We would like to extend a huge thank you to Jay in Physical Therapy for his excellent attention to detail. His diligent tracking of the additional days required for her stay ensured she could safely transfer from her bed to a wheelchair with the assistance of only one CNA. When our mother arrived a month ago, she was unable to stand or walk, and her progress has been remarkable. We were also very impressed by the medical care meeting provided for our family. It was well-organized, with up-to-date lab work and clear documentation that allowed us to easily understand her medical progress. Additionally, social workers Elvie and Ana, along with Bianca, Alejandra, and Renee from administration, were outstanding resources. They kept our family consistently informed and made the discharge process seamless. Our special gratitude goes to the housekeeping team (Gloria and Julio), the CNAs (Mayra and Alma), and the nursing staff. They kept the facility immaculately clean and welcoming, while ensuring our mother was comfortable, cared for, and able to participate in daily activities. Finally, we want to thank the dining staff for providing nutritious, high-quality meals and for being so accommodating to her dietary preferences. The rehab department itself is well organized, and put together. I would hands-down recommend this facility to anyone looking for skilled rehab. Elvie the case manager is professional and very communicative. Returning your calls immediately. She's very up-to-date on the patient's situation at all times. It's a small and everyone works as a team. From the CNA's to the nursing staff to the physical therapist, occupational therapist. They all work well together.

The facility was consistently clean and had a calm, welcoming atmosphere. It stayed organized without ever feeling chaotic, and the staff were professional, approachable, and responsive. That consistency carried across the entire facility--from the nurses, CNAs, and medical providers to social services, activities, dietary, housekeeping, and front-office staff. We received timely reminders and updates, which helped me stay on top of his care and next steps. As his primary caregiver, knowing I could reach someone when I had a question made my life easier and gave me much-needed peace of mind. Most importantly, my dad formed a genuine connection and friendship with the people caring for him. That certainly made his four-month stay feel much more personal and comfortable. PT and OT were especially important to his recovery. They were patient and encouraging while helping him rebuild his strength, mobility, confidence, and everyday abilities. They challenged him appropriately while adjusting to how he was feeling each day. After four months, his transition home was handled smoothly. I told them this was my goal. Once they got me there they put me in a diaper! After putting up with this for several days I threatened to require transfer to another care center. Then they promised to get a commode for me for my bedside. They kept promising and promising, and never did it for the 6+ weeks I was there! In addition, the first space I was assigned had a TV that would only turn off if you unplugged it. Also, their TV channels did not include KTLA, a major local channel. Hygiene was also an issue. The curtain between me and my neighbor had smear of snot or other bodily fluids on it. It took more than a week for them to change it. Later on I had a chest infection and I got some fluid on the new curtain. When I told the cleaning crew, all they did was rehang the same curtain with the side toward my neighbor (who was there for cancer treatment)! There was also a security issue. One male resident, who apparently had some form of dementia, kept wandering the halls and entering my space, claiming he was visiting a friend (no males were in my unit, and the other resident also did not want him in our unit). I told the staff and they cautioned him ("you don't want to go back to the hospital (mental?), do you?"), but he continued to poke his head into my unit. I observed them lying to residents. They took away my neighbor's wheelchair because they didn't want her wandering unaccompanied in the halls, but told her they didn't know what happened to it. They promised physical therapy four times a week but I never received more than 10 to 15 minutes two or three times a week, and when I asked for more they said staff were unavailable.

    A close family member was referred to this skilled nursing facility for rehab following a fall…read more This review is specific to the third floor for Kaiser patients. The facility is extremely busy, and nurses are frequently checking in on patients. Rooms are shared (two patients per room) and include a private bathroom, a TV for each bed, and a dresser. The facility itself is clean, though the nursing staff often seemed short-handed. During especially busy times, it was not unusual to wait 5-15 minutes for a response after pressing the call button. Physical therapy was provided once or twice per day in short sessions. Some drawbacks included physicians making care decisions without always coming to check in directly with the patient, as well as occasional miscommunication regarding medications. While some staff members appeared more short-tempered than others, the majority were friendly, attentive, and patient. We also experienced some issues with laundry, clothing went missing, and at times other patients' clothes were mistakenly given to our family member. Overall, we are grateful for the care received, but there is clear room for improvement, particularly with staff-to-patient ratios to help reduce call-button wait times and improve overall communication.

    I've tried various PTs through my insurance, but this is the first time I invested in PT out of…read morepocket, and it was absolutely worth it. I felt like my body was twisted and unbalanced, but the PTs covered by my insurance weren't able to help me until the issue I had been expressing eventually led to a severe injury where I couldn't walk for about five days. The PTs would give me exercises that I could have easily found through ChatGPT or a simple Google search, and then send me to an aide. When I had questions, the aides often didn't know the answers. They gave me A LOT of at home exercises, almost a quantity over quality, hoping one works. Then I found Dr. Gary! He is very different from a typical PT. He listens carefully to all of my issues (pain and discomfort seemingly everywhere. I'm sure I sound like a hypochondriac ), and then gives me some really odd exercises that I would never have found on my own. Somehow, I always feel better afterward. He goes all the way down to the basics, even teaching me how to breathe to properly engage my core, when to breathe during an exercise, and exactly where I should be feeling each movement. It's not just "here are some exercises, go do them." He makes sure I understand why I'm doing them and what I should be feeling. He also consistently remembers to send me my homework, often with videos. With the other PTs I tried, I had to repeatedly hunt them down to get my exercises, so I ended up having to rely on memory. Dr. Gary also responds to my questions between sessions if I'm unsure about an exercise, which has been incredibly helpful in making sure I'm doing things correctly. What I appreciate most is how much he genuinely cares about learning and thinking critically. I can tell he is constantly processing what he's seeing and what I'm telling him during our sessions and trying to figure out the root cause of what's going on. He tests and retests his hypotheses until my body gives him feedback that he's on the right track. I'm incredibly thankful for Dr. Gary and the care he puts into his work. If you're dealing with something that feels "off" in your body, I truly believe your health is worth the investment. Don't wait until the problem becomes a serious injury like I did because of the limitations of the insurance system. Take the chance! you may be surprised by how much the right PT can help. Dr. Gary is absolutely worth it!

This has been by far my best PT experience, not just in terms of addressing my injuries but also getting me back into my routine with less risk of future issues from the knowledge gained. Wendy, the therapist I worked with, listened well, responded thoughtfully, and took my very subjective descriptions of pain and turned them into objective, actionable diagnoses. Quite impressive!

The level of care, compassion, and professionalism shown by everyone at Sun Mar is truly remarkable. No matter the situation, their staff consistently go above and beyond for their patients. Like any place, there were some issues along the way, but what stood out most was how promptly and effectively they handled everything. Every issue was addressed and resolved in a timely and caring manner, and my dad's needs were always their top priority. From the nurses and physical therapists to the staff who assisted with meals, bathing, and medication, every single person played an important role in my dad's recovery. Their teamwork and dedication made such a difference.     This is a nursing facility targeting Korean patients. They serve Korean food and many activities…read moreconducted in Korean.. The rooms are average but you can't beat the service. The staff are friendly, helpful and hardworking. Even the non-Korean staff have learned to speak Korean, which is impressive! My father has been staying here for over 4 weeks and my family knows that he's in good hands.

Being my first time in treatment, I didn't know what to expect or what I had committed myself to but, Lighthouse was there for me every step of the way. From detox to completion of treatment, I received nothing but love, support and respect being as broken as I was. They treated me with compassion, understanding and operated within God's grace while taking care of me every step of the way. Through their help and programming, I was able to not just rehabilitate my health. They helped to rehabilitate my mind. They were able to do for me in 3 months what I was incapable of doing for myself. Because of Lighthouse, I have my life back. My marriage is back on track, my kids have their father back but most importantly I restored my relationship with God during my stay. I couldn't have done this without their help or support. I was failing on my own. Lighthouse made it so that I was able to focus on healing myself from the inside out. Thank you so much for church, gym, yoga, offsite meetings and all the extras like game night and movie night. It was the first time I had done (or was able to do) any of those things sober in YEARS. Thank you so much for the clinical groups, motivational speakers and panels.     4/5 Stars -- Helpful Program, Some Room for Improvement…read more I recently completed a few weeks of treatment at The Lighthouse, and overall, I had a positive experience. The program itself was very helpful, and I met a lot of genuinely good people along the way. The staff were also a major positive. Ronnie, Rob, Tim, Stephanie, Jenna, and several others were supportive, helpful, and made the experience better. That said, I do think there are a few things prospective clients should know going in. The living conditions aren't necessarily dirty, but they feel a little more like a cheap motel than a treatment facility and could definitely use some renovation and TLC. I also felt somewhat misled about the living arrangements and phone privileges. Once you're settled in, phone access is limited to about two hours a day, and you live in a house with 4-5 other people, with three bedrooms and two people per room. Despite those drawbacks, I'm grateful for my experience and for the people I met. The treatment and support I received were genuinely helpful, and I would still give The Lighthouse 4 out of 5 stars overall.
