Staff gracefully indulge my aunt, who has a rather demanding personality at times. (I wish I were as patient as they are with her!) Unlike many other nursing homes, Oak Creek staff seemed to love their work, which they perform as a true vocation. The building sits under a canopy of trees, nestled against a forested hillside. Residents have access to a large patio area with a garden, an aviary with cooing mourning doves, and chickens, with many seating areas. The entranceway is always decorated very festively according to season, giving it a cheerful ambiance. Art, music, movies, meditation, crafts, exercise, and many other activities fill the day. My only complaint is that I would love to live there myself and be pampered as beautifully as my aunt and uncle are!

My super Dad, who will be 90 soon, is the main reason she's still around and healthy. But he just survived a mild stroke and physically, he's not as strong as my Mom. As their children, we spent a lot of time helping them but with dementia, it's a very difficult and emotionally exhausting ordeal to handle. We were beyond lucky to have found J&M Home care service; especially Susan. Susan is a super bright lady who has a deep sense of compassion, empathy, patience and the knowledge of the disease. Within a couple months under Susan's care, both my parents' health have improved significantly. My Mom seems so much happier and those moments of her feeling frustrated and feared have reduced significantly. With My Mom's health improving, it has helped my Dad and the rest of us feeling so much lighter and happier. What special about Susan is that, even though my Mom speaks mainly Vietnamese and Susan speaks English, the love, care, understanding and compassion Susan has for my Mom overcomes any language barrier. I am so grateful for Susan and her heart. We are taking each day we have with our parents as a blessing. We pray that Susan will stay with my parents, especially my Mom, for a while. They will kick you out mid-care if you also have mental health issues and recieve GENERAL therapy for mental health, which is also a form of discrimination! I was receiving virtual grief counseling weekly through them, and not only what I said above, but also their therapist I had, Katherine, was lying to the supervisor about me not ever talking about my grief to her, when in actuality, I have been, but she also has been asking me to explain details to her about my other stressors too, which lead to my instincts to be honest with her. However, that has lead her to shift to only listen to those moments rather than my grief moments, hence the belief of hers that "I was never talking about grief" in our sessions, so I had stopped telling her my other details entirely with "I'm not going to explain more details about that because then you're going to say it's not grief" before she asked from then on. Still despite that though, the last session was an uncalled for the last session, with using the adamant excuse that she was ending my care because "I had other problems distracting me from grief"(literally still sticking to the story that I was not talking about my grief). I then just spoke to her "supervisor", Tom May, who, rather than make Katherine take accountability, instead revealed the real reason; he had Katherine kick me out mid-care, and according to what he said, other people too, because "we're no longer servicing those who have a counselor already"(his words). I asked him why specifically because I told him before that I have a general therapist who helps with my general mental health, but she doesn't specialize in grief therapy, hence why she referred me to them. He then said "we're short staffed and we have others who really need our care on our waiting list" and mentioned that it was the management's decision. Since that told me that he was saying in other words that those people's grief mattered more than mine and/or the others he kicked out(since he admitted he was kicking us out to cater to them on the waiting list, rather than just focusing on hiring more people if they were really short staffed), I then asked to speak to such management, Tom refused and kept saying "I'm sorry, I can't fix that", which I believe is utter BS because if they were able to suddenly make that change in the middle of our sessions, then the right thing can be done and have it be reversed back, but he refused with the "I can't" BS! Moral is, if you have mental health issues on top of your grief that you also receiving support for, do NOT pick this place! They will either deny you on the spot(I've had a lot of losses within 5 years, and have been unable to receive AN OUNCE of grief support within those years because all the places I've reached out to prior to Pathways have all said no simply because of my mental health history) OR they will kick you out mid-care, leaving you in limbo with unfinished grief treatment with the excuses mentioned above! They do not care about clients at all, especially ones that have mental health issues on top of grief, like most other grief places! It seriously is difficult for a mentally ill person to find support for grief for crying out loud!!

David helped manage the entire process with matching us up with the perfect care giver for grandma and let me tell you, grandma did not make it easy. Grandma was raised to be tough as nails and was an absolute spitfire. Which of course would cause friction with most care givers. The professionalism of the staff from Livhome is second to none. They focused only on helping my grandmother and finding ways that they could find common ground with her to gain her trust and respect. David also did a phenomenal job at not just trying one or two matches for my grandmother. He listened to our asks in what grandma was looking for in a caregiver and worked his butt off until he found the perfect caregiver that matched all of my grandma's requirements and jived with her personality. LivHome goes above and beyond in terms of quality of service and quality of care. They treated my grandma with such respect and care and I can't thank them enough for helping make such an emotional time for the entire family feel so supported and taken care of.

The food was delish. Dad didn't love the coffee but he can make his own. :) The staff has been there a long time and the ownership is also long-term which I find stable. There are a lot of pluses, but the icing on the cake was that each of three different residents talked to us in passing, and they were very cheerful and appeared happy. My dad is excited to move in soon, and it's a relief to the rest of the family. We're looking forward to my dad experiencing the Men's club, weekly happy hour, and nightly movies. I'll update my review in about three months.

My father lived in this facility for nine months, during which he fell 13 times, despite my clear…read moreand early disclosure that he had Parkinson's dementia. Rather than revising his care or offering support, the facility ultimately evicted him after his final fall, labeling him a "fall risk"--a fact they had known since day one. They violated legal eviction procedures by failing to give a 30-day notice, and informed me while I was out of the country, leaving my father in the hospital and me scrambling to find a new place for him. The lack of empathy during a vulnerable time was inexcusable. To make matters worse, the facility said the only way they would allow him back was if I paid for a private 24/7 nurse--an expense they knew was unrealistic. This was clearly a tactic to push him out while appearing cooperative on paper. Not sure if Aireen T. is still the director but she is not responsive, professional, caring, or compassionate at all. I had to involve Ombudsman to get responses from her on everything. There are also serious safety and staffing concerns. During a power outage in the heat of summer 2024, the facility had no backup generator. We overheard firefighters asking staff where the generator was, and staff admitted there wasn't one. During this incident a female resident was stuck in the elevator for literally hours on a hot summer day!! It was horrifying. Others, like the woman across from my father's room, were left in the dark yelling "hello, is anybody there?" My family and I went to her to calm her down ourselves as no staff member checked on her or other residents to reassure them during this electricity outage. They are clearly understaffed and many of the caregivers are not certified CNAs, and it shows. Staff routinely defer to residents' refusals for essential care, and hygiene, rather than providing informed, proactive support for those with cognitive conditions. Many times I showed up and I could smell he hadn't showered. When I asked staff why they would say that he refused so they didn't do it. Medication for my father was not consistently dosed at a consistent time which was also a huge problem. My father also experienced racism from two other residents, and when I raised this with the Director Aireen, they brushed it off--blaming the behavior on health issues such as UTI and did nothing to protect or acknowledge the harm done to my father. Aireen and executive management did nothing to soothe our concerns. This facility is not safe, not equipped, and not compassionate. I would not trust them with any loved one under any circumstances.
