We were happy with the care she received, that's why we brought her back again. First of all, it's a very clean facility, small and personal, but most of all great staff. The nurses my Mom had were professional & helpful. The CNA's were amazing. The ones I will mention were the very best. Mario, Smile, Rain, and one special CNA on swing shift named Michelle. Jessica on the weekends was also an excellent help with my Mom, Nursing director Teri was extremely helpful, always available to help. Vanessa social worker was also fantastic at assisting with so much. The physical therapists & occupational therapists were also great. I hope my Mom never has to go back, but if she ever did, Legacy would surely be our number 1 pick. Much love & thanks to the staff. We are forever grateful for their kind & wonderful care of our Mother. Thank you so much!

I would…read morecome to see my mother after lunch to find that she didn't have her hearing aid in her ear and her dentures were still in the case. which had dirty water in it. I had to complain to severl people before anyone would do anything about the moldy water they were storing her dentures in. Very few people here care. Good luck trying to get someone on the phone "They're in a meeting and will call you back as soon as it's over" was the standare lie I was told. There was no set level of care at which my mother would be released so as long as they could bill for her she was stuck in a hostage situation. My mother's health declined horribly while here, she lost a lot of weight and she died a month after they FINALLY agreed to release her.

    I want to express my gratitude for the excellent care at Pleasant Hill Post-Acute Care. I was…read moreadmitted with a fractured pelvis and could not walk. Thanks to the entire staff's exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! I have never had PT before and was amazed how their simple instructions and encouragement helped heal my body so that I could return home. There are too many other names to mention here, but the following are a few. I'd like to give a shout out to the nursing staff, particularly Dahlia. The nurses were so kind and patient, helping me with every need, including cleansing. I hope that I was kind and respectful, but I noticed that some patients could be quite difficult. The nurses and CNAs consistently maintained a calm, caring and professional demeanor. They catered to every need and clearly cared about the patients' well-being. Claire, the Activities Director delivered daily menus and a list of scheduled activities. What a sweet individual, dropping by each morning with a smile and a little conversation. Nicole of the Housekeeping Staff was lovely. I engaged her in conversation at every opportunity, as she made sure that each room was clean and in proper order for each patient. My case worker, Mary, was instrumental in working with doctors and staff to secure my release date. She also recommended an appt. with my PCP and referred me to out-patient PT care. Another shout out is to the kitchen staff. The meals were healthy and delicious and tailored to each patient's needs. Mistakes were sometimes made and were quickly rectified. One small concern is that breakfasts always arrived cold. Having worked in catering, I know it is difficult to serve a hot meal to 50 people at once, but it can be done. Icing on the cake: Greta, an optometrist visited some of the patients for eye exams and glaucoma tests. I did not expect this extra service and appreciate it so much! In additional, the facility has followed up with me to check on my healing progress. I hope none of you require such intense care, but if you do, I highly recommend PH Post-Acute Care to administer to both physical and emotional needs.

I stayed there for three months, and during that time I experienced nothing but care and support from the staff. The nurses and CNAs are attentive, friendly, and truly caring. I especially want to thank Marth and Arryan--they are so sweet and genuinely care about their patients. The night nurses were excellent as well. I'd also like to thank the CNAs Melissa, Beth, and of course Dalia. They were helpful, kind, and attentive. They took such great care of me and made me feel safe during my time away from home. Physical therapy and occupational therapy were also great throughout my recovery. They helped me from the very beginning to regain my mobility and strength. The therapists Jordan and Justin were fun, funny, and kind throughout my sessions. They made each visit engaging and always took the time to listen to my needs. They consistently found ways to challenge me while keeping the sessions enjoyable, and they helped me grow stronger every day. I truly appreciated their support and encouragement. My case manager, Bianca, was amazing throughout the whole process. She was always available whenever I had questions or needed someone to talk to. We definitely had our moments--lol--but she was truly supportive and helpful every step of the way. I also want to thank Claire, the activity director. She was so lovely and caring, and she truly tried to make every day fun and different for all the patients. She recognized each patient's unique qualities and worked hard to make their time at the facility as enjoyable as possible. She is truly caring and loving yet professional as well. She is a skilled nurse so our family felt very confident regarding the physical well being of our mother. We felt blessed to find 7th Heaven as it is a warm, caring, loving place and as close to being at one's home as we could find. Olivia made sure mom had healthy, tasty food that she liked to eat and kept her clean and comfortable. She also makes sure the residents don't just stay in their rooms or beds but has them in the living room which is open to the kitchen so it is a very inviting environment. As I mentioned, Olivia does all this with such care, love and grace. We are forever grateful for Olivia and such a wonderful place.

    Funny I just read the review recently posted by another person who coincidentally has same name as…read moremy mother, who Olivia cared for during the last 6 or 7 months of her life as well. Well into her 90's, my mom was a bright, cheerful and intelligent woman. Her last couple years were characterized by the typical and in her case rapid diminishing of her ability to communicate and function as an adult. In spite of having no prior history with her, and my mom's increasing irrationality and at times downright stubbornness, Olivia cared for her like a loving daughter would her own mom, allowing us to see her progress to her recent departure with dignity and comfort. It gave us tremendous peace of mind to know that, unlike the facility we moved her out of to be at 7th Heaven, our mother got the best care anyone could hope for, both physically and emotionally. I and my siblings cannot recommend her highly enough, and thank God there are alternatives like 7th Heaven around (and they are not all alike!!)

I googled Hospice and came across MD Choice that had some good reviews. I had no idea who I was calling but there had to have been some divine intervention pushing me to this call and this company. After a few questions I received a call from Dr De Leon who was the most compassionate and caring doctor I have come across. He sensed my immediate need, came straight out to the house, assessed my mother and went himself to the pharmacy to pick up the needed medications. He knew the urgency and took it upon himself to do all the footwork. He obtained the needed comfort medication, stabilized my mother and allowed my mother to pass peacefully all within a matter of hours from making that initial call. When she did pass that evening, he again, arranged for everything from the pronouncing to the calls according to the pre-arrangements. Without MD Choice and the connection with Dr De Leon, I don't know how I would have been able to handle the situation. His immediate response on a Sunday, his Taking control of the situation and quick actions made such a difference in the final few hours my mother had. If you find yourself in a situation like this and needing someone to help in this time, I highly recommend MD Choice and Dr De Leon. I am forever grateful to MD Choice and Dr De Leon for acting so quickly and with such caring and love in this terrible time of need. Not only were they able to provide significantly more daily visits from their team of nurses and aids than the other options we considered, but they were incredibly responsive, efficient, and helpful with every question or request we had--and that includes their 24/7 on-call team. Cheryl, the RN Case Manager, in particular, was a godsend. It was a huge relief to be able to rely on them so thoroughly from day one. They delivered everything we needed within hours of getting the paperwork done; and both Cheryl and an aide were there for the last few hours my brother was alive--providing support and care. We really can't recommend them highly enough!

Cell phone use is allowed and encouraged, so you never feel too disconnected from home and can effectively take care of business (it isn't realistic to be disappearing from the world for 30+ days). Tons of fantastic healthy snack options, along with the unprecedented option to have a small personal stash of your favorite snacks (again, helping me keep that connection with home). Amazing counselors like mine, Eddie and Joey, get you through the day-to-day. Every person there, from the Program Director to our "sober fun" escorts, short of the cook, is a certified counselor in recovery (at minimum, if not an LMFT); and is a fabulous sounding board and source of strength for your daily struggles in early recovery. And the cook is fabulous (man, did I eat well there)!!! You can tell everyone there, right down to said cook, really cares about the clients while they are there and beyond. AVR's program includes a wide breadth of education and therapy including: one-on-one sessions with an LMFT, family/couples therapy with an LMFT, tons of counseling staff onsite at any given time, public outings in the real world with various recovery meeting formats and "sober fun" events, CBT (how to handle those nasty thoughts), DBT (how to handle those rough emotions), traditional Process group therapy (get your gears grinding and open your eyes to reality and life), relapse prevention, anger management, self-esteem, top-rated-recovery-literature study groups, transition planning and assistance, and even alternative/eastern medicine therapy like Qui Gong Tai Chi, yoga, acupuncture, and eco-therapy. There is even regular visits from a certified personal trainer to make the most of the fantastic gym/exercise equipment! The environment is bar-none. The grounds are huge and beautiful for taking a walk or exercising. A wonderful garden to work in if you like. The house is spacious with multiple TVs for downtime. Along with board games, a pool table, a basketball hoop, a pool, and a hot tub. Lots of extra space for personal time and personal calls. And no more than 6 clients at any one time. Don't get me wrong. Recovery is hard. Lots of emotions, realizations, and work to be done. But the environment is incredibly comfortable and fosters personal growth. I spent 60 days there, and they flew by. Between the staff and the environment, AVR makes all that hard work as painless as possible. If you want to not only sober up, but get the tools to stay sober and enjoy the rest of your life with your family, this is the place to go for residential treatment.

I was still in my addiction when the last review was left and was not fair in my assessment. This place is worth looking at as an option to begin a journey in recovery.

It was very comforting, to know I had this team of professional individuals helping me move forward in healing. The nurses and the CNA's were outstanding, meeting my every need as per my Doctors orders and requests. Physical therapy, added encouragement to help improve. My physical condition was a challenge. I was incapable of taking care of myself during my healing process, and always felt that the staff was eager to encourage and cheer me to do better. The service for all meals impressed me, with their timely efforts to serve appealing meals. helping in any way they could were on time. I witnessed the staff encourage patients to reach goals, such as standing, walking, and even taking the time to read, bath and in one case help feed my roommate. There were many patients at the hospital that were in critical condition getting constant attention to their needs. The condition of the facilities was very welcoming,, clean and orderly. Office personnel, Maggie and Monet, were beyond considerate, kind and helpful. I would not hesitate to recommend this facility.

    This facility is extremely disorganized, and I am deeply concerned about the level of care my…read moremother was receiving. She was admitted for rehabilitation following a stroke and has specific medical instructions that are not being properly followed. She is supposed to have thickened liquids without the use of straws to prevent aspiration; however, even after having a meeting with their section managers about my mom's needs, the staff were still giving her straws. Additionally, she requires assistance with eating, yet staff routinely drop off her meal tray and leave without ensuring she is able to eat or drink. When I visited, I can see her tray of food is untouched on the table that is not even in reach of my mom. She is not supposed to get up without assistance. There appears to be little to no concern for whether she is receiving adequate nutrition or hydration. During day 3 of my mom's stay (same day as issue above), I found my mother shivering uncontrollably and unable to speak. After I alerted staff, they checked her temperature and found it to be 104°F. She was given Tylenol and an IV - the nurse did what she could. A couple of hours later, I requested that her temperature be checked again. The assistant used a no-touch thermometer but quickly moved it around her face and neck, then stated her temperature was 97°F. This seemed highly unlikely given how recently she had a high fever. After the assistant left, I checked my mother myself and found that she still felt extremely hot. I asked the assistant to return and recheck her temperature. It appeared he was not allowing the thermometer enough time to register properly, as he was moving it too quickly. When I asked him to hold it in one place where she felt hottest, he then reported a temperature of 100°F (I did not see the exact number, but the device indicated a red light - a fever). Overall, the staff demonstrate a lack of proper training, coordination, and communication. There are cork boards in patient rooms that could be used to clearly display care instructions, yet they are not being utilized (photo attached). This would be a simple but effective way to ensure consistency in patient care. Her inability to eat and drink independently, along with her shivering and high fever, went unnoticed by staff until we brought it to their attention. At the end of this experience, we requested that staff call an ambulance to transport my mother to the hospital, as we no longer felt she was safe in this facility. I will not be bringing her back. At the hospital, it was discovered that her urine was amber in color and she was diagnosed with a UTI. As for the facility itself, the environment is outdated and poorly maintained. The air circulation is inadequate, and the overall atmosphere feels depressing. Improvements in both the physical environment and staff performance are urgently needed.

I have noticed it's either bad reviews, slim good ones or none at all. We need to change that. Reviews aren't that hard to do and honestly help people when they are faced with picking a company to care for their family members. You are the one that truly knows how the company treated your concerns and the care they provided) Establishing care: 5 star My mother was released from a rehab center so they took care of assigning the company care (made it very easy Onboarding: 4 star My mother was released from rehab on June 30th and had her onboarding done on July 3rd. During onboarding they just go over paperwork and tell you your rights and an overview of care. Office staff: 1 star Over all company: 1 star On several occasions we had to call into the office and the service or response was not proactive or helpful, it often had us confused and left with days and weeks of no care or answered questions. For example:: -after onboarding my mother went over a week without therapy or visits (yes we understand it was a holiday but some type of communication would have been nice) -we had to reach out to them when therapist were no shows and they had no urgency -my mother switched health care providers so they discharged my mother and had us wait a full month to restart care after revaluation -they never knew who my mother was when we would call them (resulting in promised call backs or sending me to other people) -when we would call they would say things like oh it's you again, or we already know (which instead they could have called and kept us updated rather then making us wait and call) -they were unprofessional when it came to knowing their staff names (upon asking us who our therapists were they told us they did not work there or we had the wrong names - make it make sense?!) -they discharged my mom without notifying us Physical therapist: 4 star We had two different therapists Micheal was our assigned therapist and Xue was the one that would come to check in with progress Micheal 5 star Really good and friendly. He clearly knew what he was doing and was able to push and motivate my mother to get up and walk back and forth in the house. Xue 4 star He was nice but always seemed to rush around things (spent most of his time on his tablet) Social worker: 1 star Denise was horrible, she came a day early for service and clearly didn't know what she was doing. I informed her that we had physical therapy that day, however she still came for a 5 minute visit to ask if we had any questions. Our physical therapist came and she had to ask him how to use the tablet, meanwhile she bashed the company and lack of onboarding. She was scheduled to come visit again in 2 weeks. Instead of coming she called to say that she felt like we were "smart" enough to do everything on our own so she was going to discharge us. Home health aid: - 0 stars They apparently only have 1 home health aid so we never had a visit and don't even know if they put us on the list because they brushed us off every time we asked. Nurse: 3 star We had several nurses come by (about 5 total) only remember a few of their names Maritot & Darius: 1 star Beatrice: 4 star All nurses were subpar, they come to check vitals and update the doctors of any new or worsening conditions. All nurses stayed for about 5 to 10 minutes maximum. Always rising thru visits. With the exception of Beatrice, though she was always late (reason for -1 star) she was so attentive and actually seemed to care, each of her visits she oils talk with my mother, make her laugh and give her tips to get better. She would rub massaging rub on my mom if she was complaining of being sore. She would have her stand and ask her to walk to know her progress. She would stay her whole slotted 30 minutes. You could tell the at she liked her job and wanted to care for as well as see her patients get better. Occupational therapist: 4 star Apparently our area only had one occupational therapist so we would only get 1 visit a week, even though our doctor ordered 2 a week Abby 5 star She was great! She asked what my moms goals were and worked towards that. Mostly working in the independence of getting up and transferring herself as well as working her arms and eyes. While my mom was in the hospital she still reached out and checked in. Speech therapist: 1 star I don't remember her name, she visited 2 times and both times she did nothing but go over strategies (not actively) by reading them off of a page **this company does have different therapists and nurses come based on where you live, we live in Hayward and these were some of the people that we would get**
