Allegiant Healthcare, Plaza Healthcare and the other seven places he stayed in were understaffed and uncaring; especially Plaza Healthcare. He went from 16 hours of breathing on his own at Banner downtown acute care (kicked out at 100 days because he was getting better AND Medicare refused to pay) to one hour *ONE HOUR!!!* in only 29 short days later at Plaza Healthcare! Disgusting! ALL of these SNF's are horrible in AZ. After 10 1/2 long months of being gone from home, fighting for his life from Covid, he died. After my mom's stroke and brain bleed she was brought to this facility for memory care and rehab. I seriously wish I had taken photos to show what was taking place. The first evening my mom was there I walked in and she was laying on a plastic mattress with no sheets on the FLOOR!!! I demanded they raise the bed, put some rails on it and put sheets on it. They said they couldn't find the sheets to fit it! I told them to bring in a different bed then that had sheets that fit. The following day the in charge nurse told my father, in front of my mother that this was the best it was going to get. She would not be getting any better. At this point my mom couldn't speak, couldn't get up and walk, but knew what you were saying. It was the first time since her stroke and brain surgery that she cried. She knew what this nurse was saying. I was furious. I got my mom up, had her stand while I held on to her and we would just hug and sway back and forth to get weight on her legs and get her muscles working again. (Isn't this what a rehab hospital should be doing???). Within 3 days, my mom was able to walk to the restroom and use it. Now she didn't have to lay in her waste for HOURS waiting for someone to come change her. Within a week, they had a hard time keeping her down. She began talking again. Although still very confused with memories, she was improving greatly. That nurse tried to make me feel stupid like I had no idea and because my mom had had a seizure, she was now going to live an infantile existence for the rest of her life. If it was up to them, she would. They were doing absolutely ZERO memory care with her and occasional pt. I was horrified at what was going on. I had to throw fits to get her bathed. I was constantly being told they were understaffed or the 'shower girl' didn't show up. Today, almost a year later, my mom has some memory issues and dementia, but she is home and functioning pretty darn well! Especially for someone who was never going to get better! The picture is of my mom the other day, riding her bike. I've thought of taking it into the nurse and telling her don't you dare EVER tell a patient or their loved one this is the best it gets and she won't get any better. read more

I worked here as a CNA. I really did like the position. I liked the people I got to help care for and started to know them well. The job got easier as you know and learn about the people you care for and know their needs. Don't get me wrong, I felt like I was chasing my tail every day I was there but that is the case anywhere. I was PRN, worked as needed. I requested the 12 hour shifts when I wasn't busy with my main self employed job. Sometimes I worked so much I had overtime. I really did enjoy it. I had a lot of compassion for the residents and enjoyed being there. Unfortunately, the center sold and had a new owner. I had issues with my paycheck 3 pay periods in a row. From check bouncing, direct deposit didn't go through and having to put the pay I was shorted on the next paycheck then again on another future paycheck. So, I stopped requesting shifts because I wasn't being paid. I didn't request anything for a few weeks. I contacted the scheduler asking if they had openings and I was told I no longer worked there and I was not eligible for rehire. I told management why I didn't request and they said their decision was final. I told them I really liked Allegiant and I didn't want to leave. They said their decision was final. I think it was a loss for both of us. If I would have got paid as scheduled things would have been much smoother. I was sad to loose my PRN position here. I felt a loss when I was told that I was separated from employment. I miss the patients. read more

Be very careful of this place. DO NOT PUT YOUR LOVED ONES HERE. My older brother had an accident 6 years ago. Due an insurance issue he wound up in this facility. If Jennifer the so-called social worker who has a title but not the degree had done her job. 6 years ago. My bother would have received his wife's death benefits from SS. and disability. How do I know this, After my brother called 911 himself to get help Dec 21 Aligiance night team did not help him. He was in the hospital with gangrene of his gallbladder and septic. He had an infection for three months prior to calling 911, he kept calling me saying how sick he was and in pain. I raced from LA to Mesa. Banner hospital was great we almost lost him twice. the hospital told me to go the SS office in person. I met with the head of the SS office. In thirty minutes, we got his SS, we got his Medicare. And yes he was eligible for disability 6 years prior. They had sent a letter to Jennifer she never replied to it. He also was eligible for his wife's SS death benefits. Jennifer never followed through. Why they wanted him to stay low income. Allegiant gets more money this way. Over the years they actually broke his knee cap and hip. He is bedridden, they put him in machines that were not properly calibrated. HIS KNEE CAP SHATTERED. If Jennifer had done her job. My brother could have gotten out of this place years ago. He could have had proper therapy. We are trying to get him out of his as soon as we can. DO NOT DO NOT put your loved ones in this horrible place. read more

I really wish i could give negative stars. DO NOT LEAVE YOUR LOVED ONES HERE!! they dont care about their well being. they only care to profit off of them. they dont give them the proper medication or antibiotics, and PT!? ha forget about any physical therapy. Extremely rude staff and none of them know anything. It also takes a nurse 2 hours to get to the patient after the buzzer is pushed. And then seem annoyed once they finally get to the room. Horrible. I had the misfortune of being at Allegiant Skill Healthcare of Mesa. We had to select an assisted center very quickly in ordered to leave the hospital. I was taken by shuttle to this center and we were greeted rather rudely by a receptionist. The room for me was not ready and many items were not in the room. I would NOT recommend this facility to anyone needing medical issues because the staff is very unprofessional and lacks necessary training. The food was always cold and several times not what the menu stated. The case managers were not willing to assist me in getting to another facility until I stated I would call the state department of Arizona. read more

I should have suspected the worst when the ambulance driver who transported my mother back to the hospital told me," don't put her back here." There's so much I could write. I am a nurse practitioner and I know substandard care. The food was horrible. My mother developed MERSA there and there were other cases. Absolutely no isolation technique for 5 days... I was told they were out of gowns. The only rehab for two weeks was sitting mom on the side of the bed. Never a Physical Therapist- only an assistant. One cried because she was supposed to get my mom up with no help. Mom had had a complicated graft with complex dressing changes. I witnessed a wound nurse drop a dressing on the floor and attempt to then use it on my mom's wound. Her technique in the dressing change was deplorable. I went in every evening. A meal tray would be in her room untouched because she couldn't reach it. Large area of dried urine under her, often.... Urinary catheter left in days after it was to be removed. I moved her to Health West which was wonderful then I brought her home. Twice she was sent for appointments to the Maricopa burn center with no accompaniment. Ambulance dropped her off and she was left - no one knew she was there. I was to meet her there for the appointment. We had to search for her. Medical care was non-existent. Because of their lack of proper wound care, my mom had to go back for surgery on that graft. She was 94-years-old. If I had not gone in every night, she would have died there! They have done what I understand is typical. They just change their name when there are too many complaints. And they did, from Symphony. My heart breaks for the patients there who had no advocate. read more

I was admitted here after my surgery for 2 1/2 weeks. I was pleased with everyone on their staff: CNAs, nurses, and physical and occupational therapists. Almost every time you pushed the call button they came in very quickly. During these Covid times they were very careful about keeping everyone safe. When I did mention I had a doctor appointment they were hesitant but I added that it was to see the orthopedic surgeon to get X-rays and remove stitches. They arranged for transportation. If I had to go again I would request to go here again. read more

They were awful. do not send your loved one here. they are understaffed and therefore overworked. My mom has a trach tube and needed constant care and attention. she was placed in the very back of the place and the furthest from the nurses station. so when she was gasping for air, no one would hear her. I was there a lot to make sure she was being taken care of. but the one day I missed, she died. I am very sure that she had a clog of mucus in her trach tube, since ive seen it before, and couldn't breathe! and she didn't get help in time. she passed on the way to the hospital. read more

Allegiant Healthcare of Mesa, formerly known as Symphony, is not a place to send your loved one to for care. Case Management does not include family in weekly meetings or progress reports, nor does the Physical Therapy Department. They take it upon themselves to decide whether or not an outside appointment is necessary or not, without consulting family.- An appointment had been made for my mom that took 6 weeks to get before mom had entered Allegiant. I notified Allegiant of this appointment on 05/31, and received confirmation from Allegiant that same day that they would a schedule transportation for the day of the appointment, 06/20. On 06/21, Mom called advising that transportation never came, and upon inquiry was advised that she was too close to discharge- 8 days away on 06/28- therefore, they decided not to take her. Not only was family not notified to be given the chance to take her, but also the doctor was not called to avoid a 'no show' charge. Myself and my family are highly disappointed in Allegiant Healthcare of Mesa. read more

This Skilled Nursing Facility has 1. Changed names numerous times most likely for infractions with the state and licensing reasons no basis however 2. Installed a revolving door pertaining to employees and Skilled professionals 3. Lies, botches documents to skirt responsibility and accountability 4. Under staff meaning ratio of patients to skilled professionals even entry level hopefuls (non skilled) is alarming. I at one point was in a motor cycle accident and had the misfortune of being placed in this facility. I have first hand experience of this low security high oversight mismanaged bummer. Please be warned! Director of nursing the administrations top 6 and all that are the so called decision makers to my knowledge stay the same through name changes and licensing approvals. Do not be deceived. The shots they call are pitiful, callus and degrading to those practically helpless. Please, if you need to know anything be diligent with your own care or those of loved ones after all it is your body and your overall well being that maters most. I believe there are wonderful people at this facility MOST sad of all they are scared of bosses and those in power to even do the right thing " to say something" and most importantly "do no harm". Makes me very worried for those that cant speak for themselves! read more

My mom was a patient at Symphony of Mesa. She was on dialysis, had heart problems, blind, and an amputee wheel chair bound. They had moved her from the westwing from breathing monitors to the eastwing with no monitors. From the time she was admitted I never seen her doctor that was treating her there. I always requested to see the dr but I always got the run around. "Oh they only come early in the morning" so I would come early and wait, no dr. I would come everyday knowing she was blind and wheel chair bound. Seeing that they were always short staffed. I was always worried about my mom not getting the care. I always came in the morning to help get her ready for out bound dialysis. One day my dad went to sit with her he called me said to get over fast he said nurse said something is wrong. I get there paramedics where working on her. She ended up passing away they couldn't revive her. Our family don't know how long she was like that. We don't know when they had checked on her. Or if they did at all before my dad went to visit her. I would NEVER recommend this place or anything associated with Symphony of Mesa. If you love your loved ones DO NOT send them there. I regret it til this day!!! I miss my mom everyday asking her to forgive me for putting her in a place like this. I would really like to know how many other families out there has lost a loved one at Symphony of Mesa on near Broadway and Lindsey crossroads. I know they keep changing the names. If you want your loved one to get the worst care in the fanciest looking facility they can get, Symphony is the place for you. They advertise individualized care but they don't give even the most basic of individualized care. They gave my father regular meals instead of placing him on a diabetic meal plan or the renal diet to help him recover. They couldn't get it right for nearly TWO WEEKS!!! They don't let you talk to your family member's doctor and the one person they do "let " you talk to avoids patients, sneaking past them even when family have left notes on top of their purse to please stop by before she leaves. You have to do everything through the charge nurse who is really a glorified public relations representative, telling you what you want to hear. The departments don't communicate with each other effectively and the facility is run on appearances and they falsely advertise how great their services are. My father's old roommate and 2 other resident's told us horror stories about the lack of care and the poor way the hospital is run. The final straw is the fact that he got a bacterial infection in that place and not only did it take them days to diagnose it even though he had many symptoms, when they did finally send him to a hospital (a family friend who went to visit him insisted he be checked and when they did, THAT'S when they decided to send him to the ER. When we went back to get things for the evening, the nurse with the worst bedside manner in the place told us to take all his things because they were going to move him and his things out of the room (how and why would she know/say that a few hours later) and some of his things may come up missing. I know there are many options and their in house dialysis is attractive to those who need it but out-patient dialysis is an option at a facility that doesn't just tell you what you want to hear. The care there is just not proactive. It is simply storage, not rehab. If you stay on the staff constantly asking questions then your care is better but they tend tell you they'll look into it or whatever they think you want to hear or they to hide from you when you are the proactive type. I agree with Jennifer T, Mom was stuck in the back, she had a colostomy and if I had not been there every day I do not think it would have been changed or cleaned. I constantly had to remind them. Her hands were constantly bleeding from multiple blood test for diabetes which she did not have (the opposite,, Mom was hypoglicemic) I called them when I finally saw the list, they confirmed she had never tested positive for even a tendency towards diabetes. They basically kicked her out for "lack of progress" she was there less than 3 weeks. Her discharge was awful, they were supposed to call me when the transfer took place so that I could meet her at the group home. They did not and she was in a panic when I finally got there. She was sent with a full colostomy bag and no supplies to change it. She had a horrible diaper rash and a long list of medications which had never been discussed with me and were not needed. Her feeding tube was left in her stomach without any cap or even a bandage. read more
