The Heartland staff have been so…read morepresent, honest and always available to be in communication with me (even if I left a message, someone has gotten right back with me.) One person broke a promise and was a complete professional with their honesty and just taking responsibility for the circumstance that had caused the broken promise. In my world, that is music to my ears. When I am overwhelmed ... especially, with regard to my mother's care ... I need honest, present, loving people helping me who are going to deliver. I have felt as if I have Angel wings wrapped around me with these people. Life is simple: Be loving, have your words match your actions, listen to the other, reply in kind with words that forward happiness and well-being. Said another way: In any situation ask: "What would God do?" If it's kind and loving ... do it. If it's unkind ... don't do it, karma is real. May the grace of God always be your safekeeping as you flourish and prosper in all ways important to you!

    I arrived to my dads home today to visit and offer my sister some respite. To my shock my dad…read morecanceled their services. The social worker Ernestine is reportedly investigating my sister for trips to the grocery store. My dad is able to get himself in his wheelchair to go tot he bathroom or get a snack out of the refrigerator. Most of the time he is happy resting in bed. The one time he fell my sister was at home. He didn't realize his legs would simply give out under him. Hence the wheelchair now. It seems like spite investigating my sister and it's causing stress for my father. The whole thing could have been prevented but she seems to want to have a p----ing match with my sister. So unprofessional So now my dad is trying to find another option through his primary physician. Not what he needs to be doing. He should be peacefully resting right now! They can't just offer peaceful resolutions?? So disappointing The nurses are wonderful. But Ernestine has been hassling my sister from the beginning. About 6 weeks ago.

    I would strongly recommend looking elsewhere for hospice care. Continuum Hospice added stress to an…read morealready stressful situation--my mom passing. Care was inconsistent from the start. After enrolling on a Saturday, no nurse visited until Friday, leaving us on our own for five days. The nurse who did come called (with poor reception) to suggest stopping all medications, then never followed up--I had to track down the physician myself for guidance. I wasn't given the blue book--"Gone From my Sight" that explains the dying experience until the day before she died. Communication and clinical judgment were unreliable. When she had a severe reaction to morphine, it was dismissed by a nurse, and I had to escalate to the physician to get her medication changed. The day before my mother passed, I was told she was not transitioning. She died the next day. There were also clear signs of understaffing. They repeatedly offered help with bathing, but no one showed up for weeks--too late to be appropriate. After her death, the nurse who arrived to declare her death had her name wrong and lacked basic information we had already provided. So I was having to correct the spelling of her name and get my phone to give her the name and phone number of the mortuary. I had given this information to the social worker so it should have been in their system. A small thing one could argue. But did not make me feel cared for. Compassion for my mom passing was absent. It was totally transactional. I regret not changing providers earlier. I tried to initiate a change, but the paperwork was not sent in a timely manner. If you are not happy with a Hospice change at the first sign of trouble as transfers once someone is close to passing are more difficult. I should have been able to focus on my mom and dad during these difficult final weeks--not manage gaps in care.

The nurse (Allison) at…read moreVallejo Cogir and I talked to the head nurse (Angela) at Continuum on the phone twice - once at the hospital and once when my mother was brought to her apartment at Cogir. Angela agreed to provide servicing of my mother's medication throughout the night every four hours. We even discussed the hours that they would provide care during the night. Angela put none of this information in writing. This organization and head nurse cannot be trusted to do what they say they will do. Continuum says we did not call for help throughout the night. Of course we didn't because we were told by Angela they would cover us.

Fox and Associates have been truly amazing for our family during our time of need. When choosing your duty care provider keep in mind you are really looking for a service that is registered by the state to provide duty care, RN or both services. Fox & Associates has certification for both. You are looking for a great back office team who will accurately bill plus provide full daily detailed paperwork on services completed if you are submitting to insurance. We were paid 100% of all services by our long term care insurance company based on the information Fox provided, without the need to escalate or go back and ask for more information. Finally, the agency should have a great selection of duty care people in your local area who will consistently be there for you. Fox not only had a strong selection of people in Santa Rosa and surrounding area but provided us an iPhone application to see who was scheduled to come to the house over a forward looking two week period of time. Fox's is also local to Santa Rosa which gave us piece of mind that they were not simply a talent agency 50 miles away just sourcing an individual for us. Having all of Fox's staff in the area made all the difference when fill ins and special services were needed. Going through the experience I more fully appreciate the need for a great back office and management team. Fox was incredibly proactive, even having hotel rooms reserved for clients should there be an evacuation order due to local fires. They communicated with the family at any hour of the day when needed, and I was given the cell phone number of the owner and office manager for emergency situations. I feel lucky to have found Fox during this difficult time when we were caring for our loved one. With the stress of wanting to do best for your family its great to have the backing of true professionals which were always willing to go beyond the call of duty to make things as easy as possible.

Headquartered in an actual home, their campus has a two story office structure adjacent to their parking lot that houses additional private offices, conference and training rooms.
