To all new patients that are looking to choose Otay Lakes Dental Group for Dental Care. This bad review is regarding Dr. Quan Alex Lui. I have been a patient at this office for more than 10+ years. During that time, I had no ongoing conflicts, no unresolved issues, and i was never informed that I was considered a problematic patient. On June 17, 2026 I went to the office for a scheduled dental cleaning appointment. I checked in at the front desk and was treated respectfully and professionally. I was then taken in to take x-rays and moved to another area where a dental assistant polished my teeth. After that , I was asked to wait for Dr. Lui to complete my teeth cleaning. I waited for approximately one hour without complaining. After that wait, Dr. Lui approached me abruptly, rudely, and disrespectfully and told me that he refused to provide treatment , that I was a bad patient, and that he did not want to see me ever again. He said this was because of my past complaints about the quality of teeth cleaning not meeting my expectations. I calmly explained that this involved a single incident approximately 3 years ago, when I believed a prior cleaning had not been performed properly. At that time, I had requested that my teeth be cleaned again because they still felt dirty afterward, The office honored my request, the matter was resolved, and I was really satisfied. In fact, Dr. Lui personally re-cleaned my teeth at that time. After that incident , I returned for additional visits and specifically requested that Dr. Lui perform future teeth cleanings. I was never informed that there were concerns about my behavior, character, or patient status. This incident was especially upsetting because my 8 year old granddaughter witnessed the interaction. She became frightened, distressed and concerned by the way I was spoken to and asked why I was being treated so rudely and aggressively. My granddaughter texted her father to informed him that Grandma was just kicked out of a dental office, my son Jorge Martinez my granddaughters father became very concern by text sent to him. Mr. Martinez called me on my cell phone in the middle of my conversation with Dr. I expressed to Dr. Lui how I didn't appreciate in the way he was handling unresolved issue based on a past incident, and the way I was being spoken to in a very disrespectful and humiliating way. I felt like I waisted my time and I couldn't believe he was refusing services to finish my teeth cleaning in the middle of treatment. Dr. Lui didn't care or display any remorse or compassion to listen or understand me to look for better way to resolve the problem or misunderstanding. Dr.Lui walked away, another staff employee had heard the conversation. I explained the truth of what had happened, but instead of trying to resolve the situation. I was told that Dr. Lui was the boss and that what he said was final. Mrs. Gretchen then asked me to please leave, escorted me out, and told me that the office no longer accepted my PPO insurance. Afterwards I checked the office website and it clearly stated that my PPO was accepted. This left a concerned that I was being treated unfairly and possibly given inaccurate information while being removed from this office.

Using high end technology which was a first for me. I am use to the normal x-ray but they went above and beyond and offered the 360 scan. I was a bit hesitant on doing this since insurance barely covers anything these days. I was insured that they do this for their knowledge and it will not cost any extra. The only thing I didn't like was the pictures being taken of my teeth it took a long time for the tech to get the camera to work which made my visit longer than expected. I went back a couple weeks later for my fillings and they did a great job. The doctor was very caring and asked if I needed breaks which was a plus. My cleaning was great as well he was very knowledgeable and gave me great tips which helped me improve flossing. Parking lot available for parking. Office was very clean. The low score is for the inconsistencies in who you get to care for you, and the poor quality of work. When I first went here I was advised thay I should replace two older metal fillings that I had done many years ago. I was reluctant because they were not bothering me and felt great, but decided to go through and get the newer fillings done. Schedule with the doc and come in on appointment day. Was told they would only do one filling that day and not two. Not sure why, but okay. Filling done and scheduled for second filling. Show up and told that another doctor will be doing the filling. Not over thrilled with having multiple doctors as they each have their own way of doing things. Second filling done and its poor quality work. Sharp edge left, and not properly grinded down for bite. Cant feel that though since my mouth is numb. Had to reschedule to have that fixed. Went back in months later for a cleaning and again have a different doctor who does my care. I prefer to have the same doctor all the time, but having a different doctor each visit left me disappointed. To make matters worse, I write this a couple years after getting the fillings. The filling from the second doctor cracked and pieces broke off while eating. Went to a different dentists office who fixed the cracked filling and made mention that the other filling which wad done here was done poorly and re-filled that knew. No problems with that procedure at all.

I believe Dr. David Garate is a very capable dentist however I think there is a practice to overbook his services to the extent of causing extensive delays to his patients. I originally scheduled a routine cleaning but was advised to get a deep cleaning. I inquired about braces and was told I needed a frenectomy before the braces were put on. I followed through with the regular cleaning, the deep cleaning, and the frenectomy. This process took several weeks to complete. Several times I was made to wait to get my procedures started as a hurried Dr. Garate was busy finishing work on previous clients. In one particular instance, once my procedure started, he was so frazzled that he numbed the wrong side of my mouth with the numbing needle. As you can imagine, this is a very painful experience which involves stabbing you in the nerve of your gums with a long needle and injecting the numbing solution. He apologized profusely for doing this. He gave me some Starbucks gift cards in an attempt to make amends however the insult remains. This should have never happened and was completely unprofessional, avoidable and unacceptable. However the most traumatic experience came during the frenectomy. Dr. Garate applied the numbing gel to my gums and let it sit less than 60 seconds before jabbing me with the numbing needle. As it was previously explained to me by one of his assistants, they leave the gel on for far much longer in order to help ease the pain of jabbing you with the numbing needle. Dr. Garate wasted no such time and I felt EVERYTHING. Then came the procedure of the frenectomy, which uses a laser to cut through the muscle tissue in between the front of your two front teeth and behind it. It is literally burning your flesh. When giving post-op instructions for the pain once the numbness wore off, Dr. Garate told me to simply take Ibuprofen. Mind you, I just had the inside of my mouth burned for over 30 minutes. When I tell you this was some of the most excruciating pain I've ever felt in my 48 years on this planet once the numbness wore off...it brought me to tears over several days. I lost 5 pounds due to my inability to eat comfortably due to the pain. I had to call in to be seen way in advance of my scheduled follow up to get relief. Once seen, Dr. Garate halfway admonished me for taking over the counter pain relief remedies such as Orajel when he left me to my own devices as he could only think to suggest Ibuprofen in the aftermath of the frenectomy. Ibuprofen could not touch the pain that ensued from the burn wounds to my inner mouth after this procedure. He then filled stronger pain relief medications on my behalf to my local pharmacist and provided vitamin E pills from the office that he instructed to burst and rub on the area. My question is this, he obviously has done this procedure numerous times in the past. Why not anticipate the pain level and provide prescriptions at the conclusion of the procedure? If not that, why not instruct on the Vitamin E remedy in the least? Why send a burn victim home with dreams of Ibuprofen? Why put a valued patient through this painful ordeal unnecessarily? What amount of Starbucks gift cards could Perfect Smiles distribute to me to make up for what they put me through???

She did a root canal and crown. The crown fell out twice, and the second time the tooth broke off, she didn't want to deal with it and was supposed to to refer me to an Oral Surgeon, she never did so I loss the tooth. Go somewhere else if you like having your teeth. She plans for the future, by doing bad work so you can come back, and she can more money.
