I hope you don't have to use a facility or rehabilitation center, but if you do, this is one of six I've spent extensive time at as a visitor and I am beyond impressed. My first pick, for sure. A family member has been here for nearly one month due to an illness requiring the level of care provided at a skilled nursing facility (SNF). I've visited at least 5 to 6 days weekly. Every visit I am more and more confident/pleased with the level of care and cleanliness here. You enter the bright/light building and are warmly welcomed/greeted; currently there's fun, festive decor through out. The holiday decor is a great addition, nice touch to bring some holiday cheer. There's a beautiful library/room for residents/guests, not quite complete, but nearly ready/renovated. The entire property is slowly being updated and renovated and it shows. It is evident that there are a lot of changes taking place here (literally and figuratively). The property has been maintained pretty well, considering. All of the staff from the LVNs, nurses, therapists (OT, ST, PT) and so on have been extremely helpful and provide the type/quality of care to match. My family member is always treated with respect and dignity, no complaints expressed. There are daily activities (often enriching), a gym, and the food menu is quite extensive. The meals have been described as both "nutritious and tasty." The dinner menu offers a month of varied meals; without repeating a meal! They even asked if there are items my family member doesn't particularly like. Some additional things I've observed, the staff is able to communicate clearly and collaborate effectively. The staff has been attentive and responsive; exceptional round-the-clock care, truly. There's a large, private parking lot, so parking isn't an issue here. Additionally, there are policies and procedures to protect residents' possessions. They take a thorough inventory of personal belongings and provide plenty of storage space for the belongings. Things haven't disappeared here. A BIG shout out to Tine and Wendy, so kind and caring. They truly are exceptionally hard working, so empathetic, and patient. Escondido Post Acute Rehab offers a high-quality skilled nursing program. One of the best SNFs around! The administrator, Daniel D. is empathic and eager/open to feedback. If i could give them less i would. My father was there for four days, in that time no shower. No access to a phone. Dirty room in bad repair. A staff that was rude and not willing to assist basic needs. When my wife visited, she asked the nurse to assist my father, whom acted like it was a inconvenience to do so. So after discussing it with his primary physician, the decision was made to transfer him to a different facility. During this process, i was contacted by several staff members of the facility and the Attending doctor, saying "your father is fine where he is and doesn't need to be moved!" I am sorry, family and his primary physician have the right to choose his care not the staff! They dragged their feet for 3 days on his transfer. After several calls were never answered by their staff from me or the new facility's intake personel, i finally told them that i would be filing a suit for willful negligence of care for deliberately impeding his transfer. All final paperwork was sent over immediately after my comment and my father was transferred within a few hours he's doing much better at the new facility. My mother was in there care from November 2025 threw December 2025 Mom came in after hip surgery she had damincha and so she needed to be fed and they where to do a oral check for food .Well they did not do this I got a phone call almost 9pm that mom had vomited earlier that afternoon and was having trouble breathing. 1 why didn't they call me as soon as that happened. Any ways if you're loved needs extra help or can't communicate there basic needs RUN SKIP THIS PLACE .She had a bed sore that wasn't reported and 1 on her foot .I spoke to the nurse up front about all my concerns and she said we'll if you're not in there faces daily it's not gonna get done and maybe not even then . There ratio of staff to patient is a joke .Anyways mom was sent to the er and from there we brought her home to finish her journey. This place never realy had supplies in the room And for the life of 1 of the staff was arguing with her about how she wasn't hurting her when I walked in realy she has dementia very unprofessional .That day we set it up for mom to come up the day for is when she ended up in er . Honestly the staff was amazing except for one b*tch name Caitlyn one of the nurse assistant at station one I don't know if work she was on her PERIOD that day or she drop her TAMPON accidentally when she walked inside the skilled nursing facility but anyway let me explain what happened and my physical condition I was a patient there paralyzed from the neck down I will ask the nurses that they could flip me on my side so I won't get any pressure wounds from laying down in bed because I couldn't turn myself obviously because I was paralyzed from a car accident and when I ask her nicely she will roll her eyes on me I shook it off first cause it was my first time with this nurse assistant so I didn't say anything to her and she didn't roll me on my side all the way still I was still on my pressure wound that I wanted keep weight off it so it could heal can I ask her again can you try me all the way because that's where I have my bed sources then she said she couldn't do it and I told her why not and she said because I can't and I asked her can you ask someone to help you and she said I can't right now someone else needs me and I said OK so I asked another nurse assistant that was walking by I'm explained how to do it how I wanted to be turned and she did it then two hours later I called them back and she end up coming back the one who couldn't turn me and she said what do you want this time with an attitude and I told her can I get some water then she says you ask for too much Zarif I already know you even though it's my first time having this nurse assistant then 45 minutes later I called them back for more water then she's told me again and I said yes I drink a lot of water then after that when she had to feed me if I knew this girl Danielle who was another patient and I said yeah that's the homegirl what about her that she said you too will be a good couple I see you guys together and I told her why you say that is it because me and her are same age and also paralyzed making it obvious what she was trying to say, that's like going up to someone and saying to 2 cancer patients I see you guys together only because they're bald going through chemo therapy as an example and also when my parents came visit me in my dorm she was taking my blood pressure and I was talking to my parents then she tells me Zarif be quiet and I was like why and she said because I talk too much she said that in front of my parents even though I wasn't talking to her I was talking to my mom and dad both of my parents looked at her I wanted to say something to her what's her problem that I wasn't even talking to her i was talking to mom and dad but they didn't want to cause any problems cause they wanna keep continue visiting me at the facility then finally my parents left to go home I told her if you don't feel like working today you could go home so you don't have to be working with me because I'm paying my insurance to pay you to help me and when I ask you for something you get irritated and mad for something simple then she told me don't talk to me like that and I told her well it's true you're getting paid too do your job then she gets butt hurt when I told her that then we start arguing and she'll walk out come back in 35 minutes and try to think about a comeback to say something to me I had to deal with her BS for 16 hours because she was doing double time and I had to ask her to turn me again but this time she left me close to the edge of the bed and I got wedged in between the bed and the metal safety rail and I was falling and slipping off from my bed because she turned me close to the edge and I was yelling for help then she comes in and she says why are you yelling because half of my body was hanging off and my head got stuck in between the rail and he was pooping on my neck and head where I got surgery then she takes her time putting on her gloves and made me suffer first before she helped me I think she knew what she was doing that's negligence right there she's didn't care so I told one of the nurses what happened and told their boss I don't know why she wasn't fired I don't know if I'm the only one that this happened to but that was a fair taking advantage of someone who can't even help their self that would've ended bad for me can't believe there's sick twisted people exist like that they're just there for the money not to help people who are sick or paralyzed that can't do nothing about it read more

Staff is amazing and honestly can't complain about much except the fact they kept me passed my discharge date for absolutely no reason. When it was the day for me to leave it was like everyone forgot about the whole plan for me to leave i have a feeling they were trying to keep me here as long as they could for insurance money because the reasons they gave me were just absolutely bs whoever the head person here is should be ashamed of themselves we are human beings that have lives to get back to read more

My dad was temporarily placed here for rehabilitation. This is not your typical skilled nursing facility. As soon as we walked in the door it smelled really good and My mother and I were greeted by the most kind, loving, and considerate staff. My dad's care was nothing short of amazing. All his caregivers were the most tentative, caring and compassionate people we have ever met. One caregiver in particular was named Windy and she treated my dad like as if he was her father. She called him papa and tended to all of his daily needs. The x-ray tech Tony was awesome as well as the physical therapists. My dad said the food wasn't bad and the dietician's came in from day to day to check on his food quality and adequacy. The rooms need some upgrades and cosmetic fixes but as far as the care it's nothing short of the best. My dad has also made some lasting friendships and has left there with a sense of knowing if has to receive placement in the future he'd like to come back to a place where they made him feel at home. Complete lack of communication. Voicemails, messages, emails none returned. 4 days after admission they got an appropriate bed and commode. This after I received a call about advanced directive. That poor girl got an earful and called the DON at home. Employees are heavily accented and not understandable. Director is only reachable via email, they won't get you in touch w him. The DON, just wants to 'move forward' ha told her 1st thing is better communication. Yep she failed to answer email or voicemail. In my experience most of the CNA's aren't doing there job. 1 refused to change urine soaked sheets and left it for next shift. Last thing a UTI patient needs is to lying in urine. 2 hours later it was changed. Hope the director and CalPH sees this. read more

Mom been here a year and a half now. She's been receiving a good care most of the time. Thanks to the nurses at station 4 where mom is staying and to her CNA's . She's also receiving a good nutrition, the dietitian is always discussing choices of foods as mom is sometimes picky.The assistant director of nursing is also helpful every time I run to her if problems arise and in no beat she tries to solve it! Only now that I know Director of activities exists. She introduced herself as the new director of activities and asked what mom needs and enjoys to do. She comes every morning to cheer my mom and with a newspaper on hand she points the date of today just to orient my mom . It's nice to know this little things that they do despite my mom's dementia. As I'm writing this review my mom is busy reading the newspaper although she forgets what she just read in a few seconds! read more

I had never seen such a lack of communication, to the patient or the family, my mom (in her 90's) just got transferred here , and it's been 2 hrs and not 1 word from any one, not 1 cup of water , not even a "how are you doing" .....while I'll the nurses and staff are huddled up in the front desk having a good ol time , laughing and all..... one of the worst experiences we ever had . Man, I wish she was back in the hospital, at least she would be having fluids .... And some care . I wish this on no one. I was here in 2010 and again in 2011. The place was a joke, I swore I'd never come back. They have changed management companies since then. Here it is 2024 and I decided to return for care. This place has done a 360, caring staff as well as administration. The food is very important for healing and health, it's very balanced, nutritious and tasteful and adjusted for your specials needs. My room was cleaned everyday with very nice housekeeping staff. Therapy, PT and OT was done well. The staff is aware of your needs and Taylor your routine accordingly. The therapy area is small and limited as far as equipment but they get the job done. As any building this age, it needs a remodel. They are doing some now but could use more. They do have Wifi as well as streaming channels to watch with a huge variety of choices. There is a nice garden area to visit to get out in the fresh air. They also have activities to join everyday, bingo, arts and crafts, puzzles and more to keep busy. My over all stay has been very positive and if needed, I will return. Something you need to be aware of and this is true with every facility, they cannot order your medication until you are here and signed in. They have to send out to a pharmacy to order and then deliver, this will cause a lapse in your medication. Please keep this in mind when you go to ANY skilled nursing facility. read more

I would like to say how happy we are with the care of our 97 year old mom. From the Social worker, receptionist, the physical therapy and the amazing nurses. They all have been amazing. Such a happy place even met the head cook. Wish I could remember his name. We met him on his break and it was pizza night. He asked us what our mom's favorite pizza was and her room number. And surprise! Her pizza had everything she likes on it. She loves the food and says it's always hot. Our family visits a lot and we couldn't be more pleased on how they make sure she is comfortable and clean. The place is very clean. All the nurses are friendly and attentive. She has a special bond with Em. Thank you so much for taking care of our mom. She is receiving the highest quality of care and we are very grateful! read more

My name is Bryson White and my stay at this place was nothing short of amazing when I got there I could not walk and was told I never would but they have a Physical therapy and Occapational therapy group that was sent from god because not only did they get me walking they became family and the nurses and CNAs were something special I want to put them all by name but I don't have enough room but note the Director Dan is amazing and I would do it all over again and leaving them is like leaving family I love them all and they will be missed. read more

If I can do less than one star I would. My sister has been in this facility for over a year because they have some kind of shady handshake deal going on with Kaiser and this was the only place they would put her into. The effort to do physical therapy with her was so non-existent it's safe to say they didn't do any. When the antibiotics my sister was on caused her major stomach troubles they just stopped bothering to feed her or even hydrate her. After several hospital stays to deal with mal nutrition, dehydration, seizures caused by dehydration, resulting in g-tube needing to be placed, and pneumonia, so bad because it was left untreated for many days, we are hoping this last hospital stay will materialize in a better care facility because my sister no longer has Kaiser and we hopefully have options. Thank God! Be very very wary here...... I feel if my sister returned here she would die. read more

My mother was admitted after a fall and after visiting her we had her transferred as soon as possible. In the 24 hours she was in the facility she never received the pain meds she was prescribed for her fractured back. This seems like neglect to me and it's so scary that they are able to operate. In her short stay she was served cold dinner, her room reeked of urine to the point of it being hard to breath, and she sat in pain for hours. I hope other people aren't unknowingly transferred to this facility in their time of need. Thankfully my mom had family around to get her out! read more

I was admitted to this facility after an accident and broken bone. My family, friends and I were very impressed with the quality of care I received. The food was good and the alternate menu was a nice bonus. The majority of employees were focused on their service to patients, and the office staff was very helpful in expediting my discharge. I was impressing with the kindness of the staff from Crystal in admissions to the CNA's, Nurses and the Director of nursing Angie. They have recently lost almost all the PT& ot staff & that explains my abysmal treatment except with Joseph PT who evaluated me & helped me get oriented to the place. He was the only staff that seemed to know anything or even cared. I got no therapy at all. I left before Joseph returned. They put 4 women with toileting issues in 2 adjoining rooms with 1 toilet between them. Impossible situation. No supervision by anyone above an LVNurse on weekends & nights & staff seemed to be having a party in halls with very loud voices laughter & music all nights. No sleep. CNAs were very nice & tried to care for us but when things went wrong or wrong things happened in "care" I got, they had no authority to change or question what they had been told. I demanded to b released because they refused to call & question orders that were in direct opposite of what Dr ordered when I left the hospital. My condition waS worsening & I insisted on leaving so I could implement Drs orders at home. I had to wait all weekend till Dir of Nursing came mid morn Mon & released me about 5:30pm. My roommate left AMA because she refused to wait any longer to get paperwork done releasing her. I would never accept going there again for any rehab. They need staff who can manage without anyone in authority present. Management was totally absent during the 5 days I was there. Many patients were out of control & the CNAs were overwhelmed. It was a dangerous situation & was extremely poorly managed. An accident waiting to happen. read more

I had a terrible visit in this place when i visit my grandma last week. When i arrived, grandma needed help changing her clothes and using the bathroom but her cna and nurse were nowhere to be found. i asked the cnas at the station at the back for assistance. they told me they are going to look for her cna and continued to chatting and use their phones. I even saw some nurses hanging out in an empty room whos also in their phones. Good thing this one this one staff member is exceptional. He approached me asked if needed anything , he put this belt thing he's holding and immediately helped my grandma. He was gentle with her and knew exactly how to handle her stubbornness. Before he left the room he even thoughtfully gave her a drink of water. Thank you Renzi. i wish there were more staff like you. As i walked around the facility i noticed some inconsistencies in the environment. The main dining room was nicely decorated and the activity room but the rest of the facility including the hallways and nurses stations was neglected. There was also another visitor complaining about loud laughter coming from the small office at the corner for the hallway infront of the main dining room which was disturbing her family member who was trying to rest. For a place that's supposed to be quiet space for healing, this was quite disappointing. read more

I will be forever grateful for the staff at Escondido Post Acute. My father was placed in an awful facility in El Cajon, against our wishes, and was not cared for with dignity or professionalism. After much effort and no thanks to our insurance company I was able to get him moved closer to his home in Escondido (Escondido Post Acute). Although this place could definitely use a remodel and like everyone faces staffing shortages, my father was treated with dignity and care. As the primary care giver I was kept in the loop and treated respectfully. My questions were always answered. He received great PT and OT. I could finally sleep at night knowing that I had the names, numbers and support of specific people willing to help me. I look forward to seeing even read more

This place cares for elderly and the infirmed and the rehabilitating. Staff is friendly and kind and patient. Gardens are beautifully maintained. Parking lot can get full during shift changes. Moderate odors can be experienced, but, remember, it's a care facility. read more

I have been trying to resolve a billing problem for almost a year. My husband was an inpatient at the facility in October, 2022. He exhausted the Medicare allowed days. We paid the required Medicare co-pay for the rest of the month. Ok so far. His medical condition required care for a few more days in November, 2022. We were approved for Medi Cal beginning the month of November. Escondido Post Acute filed the claim with Medi Cal and it was denied because we had not reached the share of cost. I contacted Medi Cal and they confirmed we had met the Share of Cost after the facility filed the claim and for the facility to refile the claim for November. The billing department refuses to refile the claim to Medi Cal. They say we must prove we have met the Share Of Cost before they refile, I have asked them to bill Medi Cal and let them make the decision. They flatly refuse and go into their medicare billing scenerio that should have no bearing on this situation. I told them to contact the facility that he was transfered to after he left Escondido Post Acute whom we satisfied November 2022 Share Of Cost with. Escondido Post Acute has not contacted them and the billing department says the bill is going to be turned over to collections. read more

Unfortunately i have placed my trust in a very unprofessional staff and facility. In a difficult time my family was going through when our uncle was very ill and weak one of there so called "nurses" was supposed to be assisting him to the rest room he was a bit slow to move like many of the other patients , the" nurse " pushed and hurried him in a very impatient and rude way almost causing him to fall and making him feel unsafe . After he asked her to be more considerate (in spanish) she said to him "english ...SPEAK ENGLISH!". This is called abuse!!!! We deeply regret making the mistake of placing my uncle in this rehab center ...it will never happen again. We hope for a better managing staff for this place ... Who knows how many times this situation has happened , Also...we were denied this nurses name and were told it would be "taken care of" . no comfort there. read more

This place looks gloomy, depressing, filthy and smells like everyone has peed themselves for a week with out a bath. You can instantaneously see they are very short staffed. I bet they have plenty of staff when the state comes so they can stay open. Poor patients, poor nurses, poor CNAs. read more
