PURE CLASS, and TRUE HUMANITARIAN, QUALITY CARE AND STAFF. I waited a few months post-op to give this review. I'm blessed to have Dr Leavitt as my eye care specialist and surgeon. The staff here also reflects the leadership at Cascade Eyecare Center, as they say "it all rolls downhill, for better of for worse", and it's definitely for the better here. I cannot recall, or know all their names, but a few- Cassandra in the eyeglass section who ordered a pair of Maui Jim sunglasses for my wife's birthday, Sara (one of Dr Leavitt's assistants), and Tiffany at the payment and scheduling window- and those whose names I don't know or cannot recall, please forgive me. I had cataract surgeries on both my eyes and the resulting restoration of quality eyesight in my case is STUNNING, and I'm ever-grateful to Dr Leavitt and his staff, including those in their surgery center. ABSOLUTELY TOP-SHELF. Point being, QUALITY, QUALITY, QUALITY. NO photos here....and your personal mileage and results may vary, but I went from just hideous vision....to not needing prescription lenses whatsoever, and only use 1.25 readers for very, very VERY small detail work or reading minuscule things. 98-99% of the time I need no help. I see that a Ronald had the same experience that I had due to a worker named Stephanie. She refused to bill my VSP insurance correctly, and as he stated, she was yelling at me a couple times over the phone. They are a VSP provider. I had the eye exam on 9/3025 and it took until 12/30/25 to actually get the glasses because of her not doing her job correctly. I have gone there about 20+years and this was not the first time she would not bill VSP correctly. I am praying for her because it's very obvious there is something not right in her life. She can destroy Cascade eye center if she doesn't stop her outrageous actions, or maybe she should not work there. It would be nice if she, or someone would at least apologize. read more

I brought my grandmothers eye glasses in, she broke the frames & was looking for a similar frame to put her lenses in. Casandra & Debbie made miracles happen!! I honestly wasn't expecting them to be able to find a frame to fit her old school lenses but they found a donated frame & fixed them right up, free of charge. One of the best run businesses in Grants Pass, both the Eyecare Center and the Surgery Center are well-staffed with competent staff who are well trained, compassionate and the best teamwork I've ever seen. I've been going there for more than four years and just had 20/20 vision restored by my wonderful surgeon and general ophthalmologist, Dr. R.D. Leavitt. I was greeted at the door with a pleasant person, who took out temperature, she told us where wait until we were called for the appointment. All the girls where wearing masks. But the atmosphere was upbeat, helpful and efficient. The eye exam and the tests for Glucoma took less than 40 minutes. We even picked out and ordered glasses, the front desks personnel were helpful and effective. All total we were done in an hour. Definitely recommend this optometrist! read more

Excellent company. I am always greeted by friendly staff. Doctor Levitt is great. He did my cataract surgery and had no problem with recovery. my Insurance sent me to this place to get my prescription for glasses. The doctor was great. My appointment was supposed to be 100% paid but I received a bill for $50. Later on I misplaced my prescription so I called them for a copy of it. They transferred to billing. A girl named Stephanie answered. I told her that the program I am on paid 100%. She started raising her voice andsaid she had talked to my insurance and they didn't cover a part on the prescription. I didnt even ask for the thing I was being charged for. I told her I just talked to my insurance and they would take care of it. Then instead of listening to what I was saying, she rudely started raising her voice and talking over me. She would not even listed to my Caregiver when she called her back. She did the same thing to the caregiver when she tried to explain what is going on. Because of Stephanie I will not recommend Cascade Eye to anyone. Stephanie does not need to be talking to people on the phone. Stephanie should be fired. read more

My comments are for the eyeglass section of the center, specifically Janice. There was an issue with the frames I bought there. The lens fell out, after being repaired the problems continued with that one screw working itself loose. The last time it was tightened, it cracked the lens. Janice replaced the frame and put a rush on my new lens. I had my new glasses in just two days. Her help was truly appreciated. Janice is an asset to the eyecare center. read more

I requested an extension to my prescription it expired in late March. I have surgery at the end of this month(May). The glasses I purchased last year are giving me issues. The right lens jeeps falling out. I wanted to get an extra pair before I go south for surgery. I was told they can't extend my prescription. I've been with them since 2015 or so. My eyes haven't changed much. I might need cataracts down the road. Oregon has no law about expiring eyeglass prescriptions so it must be the business policy. I don't like dealing with "no exception" businesses. read more

I must update my review of Cascade. In my past review I had given a 4 instead of a 5 star. The hesitation was my first impression. When a new patient walks through the door, the front desk could be considered the "foot soldiers". The battle is usually won with these men and women. And although the frontline has the most riding on their shoulders, they are also the ones least praised. It is probably next to impossible to hang a smile on your face for 9 hours. Brushing off any negative dust that happens to fall on you from the past visitor. We do expect a lot. My last review was posted as one of the final patients to walk through the door of a busy business. I, myself, worked with the public for 23 years. I, however, had the luxury of doing this over the phone. I had my share of hard days, but my occupation allowed me the luxury of looking haggard while smiling with my voice, and occasionally rolling my eyes while on my last nerve. Public service, front desk, probably the most difficult to pull off successfully. In my humble opinion, this has been accomplished. Bravo. This business is one to visit. I know that when I walk through the doors I will be taken care of. Clean, Pleasant, Professional, Complete. The Staff appears to be hand selected from start to finish. My experience has taken be over the past two years and two cataract surgeries. So my face, while not the most beautiful, is probably one of the more familiar ones to cross the threshold. And perhaps I now view these gals as family. But they are family that I gladly chose to visit. My Dr. Guymon is perfect. His direct staff is amazing. The scheduling clerk, (Laurie?) knows her business! And even though I have never met the office manager, I did have occasion to speak with her after my first posting on Yelp. If you don't think they care, you might want to think again. She took my suggestions and even my criticisms and ran with it. My only suggestion at this point would be for the public. If you don't get a smile right out of the gate, give one of yours. And if you aren't clear on what your insurance covers, ask your insurance company. read more

Had the second cataract surgery today. Vision still blurred for the moment but my vision is more equal between the two eyes than it was. Tomorrow will be a very different walking out without needing my glasses. I still need a new prescription for my reading glasses, the next step. Thank God for these small works, and being placed with the right doctor's who actually work to help their patients. I was seen by Dr. Leavitt, he was an excellent man for the job. All the staff I encountered were equally capable, and I had every confidence in their abilities. The entire experience with all those I encountered at Cascade Eye Care has been one of the best experiences with any medical facility I have had to deal with. Thank you all very much! read more

Our experience was service excellence! A family member we were providing temporary care for had terrible glasses. We brought them and "Mom" in to see what they could do to help. They did were not concerned about payment, immediately set to cleaning them. They could not be cleaned but they have a relationship with a local philanthropic non-profit and were able to locate glasses that worked perfectly. The office staff were incredibly kind, generous and supportive. Because of this they will be getting all of our eyecare business. Been going there for about three years. Doctor Guyman was okay. Did a good checkup. I ordered contact lenses. It was hard to get them in my eyes. At one point a lease was stuck under my lid. I called and they had me come in. The assistant removes the lense with out any problem. They said I had to see a doctor too. It was not Dr. Guyman. He proceeded to do a checkup on my eyes. He kept asking his assistant if it looked like I had narrow eyes. He then proceed to give a drawing explaining that my type of eyes were prone to the ducks behind the eye could become blocked and the pressure could make me go blind. He recommended putting a hole through my eye with a laser. I've never had any problems with my eyes. I went home and researched it and I decided it was not needed. They set me up for the laser procedure. I cancelled it. Dr. Guyman has never said I had any problem with narrow eyes. I don't trust them anymore. That was over a year ago. I will get another exam somewhere else if needed. read more

I saw Dr. Rob Levitt for cataract surgery assessment and we decided it was time. My first impression was that this was a smoothly operated comfortable clinic with warm, friendly and professional staff. In the surgery process I had contact with many team members over several appointments. You could tell they knew their jobs and that you were in the hands of a caring group of folks. Fortunately my astigmatisms were correctable with Toric Lenses and the cataract surgery was the perfect time to do this. I've always had bi-focal glasses since my near and far vision required correction. Dr. Levitt was able to give me 20/20 vision for both distance and near sight. In my case he was able to eliminate the need for glasses. I can even read small print as well as see the leaves on distant trees. It turned out that my first impression was right. read more

I am "stuck" with a pair of glasses, and $422 out-of-pocket. I would love them, but one of the lenses doesn't quite fit the frame. A corner of the lens feels like it wants to "pop" out. I've been back twice, and told all of the reasons I am wrong. No effort was made to "right" anything. They must have laughed after I left the office, each time. read more

Horrible front desk help. We went in prepared with the new patient packet printed from their website and the lady at the front desk says she's never seen these forms before. Then my daughter filled out the HIPAA form giving permission for the office to speak to me her mom if necessary and the front desk lady acted like my daughter was mentally disabled. Then after the exam we asked where the OHP frames are to choose from and was advised the Dr would pick out some old style frames to choose from because they don't have a OHP section Needless to say the Dr never came out to select frames and we left. I certainly will be reporting this to my CCO Allcare. read more

Dr. Guyman seems a caring professional, and the check-out staff are helpful. I'm sorry to say that reception and frame service are by turns helpful & professional or rude & hostile; I never know which to expect. I'd recommend a thorough training in customer relations for those folks. read more

My husband and I have had nothing but problems with our glasses! It used to be that the actual doctor went thru the eye chart testing while you looked through their machine. They do not do this at this eye center and I'm pretty sure that's why there are so many prescriptions being done incorrectly. My husbands glasses are so bad, the he currently wears his old one until our insurance will cover for a new pair next year. Mine are just horrible. When I first put them on, I told the girl that they were not right. She told me their 'pat' answer of how they just take time to get used to. So off I went, with glasses that i could barely see out of. I came back to complain about them and after telling me that I just needed to get used to them, they finally checked the lenses. Instead of being a 6 rounded lens, it was an 8 rounded lens!! So back they go, up to Portland to be redone. I get them back in 10 plus days, and they still are not right. It's literally as if someone put my glasses on my head all askew...that's what it was like trying to see out of them. I finally see the doctor again, and he says my script is correct but he has one of eye tech's check my lenses. Well don't you know the lenses were put into my frames all crooked!!!!! By this time....several weeks...I could no longer wear my old glasses as my eyes were adjusting to these crooked ones. These lenses are still not right as they are not put into my frames correctly. I will never go back to this office. They have lost my business, my husband's and any of my friends that have heard my story. Quality of care is very low. read more

Sept.10-2018 My husband and I had appts for eye exams with Dr. Leavitt at Cascade Eyecare Center in Grants Pass, OR. This was our first visit here. The staff was somewhat friendly, but I just felt a bit out of place. Kinda like the new kid on the block. Dr. Leavitt was okay, but again not overly friendly or that welcoming I did not feel. It felt more like let's just get this done. Because we had our eyes dilated we were not able to pick out new glasses at this time. This surprised me because I have never had that happen before. We did end up going to Costco for new frames instead of going back to Cascade. A couple weeks later my glasses needed to be adjusted so rather than drive back to Medford I went to Cascade in Grants Pass. The women that was helping me asked if I got my frames through them and I told her no. She was okay with adjusting them anyway and she did. I tried them on, she checked the fit and all was good. Then, a couple weeks later October 25, 2018 my frames needed to be adjusted again. So I went back over to Cascade. The same women said, "no problem." Only this time after she gave me back my glasses and I put them on she just started to walk away. I asked her, "do they look straight?" She said, "yes." I said, "okay." She walked away, I started to walk out and man my vision was so blurry. I thought to myself, "I guess my eyes are needing to adjust from sitting without them on." I actually had a difficult time seeing and driving home. I thought it strange, but this new prescription Dr. Leavitt gave me is so different it has taken some adjusting these past several weeks. Hours went by and my eyes are not adjusting. I thought, maybe they need to be cleaned? No, she just cleaned them. Finally I took them off and looked at the lenses. OMG they looked scratched. No. They looked as if the lens had melted! I freaked out! I could not believe what I was seeing. Why did I not go with my instincts right when I was walking out? I felt as if something was wrong then. Even as I was driving home I was asking myself, "what is wrong?" My husband drove me back over to Cascade. The women that had helped me earlier was out to lunch and the women that was there looked at them and said, "well you did not get them here and so there was nothing they could do." I tried to explain to her that earlier when I was there they were fine. That somehow something must have happened when the other women adjusted them. Maybe when she heated them up for the adjustment. Again, nothing they can do. I have to go to where I purchased them and I signed a paper agreeing to that. I couldn't believe it. Seriously I was blown away that they were not going to take responsibility. DO NOT RECOMMEND CASCADE EYECARE CENTER IN GRANTS PASS, OREGON. read more
