A mistake I will never forgive myself for .... She arrived there May 23 and by June 15 I no longer had my mother . The absolute worst I've ever seen in my entire life. I do home care for for a living and could never treat a human the way they do . Most of the staff deserve to rot alone one day . Like you treated my mother with me even being present multiple times a day everyday you still couldn't stop them...... unanswered calls, call lights, unanswered questions never had a plan never knew when my mother's wounds were gonna get changed. I wish I could give this facility zero stars. The care my grandmother received here was deeply concerning and ultimately devastating for our family. She was only a resident for 12 days, and during that time my mother visited multiple times every single day--morning, afternoon, and often again in the evening. What she witnessed was alarming. My grandmother was not receiving the wound care that had been specifically ordered by her surgeon and physicians, and her ostomy bag was not being properly cleaned or maintained. My mother repeatedly raised these concerns with staff on a daily basis, yet there seemed to be little urgency or accountability. Despite our family's efforts to advocate for her, my grandmother's condition continued to deteriorate under their care. The most heartbreaking part is what happened next. My grandmother began hemorrhaging and was found around 5:30 a.m. Our family was never told that staff had waited approximately two hours before deciding to contact emergency personnel. We only learned about this delay after another staff member privately pulled my mother aside and informed her of what had occurred. The fact that such a critical delay was not disclosed to our family only adds to our concerns about the level of care and transparency provided by this facility. By the time she was transferred to the hospital, she required ICU-level care. Tragically, she passed away. Our family firmly believes that proper care, timely intervention, and adherence to her physicians' orders could have made a significant difference. Instead, we lost a beloved grandmother whose life mattered and deserved better. Because of the seriousness of these concerns, we have filed a complaint with the state and requested an investigation into this facility's practices. I would never recommend this facility to anyone. If you are considering placing a loved one here, I strongly encourage you to thoroughly research all available options. No family should have to experience the pain, heartbreak, and loss that ours has endured. read more

Mom wasn't there long enough but the little time we were there we could see the care she needed would not be sufficient. We had to make the decision to get her back to the hospital. It's 24 hours to see the Nurse Practitioner, needed Iv fluids had to order 4-5 hours waiting , blood draw 2 hours as for the place it smelled clean enough but could use a makeover. Parking is terrible. Mid afternoon they are playing loud and I mean loud Mexican music down the hallway giving out ice cream in Dixie cups when you are feeling poorly very annoying. Under a different scenario I m down for the music and margaritas. But not when you're going down the hallway with a boom box the size of a suitcase. Can't comment on the food. Brief meeting with PT and OT seems to be good. read more

I'd give them zero if possible. We placed my father, who has Alzheimer's disease, at Apache Junction Health Center for a 30-day respite stay while my mother and I recovered from surgeries in January 2026. Unfortunately, our experience was deeply concerning. A formal complaint has been made with ADHS and is under investigation. Despite providing all of his medications upon admission (including detailed instructions), we later discovered that one of his prescribed antidepressants (Sertraline) was never administered during his stay. Multiple staff members contacted us about his severe depression and uncontrollable crying, even suggesting adding new medications--without us realizing he wasn't receiving the one he was already prescribed. Communication was poor. We were told we'd receive follow-up calls from nursing staff and the facility physician, but those calls never came. This left us making critical decisions without accurate information. By the time he was discharged, his mental state had significantly declined and he is still not recovered from this. He was far more withdrawn and cognitively worse than when he arrived. To make matters worse, several of his personal belongings--including hearing aids, eyeglasses, and clothing--were lost. His discharge papers indicated he had no hearing loss/aids, yet the staff admitted they could not locate them. Incredibly frustrating for my dad. The facility was unresponsive in addressing our concerns about my father's medication after discharge. While I was able to get my dad to an audiologist in early February, the VA was not able to provide new hearing aids until mid-March, so my father was affected for about 10 weeks. If you know anything about dementia and hearing, you understand how traumatic this was for all of us. We would strongly urge other families to carefully consider their options before placing a loved one here. I would NEVER recommend this facility. The guilt of placing a loved one with someone beyond family is tough enough, so to have endured the lack of care with this VA-recommended facility is heartbreaking. read more

We just endured 4 days of absolute hell at the Apache Junction Health Center before getting my mom transferred back to the hospital--thank God. She was sent there with external fixators on her leg and didn't receive pain medication for 12 hours at one point. A doctor had ordered blood thinners to prevent clots before surgery, and she only received one dose the entire time she was there. That is unacceptable and dangerous. Her room--shared with another patient--had a broken air conditioner in the middle of Arizona heat. A work order was placed, but no one ever came. Just like so many of the patient call lights that were constantly going off and ignored. I witnessed people being left with full bedpans for 30 minutes or longer, and patients waiting upwards of an hour for nurse assistance. To make matters worse, the only fire alarm on that hallway had an "Out of Order" sign on it. How is that even legal? This facility is not just understaffed--it's unfit to care for vulnerable people. If you have a loved one here, watch them closely. This place shows a shocking lack of empathy, urgency, and basic human care. My mom's condition declined rapidly in just four days because of their negligence. Don't let your family go through what we did. read more

If you really want to make me angry, insist that I didn't experience what I've just told you I experienced. My mom went there for rehab after a fall with a dislocated shoulder. I observed CNAs pulling on her hurt shoulder during transfers, and I was discussing with a nurse that I found that behavior concerning. The nurse said, "They don't do that." We went round and round for a few minutes until I finally just said, "Okay." I SAW THEM DO IT!!! DO NOT tell me I DIDN'T see what I SAW. You ask for a pain pill, and they say they'll be there "in a few minutes," but it's about an hour later. I guess "a few minutes" means something different to everyone, but it's really cruel to make someone who's suffering wait for an hour for a pain pill. Some of the food is good, but most of it is really disgusting. Patients are constantly screaming for help because the nurses ignore the call button. My mom was released today, and I was horrified to discover that the minor bedsore she had when she got there has turned into a FIVE INCH CRATER! NOBODY there EVER told us it had gotten that bad, and I'd like to know just what the HELL they did to make it that much WORSE than it was when she got there. I also don't know what they did for physical therapy, but it sure as hell wasn't getting her walking very much because when I got her home, it was really difficult for her to walk, and she was afraid of falling, even with her using a hemi-walker and me supporting her. Also, they didn't give me any medication or dressing for the bedsore, so I don't know how I'm supposed to take care of it without the proper tools. Let's face facts here: this place is an oubliette. If you hate your family members, send them here because the staff will ignore and abandon them at best, and torture them at worst. If I had only known it was going to be this bad, I never would have put my mother through this horrible experience. She wanted to leave from the very first day, but as others have said, they threaten you that your insurance won't pay for the stay if you leave against medical advice, so you feel forced to remain. read more

What could improve was not lose patients belongings such as clothing and HEARING AIDS. All the CNAs were good, some better than others but they all seemed to care for the people on the ward. Nurses were a different story. Only 1 of the nurses seemed to care. I believe Sheltyl was her name. Lady with a beautiful accent. Physical therapy was great. Food was bad and was not designed with patients in mind. My husband hates egg and will not eat them. He got eggs every morning and sometimes at dinner. read more

Care is not great, I've heard of places that are worse. My mother in law was there for just under 2 weeks. They put her in a diaper so they didn't have to help her to the bathroom. The last 2 days she was soaked when we got there. She did her therapy so she could come home after a short hospital stay. My grandma has been to Apache Junction Health Center twice now, and both times the care she received has been wonderful. The rehab team really helped her regain her strength, and we saw noticeable improvement both times she stayed. The staff is attentive, kind, and always willing to help. It gave our family peace of mind knowing she was in good hands. They have a big courtyard, it's such a peaceful place for visits and fresh air. The residents here have to beg for basic care. CNA are all foreigners. Stick together and push everyone else out. Management is ineffective and ignore complaints filed from residents. Food is served cold and inedible. Residents are left in their room for hours without care. I highly recommend finding somewhere else to take your family member. read more

I usually don't leave reviews, but I feel there aren't enough to do justice on here. Both my Grandma and Grandpa stayed here. They both had a different nurse and therapist for rehab after their car crash. Not only were the therapist amazing, but so were the nurses and the entire staff. It wasn't home, but everyone did their best to make them feel as comfortable as possible. The facility is older, but it is kept clean and has a lot to offer. I hope my family doesn't have to go back to rehab, but if they do, this is where I'm sending them. read more

This place sucks, I was there for Iv antibiotics & they didnt order medication, I had to remind them to give me iv, the food sucked, It was always too salty to eat. I had issues with sexual harrassment & they did nothing about it, They tried to confine me to my room. Even the dr told me to bring in my own medications to take. I didnt trust them since they would lose my medications. The nurse even told me I didnt have insurance since they forgot to get proir auth for my medications. I did have a great cna named Vangie. We spoke to other patients and family members there and they had nothing but great things to say about this place. We realized that people will leave not so good things when things dont go their way. Ive been here since August with a broken right leg and the care that Dr Chun has given me sucks!!!! Dr Chun has neglected my care and tiday would not release me and told me that if I tried to go AMA they would call the cops on me. I'm had Pneumonia, low oxygen, rashes on my bottom, back and between my legs. I've picked up all of this since I've been here.So not a sterile environment. So I would not trust them to take care of your family, friends and loved ones.Take them where they will get good care and not be treated like crap. My main issue with this facility was the difficulty my family and I had communicating and getting help from the staff about our loved one's care. I'd call administration, the front desk and the nursing director and the lines would just ring. No voicemail. When I said I wanted to talk to someone about his medical care, they were going to transfer me to a social worker. What? I found out from the front desk the nursing director wasn't in the facility that day (and no voicemail on!) When it was time to transfer our loved one to a long-term facility, there was no outreach from the social worker. My brother attempted to get a hold of her and someone in a completly different role called back that couldn't help at all. All the while, the facility was telling him our loved one (who is bedridden and needs 24-hour care) would be discharged that day. It was a lot of unneeded stress. Once my brother did get in touch with the social worker, she was very helpful. However, upon transfer, our loved one's meds were left behind and no one told my brother $1,800 was due to the new facility the day of move-in. I will also say his day nurse I had interaction with was helpful and they did a good job with COVID procedures. read more

My mom was sent here after release from Banner Baywood Absolutely awful care. We had to contact Case Managers about her care. They shared nothing with us. The receptionist was rude every time we dropped off clothes and cards for her. They would let her lie in her feces for hours I would have to call the nurses station to go take care of her They gave her 15-20 minutes of therapy a day. My mom was painting and moving 4x8 sheets of plywood 2 days before her fall. She had luncheons for neighbors and they let her lay around in bed 20 plus hours a day. One day she rang for the nurse to help her to the bathroom and the nurse came in and said get up snd to it yourself. We were contacting the office daily with issues. Ariel was very helpful with a couple of outside visits but I would not recommend this rehab center to anyone read more

I nick named this place the "hell hole" and that was being kind. We had to BITE our tongue there for fear of them making my moms stay even worse than it was. If a hospital says they are sending your loved one here, PLEASE DO NOT let them. One night when they didn't take my mom her pain med after 2 requests and over an hour later, my mom called my sister, who called the nurses station, the nurse finally went in and scolded my mom for calling a family member. My mom would ring her bell to go to the bathroom, an hour wait almost every time. Someone would come in and say, oh let me go find your nurse, what a JOKE. She got up a couple of times and went on her own and got scolded and then they moved her walker and wheelchair so she COULDN'T get out of bed. They didn't change her socks in 7 days. They didn't get her a recliner until after 6 days. They didn't get her xrays and ultra sounds for days, waiting on orders, one excuse after another. They really need to take that sign out front down, it's OBVIOUSLY old and inaccurate, 1 star at best. They kept her on oxygen when she didn't even need it, just to keep her there longer? We kept trying to get her out of there and they threatened us, if you take her out of here, your stay won't be covered by insurance blah blah blah. They bill insurance for all kinds of stuff you didn't get and they keep them there longer than need be because they want to YANK your insurance and lie about the charges. They said she was getting PT when she never even left her room, when called out on it, they said, oh, my mistake, wrong day now that I look. They came in the room one day to tend a stroke victim, they had the WRONG room but were preparing to do rehab practices on my mom for a stroke when she wasn't in there for a stroke. The violations and neglect in this place are REMARKABLE. If you have a loved one headed there, do anything you can to avoid it. More than once we asked to speak to the case manager, one day we waited ALL day, she NEVER showed up, too busy... Fill out your daily breakfast and lunch menu items etc, NEVER got what was requested, they just pretty much bring you whatever they want apparently... The hair saloon, NO RUNNING water in the hair bowls so you can't get much of any hair care but a trim... Nurses sit at nursing station and bark about the patients and who they are and aren't going to attend to, REALLY? ? ? If an ambulance or hospital said they were taking me there, I'd say, just take me straight to the morgue... RUN FOR YOUR LIFE!!! The reviews on there website are a JOKE and people like me with honest responses can't leave a review there, they only allow good reviews. There was ONE really nice lady that helped us most days she was on shift, I WISH I could remember her name, I'm going to try to get it and come back and add it here. This is a 5 star facility. We unfortunately had to do a lot of research when my father fell and hit his head... after asking everyone we knew in the business, this facility just kept floating to the top of the list for physical rehab. We were referred to Jolene Nelson who was extremely compassionate, professional and informative. She answered all of our questions including the difficult insurance ones with certainty and clear options. She also followed up while my father was still in the hospital. My brave stepmother decided to aid my father back to health at home - which I am not sure she would do again. With this recent health scare it has become a frequently shared story of my father's. It is amazing how many of his friends have recovered at Apache Junction Healthcare and gave rave reviews. If in the future my family needs help, we know what facility we are using. read more

The Physical therapists were excellent. As was the Case Manager. The nighttime floor nurses, however, messed with my mother's pain medication, and did not give it on time. (She was in this facility recovering from complications of surgery.) when the daytime nurse pulled the same stunt, I removed her from the facility. Better to recover at home with home health than to remain in a facility that does not provide the standard of care, and causes her to suffer more and, thereby, impedes her recovery. read more

My elderly Mom was transferred to this facility from a hospital. After we got her settled, we went home feeling she was safe for the night, it was around 10pm. The next day we found my Mom bloody, bruised and totally confused! We asked what happened, they told us she fell and they took her to E4 around 11pm last night. Not one person in our family was notified. There was my Mother laying in bed, bloody, bruised and totally confused. We could not get her out of that hell hole fast enough. Do not allow,your loved ones anywhere near that place. read more

This really didn't start well. They were the only people able to take my wife and provide transportation to and from dialysis and rehabilitation, so we were stuck. The only part of her stay that was efficient was the transportation system. The hospital social worker contacted several care facilities, but this was the only one that could supply transportation to Dialysis 3 days a week at 5 AM. The facility proper is set up like a 1960-1970 type care facility, very minimalistic and sterile like a hospital room. The visitor parking is much too small, so some visitors end up having to park at the Goldfield Hospital next door and if that was full, the receptionist at check in suggested I park in the employee parking area at the rear of the facility, which creates a considerable trek for a crippled up old man. When you are first admitted, you are in somewhat quarantine. That unit is the farthest from the front desk and again is quite a jaunt for a crippled up old man. If you have a mobility scooter, you might want to take it. There are several doors around the perimeter of the facility, but no one is allowed to use them because they have no personnel to monitor the visitors. I assume this is all Covid related but I believe the restrictions were lifted last week by the federal government. From what I saw the management is not very accommodating. The nurses were totally overloaded the first night we arrived, so that made them late delivering pills. Some of the pills are time sensitive, especially for patients like my wife who have heart issues as well as the kidney failure drugs. Some of the nurses were more efficient and accommodating than others. After approximately 7 days one of the nurses discovered that whoever had entered the schedule for medications was completely confused and corrected the schedule. My wife is required to take phosphate binder pills just prior to eating any meals or snacks. For 6 days these came hit or miss and were delivered up to 2 hours after her meal was delivered. At check in we specified this need, but it did no good. We even suggested that we bring some from home and were told that we couldn't bring them and they would make sure she got them at mealtime. Seldom happened. The food is another issue. For those of you who are unaware, when you have kidney failure, you have a renal diet which is somewhat restrictive. Canned Fruits and other canned items have a lot of preservatives in them and are not allowed but this facility was made aware of this restriction but they continued to send them anyway. I know for a fact that my wife was not the first kidney failure patent they had cared for. This is a convenient landing spot because some of these dialysis patients are scheduled for treatment at 5 AM and they are only less than 4 miles from the dialysis center. Another patient had diabetes and advised them of this fact but that didn't stop them from continually serving him sugar. I believe they could use a dietician and a nutritionist. This is not just a like or a desire, it is a need and should be a requirement. read more

THEY TREAT PATIENTS TERRIBLY HERE!!! They don't bring water routinely or move the patient around to avoid swelling and bed sores. They left my grandmother on a dirty wheel chair with no covering. They leave the patient under a bed pan over an hour literally. My grandmas been constipated since she got transferred from the hospital they wouldn't give her a lactose free diet which she requires and she will call me and press the call button for a nurse and we will be on the phone over an hour for help if she has to use the restroom she uses it on herself for waiting for long they let her diaper overflow bring her cold food its inhumane they abuse seniors dehydrate them don't let them exercise to get stronger let their feet swell and they haven't showered my 91 year old grandma in over a week the one travel nurse said it's "complete chaos" at this facility someone high up needs to help these elderly people with no voice !! *******UPDATE****** my grandma got a uti for the nurses not using gloves as they take specimens from their patients and they made her wait to be changed for a long time save your family member and don't take them here the nurses are all on their phones and gossiping instead of bringing their patients water and wearing gloves and not like to bath their patients a lot of elderly people complained about the same stuff when I was at the center They misdiagnose their patients it's like an asylum when you go their it's so depressing how these people get treated by the "healthcare" "professionals ". read more

My Dad was sent there from the hospital after a serious stroke, and no more medical care was available. This place was severely understaffed and he did not get enough attention. A few people seemed to care, but most were pretty cold. It was a depressing place and the food was so awful looking. It's probably an OK place if you have a non life threatening issue where you need a little nursing care and PT (the PT department seemed OK). This was not the case for my Dad. After a week, we decided to bring him home on Hospice, where the actual human care and concern was the complete opposite, they cared. Sadly, my Dad passed a week after we brought him home, but I am forever relieved he did not die in this facility. Depending on your loved ones medical issue, I would not recommend this place (probably not any nursing facility-they are probably all not good enough for your loved one). This facility is a JOKE! A majority of the staff is also a joke, they neglect the patients and sit around on their phones at the nurses station ALL the time. A lot of the staff needs an attitude adjustment. Every time my relative needed something, it took at least 10 minutes to get ANY assistance. I would stand out in the hallway waiting for a nurse when they wouldn't answer the call button. No one would acknowledge me. If they ignore the patients when they have visitors, how are they treating the patients when visitation is over? The first night my relative stayed here, the facility did not have the correct medicine to provide. Their excuse was " Oh, we forgot to order some more but here are some cough drops". We are taking my relative to a whole new place. Do not take your loved one here if you want them to come home alive. Let's just say that this so called "care" is so bad that I want to give this place a zero but that's not possible so I unwilling give it one star. Do not listen to the signs on the building. read more

My dear wife has just completed a 2 month stay. She went in on a guerney for rehab to regain her strength, and walked out on her own power. During her stay, she had several problems to address - the most acute of all her throwing up everything she ate or drank. While, she had outpatient appointments at a Urologist, and Oncologist. A blood transfusion was required at another facility 7:00am in the morning. The transportation to and from these places was always on time, and immediate response. The staff, nurses, and admin were pleasant and caring. My wife used the "help needed" red button quite often and received quick attention. The facility is always kept spotless, there is a beehive of work going on. Activity rooms, physical exercise - monthly calendars of events. It is an older facility, but top notch service and care. In my 2 months there (I visited my wife everyday, sometimes 2 times a day) - I got to know and observe the staff - they conducted themselves as professionals with a mission to provide the best care for their patients. So, my highest recommendation and thanks to the folks at Apache Junction Health Center. read more

My sister was discharged from Banner Heart Hospital to AJHC two days ago, so I cannot yet comment on the care she is receiving but their visitation policy is tyrannical: maximum of two visitors per day and they must make an advance appointment the day before the visit. The staff who schedule these appointments can be very rude and inflexible. This is her home now and for the foreseeable future. She should not have to endure this isolation and I will be looking for another facility with more humane policies. read more

My mom stayed in Apache Junction Health Care Center for respite care for 5 nights in July of 2019. When she arrived she was able to walk all the way to her room by herself (with the walker and oxygen tank). My mom doesn't have Dementia or any memory problems (in fact, she was an engineer with masters degree), but she is 90 years old and doesn't speak English fluently. She is also very shy and doesn't like to bother other people. During her stay I called her on the phone every night and she started complaining of shortness of breath the second day. When I asked the staff to check her vital signs (she has COPD), they told me the vital signs were normal. This happened several times. The staff didn't help her to take compression stockings before bed so mom had to have the same pair of stockings on for the length of her stay (5 nights!). When she pushed the call button the staff took forever to answer the call. They never helped her to get out of her room or at least to go to the day room across the hallway. Nobody offered my mom a shower or a bed bath. Basically she was sitting on the side of the bed all day long. At the time of discharge I asked the nurse once again if they checked her vitals and she said the vitals were normal. We had to wheel mom out of the facility in the wheelchair and took her straight to the Banner Baywood hospital. In ED her oxygen saturation was 64% on 4 liters and they had put her on BIPAP right away and of cause admitted her. Mom stayed in the hospital for 4 days. We will never be back. read more

My Father broke his hip and went to Apache Junction Health Center for Rehab after surgery. The staff was non-attentive, left him in places without his walker so he couldn't move. Did not check on him regularly, so often his bed was wet. He developed heel bedsores and buttock bedsores all due to poor nursing care. My dad stayed at this facility twice. We had other choices. After his second hospitalization we would never have chosen any other facility. I have been a nurse for almost 30 years, a manager for 10 and to say I am picky and hard to please would be an understatement. I found the nursing staff to be competent, caring and well trained. The rehab staff is excellent, they got my dad home twice and it seemed to be near impossible. My dad has a difficult personality and they handled him perfectly. My dad loved the food, fresh water is delivered twice a day and there are activities for the residents. The staff meets with families for care conferences and when I brought up a few minimal concerns they were taken care of immediately (like food preferences). The discharge coordinator had everything set up for discharge and we were so grateful! Hope this review helps, I understand how difficult it is to make decisions on healthcare. read more

poor service!! they yelled at my mom after she pissed in bed,she would wait 1to2 hours for nurse to come to her room,,she would tell,scream for someone to come to room and no one would come....they have one or two staff at night for the whole place....I was there at 8pm visiting my mom and she kept hitting nurse button and no response,,,I went to nurse station desk waited for 20mins standing there no one ever came,,I started yelling hello hello still no one ever came!!! Dad had to stay here for a short recovery & a couple individuals spoiled the experience with their bad attitude. For example, Tina Marie, a physical therapist, wouldn't even greet him when coming in... She would go directly to the wall and do paperwork on it with her back to us, and when dad stood up to go do his PT, her response once was, "If he falls, that's on you." This isn't the type of curt language used by all the staff, but there are a few individuals that need to be pulled aside for a course on how we greet and treat each other in this country. -R.M., an HC Admin. read more
