meds, they don't work because the pain is too much! They also take forever to help clean you up. My family member has sat in their own mess on many occasions waiting to be cleaned up for over 30 minutes. And on some days when I've gone in, they were not cleaned all the way and still had soil on them and on the bedding. Very dissatisfied with the quality of care. Most the staff I believe are there just for the paycheck and not for the patient. The food is also horrible. Warm things are cold and cold things are warm. Seen toast that was not ever in a toaster, cheese never melted, wrong items all the time and always missing food items. I don't think the kitchen staff read the meal tickets. Not many options either especially if you're on a special diet. Food items loaded with sugar, fat, calories. Meals are more like horrible school cafeteria food . When visiting, when you walk in, you are hit with the most atrocious smell of I guess deodorizer?? It's very strong and overwhelming! But once you walk past a few residents in wheelchairs and smell urine and stool, you realize why they use the strong perfume smell. Parking is another issue. Not many available spaces and only 1 handicap that I saw. Roads to get there are in very bad condition as well. Lots of uneven asphalt and pot holes everywhere!! Definitely drive slow! We were given several options for a skilled nursing facility. Valle Vista Post Acute is highly recommended. When we visited, we were greeted by friendly staff, and had all of our questions answered. The building is clean and very welcoming. Ana and her team provided impeccable customer service. My husband was in Valle Vista Post Acute for about 2 months from August to October 2023. He was rehabbing from a leg amputation. My husband also has dementia, so it is a complicated situation. The care there was very good, the nurses were attentive, he was kept clean, and his wound care was superb. The most important aspect of the stay for us was the physical therapy. I can't say enough about the physical therapy group, especially Rod, he was caring and took the time to overcome my husband's muddle mind. My husband came home being able to transfer himself to his wheelchair easily. They also worked with me to help train me on how to transfer him from the bed to the wheelchair, help with how to pick him up if he fell, and how to transfer him to the car. This has made the homecare for him much easier. Although the facility is older, it has been updated and is clean. The view around the facility is beautiful and there are nice areas to take your family members out to see the views. read more

Absolutely HORRIBLE facility!! Do not take your loved ones here. They do not care about their patients and will completely ignore cries for help. My grandmother sat writhing in pain for over 5 hours after having surgery 2 days before for a severely fractured hip. They have absolutely no compassion and have no business taking care of anyone, especially their night staff. After calling for help for HOURS, no staff member ever came to her aid. Her roommate thankfully called 911 where she was assisted back to the ER in an ambulance. I don't know what she would have done without her wonderful roommate. Staff never helped! I wouldn't even let my pets be cared for by these people! The following night we had no choice but to re-admit her to Valle Vista since it was the weekend and no other place was available. Less than an hour after our family member left, my grandmother fell and was left on the floor for an hour until paramedics arrived. We were called at 1:05a, with my grandmother still laying on the floor, when we had to instruct them to call 911. Where was the staff? Why didn't they even attempt to get my 91 year old grandmother off the floor? How long did it take them to respond to her this time? No efforts were made to help my grandmother. No one lifted a finger outside of calling her family. So again, she sat writhing in pain, on the floor this time, until paramedics came to help where she was taken back to the ER. This is a horrible, sad excuse for a "care" facility. Please, do not take anyone here. Awful place. read more

PLEASE STAY AWAY FROM THIS FACILITY!!!!!! We had my mom go here for a respite visit (5 nights) she was admitted June 3rd, 2021 at 11:45am. The following morning June 4th, 2021 I called the facility to check on my mom when the intake coordinator stated "we have more paperwork for you to sign" ok cool I'll just pop in and visit my mom while I'm there. Dear lord.... When I walked in roughly around 9:15am I found my mom now comatose, laying in her own feces now dried to her and still in the same diaper she was admitted in. (I packed different colored diapers for her stay) her catheter hose was twisted shut, the humidifier was placed back on her oxygen machine after they were told to not put it on because my mom had lung cancer & there was SERIOUS concern of aspiration of fluids, my mom had not been administered any of her medication the first day she was there, I tested my mother oxygen levels and they were low 70s. I immediately went to the nurses station to ask them to explain to me why my mom was being neglected and they all just stated at me like I was speaking a foreign language. I called my hospice team and requested and emergency transport from the facility. As soon as I went to walk back down to my mom's room 2 nurses showed up with a sitting shower chair (my mom was bed bath only AND comatose) I told them to get out. Then another nurse showed up to try to change my mom. I told her: "Get out, I trusted you to take care of her when I wasn't here & clearly you cannot do your job. I'll change my mom myself" I refused everyone who came to the room until my transport team arrived to take my mother home. At the point where my transport team and I went to wheel mom out the nurses tried to tell me: " you can't leave with her, her oxygen is too low" my response was: "trust me, I know! And you only know because I did your job! Can you tell me how long my mother has been deprived oxygen?" They had no answer. We got my mother home.... She died less than 8hrs later on June 4th, 2021. This has been reported to the California department of public health. The San Diego county ombudsman, and I've contacted an attorney. I have witnesses, and even photographic evidence. My mother didn't deserve this & neither does your family member. read more

My mother spent 2 weeks here after a week's stay in the hospital. The majority of the staff here were fantastic. There were a few exceptions. Also the facility was clean! Again, with a few exceptions. The food, as you would expect in a hospital type situation was less than appetizing. I got a call back from the San Diego government official that does investigations and he said the facility had no records of a bedsore so there wasn't anything they could do. I pointed out the fact that he had been home less than 24 hours when we had a nurse examine him and he had a stage 2 bed sore.. we were told by the nurse that it would take more than 24 hours for the sore she say it was almost a stage three. That meant the facility not only gave him the bed sore but his care there was so bad it went unnoticed by the staff or they willful failed to report it. My step father pass November 5th. How the elderly are treated in the facilities should be monitored more so they do not get away with their shop of horrors and neglect read more

I was transferred here from Palomar Hospital in Escondido, CA for a broken tibia and fibula. After emergency surgery with two titanium rods and screws, I stayed at Palomar for a month. They had occupational therapist and physical therapist at Palomar Hospital which were great. I was then transferred to Valle Vista Post Acute Care. The nurses wore tags with Covenant Care on them. They put me in a bed without any way to reach a nurse. I had to borrow my roommates walker to go to the bathroom. then they yelled at me for getting out of bed. Everthing smelled like urine and was obviously not disinfected. They continued to leave me alone in my bed without anyway to get assistance. I urinated in my bed three times at night because I couldn't find the walker in the dark, and no one came to check on me. I called and got a ride home from a friend. They yelled at l me because I did not trust them and felt unsafe. I just needed out of the discusting conditions. PLEASE DO NOT GO HERE! PLEASE DON'T SEND ANYONE YOU KNOW HERE. IT IS UNSAFE! My sister was admitted to Valle Vista on June 25, 2021 and my family removed her the following day. She was transported from a hospital to Valle Vista to regain her strength following brain surgery. When she arrived, they had nothing in place for her. (NO pain meds and NO "food" for her feeding tube) They weren't sure if those items would arrive before morning. The charge nurse hadn't shown up for her shift so the nurse at the desk was edgy as they were scrambling to get my sister's room set up. It took three attempts with different oxygen machines before one worked. (Faulty machines....outdated electrical outlets.....or both?) Because of "policy" a family member wasn't able to stay the remainder of the night in her room to see if she was safe and being attended to. Besides the lack of medical care, the room was run down and depressing. The wall behind one of the beds had been repaired but not repainted (big white patch). The remote to adjust the old bed was yellowed and dirty. There was an old fashioned fluorescent light hanging on the wall next to the bed but the string (attached to a little chain) to turn it on was at the foot of the bed. How would a person who is rehabbing possibly reach it? I couldn't stomach going in the bathroom but another family member did, and suggested that I avoid it. While looking around at the faculties I couldn't help but think, "With all of the job availability in the health care field, who would choose to work here?" And then it all made sense! My mother is currently at Valle Vista after a having complete hip replacement about 3 weeks ago. She asked me to take the time to write them a good review. She said she feels they work very hard to take good care of her. They are kind and caring and the food is good. They have been very welcoming any time we have visited. I also feel that they are thorough in their routine checks and care. They offer activities such has movie nights, facials and musical get togethers. My mom has said more than once that she appreciates all they do and how they treat her. We definitely recommend Valle Vista. read more

My mother was recently here after suffering a fall. She was so hesitant to go to a skilled nursing facility but we knew it was a necessary step in her recovery. We are SO glad we chose this facility for her physical therapy needs. My mom had a great experience. All the staff, from the CNAs to the kitchen, were so kind and always so helpful when we had questions. The therapy team was FANTASTIC, they really get to know their patients and that makes a big difference. My mom is home now and we couldn't be happier with how well she's doing. We moved my grandfather from a different facility, which we were having major issues with regarding his level of care, and since our family has been very happy with Valle Vista. My grandpa's nurses have been very attentive and patient with him. He requires a lot of assistance and we have never had any concerns with thinking he was not getting the help that he needs everyday. My mom is a grumpy old lady that loves to complain. So when she was sent here for therapy after her surgery. I was like I feel bad for those nurses that are about to experience the wrath of my mother lol. But they were able to deal with her and make her feel like she was at home. Everyone that works here are very nice and very professional. Nursing homes are not as bad as people make them out to be. Well.... at least not this one anyways. The only complaint that I agree with my mother about this place is how loud this place can get at times. Even at night there's so much noise that it makes me wonder how does anyone sleep here at night. They should have a certain hour where it should be quiet time. Left the facility 9/26/2020. My mother went in to this center after having her gull bladder removed and succumbing to an infection. The first two days, she was visited by the staff constantly, "mama, mama," the mostly Filipino staff would call out. After the third day of three weeks, they abandoned their care and concern. They refused to help her to the restroom, to explain her treatment and/or condition. Her "cell-mate" who was a morbidly obese patient would yell and beg for help to use the restroom and when no one came, she would absolutely soil the entire restroom and herself. Imagine your own mother, recently operated and having to use the restroom after that person's visit. They pumped my mother with Ozycodone like it was going out of style- now I know why! Not everyone is malingering pain to get a fix! Many patients have ailments which need medical attention, not sedation. My mother ended up with a strange ear infection and can barely chew (a condition she didn't have prior to her internment there. If your loved ones is in fact loved by you, don't do this evil thing to them and allow them to suffer what my mother went through. My mother shared "we all cried, begged and pleaded, day and night." And, other comments before are not exaggerating; the "food" is absolute garbage. read more

My main complaint is that I don't think they brief each other between shift changes but if you give them a few minutes they can figure out what you need from them. Unfortunately I have to give at least one star but if I could I'd give absolutely nothing! This facility is racist as all get and they MURDERED my father! They took forever to send me his medical records, and once they did, they were incomplete!! They were trying to be sneaky and left out the last week of his life. They neglected him and his plan of care and thought that I wouldn't notice. I have voicemails from my father that they were abusing him and neglecting him!!! They intentionally deprived him of his inhaler and any type of oxygen treatment for 5 days!!!!! That's when he suffocated and went into cardiac arrest. Once my attorneys contacted this facility and medical records, they quickly coughed up the remaining missing week, and I know for a fact that they were tampered with. This place needs to be shut down. DO NOT SEND YOUR LOVED ONES HERE. THEY WILL BE ABUSED, NEGLECTED AND WILL DIE! This place will get their karma I promise you! read more

Excellent physical rehabilitation facility and nursing center. Staff is very responsive, kind, striving to exceed expectations. The hospital administrator has an open door policy and is very receptive to patient concerns and suggestions. The food is excellent under new management and is always willing to exchange food upon request. The Physical Therapy department is equally excellent and kind. We toured the facility because they have a good rating from Medicare. The facility was dirty (black, grunge around the toilets in the rooms, smelled like feces in the hallways, patients belongings were on the patio, and so MUCH more). When asked about staff turnover, the director told me she really didn't know. WE visited because I needed a rehab center for my dad. Please search all of the places on the list you are given at the hospital. Don't pick a place just because it is close or has a good Medicare rating. read more

Had looked at a couple of places for my Mom who was going to need assisted living care. I picked Valle Vista which seemed to be the better of the two. Wow, was I wrong. My Mom was there less than 24 hours and she called me at 6:30am the next day and said she wanted out. When she used her call button for help the caregiver said he was having dinner. Not acceptable. Lots of patients screaming all day and night. If your loved one has their mind do not, I repeat do not put them in this horrible place. So glad my Mom who is 88 still has her mind. We placed her back at a group home she has been living at for 3 years called the Cottage in Escondido. I highly recommend it. It is not a medical facility but they do accept hospice. I was personally admitted into Valle Vista Convalescent Hospital on 02/10/16, following amputation to my left foot. I remained in their continuous care until my subsequent discharge today, 04/02/16. Upon my arrival at the facility, I was greeted by a host of personnel, who proceeded to introduce themselves, detailing their titles, and how they might be of service to me. The nursing staff immediately took to their duty of settling me in. I found them to be very professional, and greatly appreciated their "that's what we are here for" attitude. I was treated as one might expect to be treated by family. The kitchen staff did a superb job of learning our personal preferences, and honoring such. Our meals were always tasty, of ample portions, and promptly served to ensure proper temperature. The housekeeping staff did a magnificent job of keeping our rooms, as well as the general areas, sparkling clean. The laundry staff did tremendous work at laundering all of the soiled linens, and our personal garments. The maintenance staff routinely surveyed the premises for needed repairs, and wasted no time in tackling such. The transportation staff was always ready to chauffeur us to any appointments that we required. The activities staff miraculously kept boredom at bay through movies with popcorn, arts & craft, cookie baking classes, bingo, root beer floats, etc. The physical therapy staff expertly assisted us as we worked on mending ourselves. The social service staff diligently searched out answers to all our needs as we prepared for discharge. The administration staff obviously possesses a clear & concise understanding of the Covenant Care mission, and it is no doubt through their leadership & dedication that this facility has earned a five-star rating from this extremely impressed and satisfied former resident. I have been a caregiver for 27 years and have never been so disgusted with a SNF in my life!!! Robert Adkins was admitted to Valle Vista on August 18, 2017. He was neglected from day 1 and was admitted back to Palomar Hospital after his health had deteriorated to the point he was severely dehydrated and was starving. There was no one to help these people on the weekends, If not for Roberts family he would have passed away in that hell hole. Robert was admitted to Palomar on August 27 where he was put on Elizabeth Hospice and died peacefully September 4, 2017. My family put my grandpa in this home because he needs assistance with certain things since he is in a wheelchair. This place never ceases to disgust me on a week by week basis. I visited my grandpa the first day he was put in this place, and I had to ask a nurse 3 times to change his catheter bag. All day that I was there I didn't see one nurse check on him. The next morning when I went to visit him, at 10am he was still laying in bed waiting to be helped up and into his wheelchair. There are countless nights that my grandpa calls my mom or one of us because he presses he help button and the nurses ignore him. The entire family has called this place asking to help my grandpa, when was the last time they checked on him, why are they ignoring him, and they finally refused to take our phone calls. My grandpa ended up in ICU last night because of another filthy, unchanged catheter bag. He was close to death. We are currently looking in to a non-abusive, better home for him. When I went to visit my father in law, I noticed he was unkempt. I immediately asked that he be changed into clean clothes . The LVN stated that he had been sleeping all day. Unacceptable! 2 days later, we went to visit again and it was obvious he wasn't cared for and worst of all, he was running a fever. There wasn't a doctor on-site so, my mother in law called an ambulance to take him to the hospital. It is obvious, someone wasn't doing their job!!! My father in law was neglected once again by this facility. It seems they do not care about their patients.... I suggest you visit!!! You would be UPSET!! It is dirty and the patients are ignored on a constant basis. When you are there, the patients want to interact however, are ignored or shooed away. It is hurtful watching this! I suggest that someone visit this place and close it down until it has qualified caring staff and the facility has been cleaned up! read more

The staff are very friendly and I can see they work very hard to accommodate all of the residents need. Staff has made me feel like their own family too. The facility is clean, no odor and my mom is very happy with their care and service. I am very thankful for Valle Vista, I feel very at ease knowing my love one are in good hands. read more

I don't know how Valle Vista was operated in the past, but I know that there is a new Administrator and a new Director of Nurses that care. They are out visiting with the patients and families and follow-up on the smallest of concerns. Their attitude is reflected in the nursing staff attitude. Each day the food is better and better with lots of fresh fruits and vegetables. Mom is due to have a second hip surgery in a month and we will come back to Valle Vista! read more
