He only used a puller to pull the dent out; he failed to reduce the obvious tension 'bubble' immediately above the dent - usually performed by light tapping over the area of tension. So, at the end of the repair, the fender looked better but distortion was still clearly evident where the fender bulged outward just above the location of the dent.
