Ended up having an awful experience and should have written about this right away. (This all happened just before COVID). Short version is there were a litany of mistakes that added up to a big problem. Things like the physical therapy department refusing to do PT and OT for 9 days, just letting our sweet 99 year old loved one waste away and get weaker and weaker; nursing staff not following up on a STAT order from a doctor to rule out pneumonia (after they called for an ambulance - it was that serious), repeatedly giving her the wrong food or food she was not able to cut/eat on her own, and would outright ignore her call button unless a family member was present and went to search for a nurse or nurse's aid. Many missed communications about orders between the facility and the doctor, with the facility saying it was the doctor's fault. After a worthless attempt at working with the ombudsperson, we finally reached out to the doctor directly and miraculously the problems went away. Spoke to the Director three different times and was left with promises of improvement with no action. She would smirk at me in the hallways, and go out of her way to say "hi". Frankly, I could care less about insincere greetings. Patients deserve the care and attention they are paying for, not lip service. Icing on the cake was at discharge: Our loved one's regular doctor had retired while she was in rehab for 2 months and she was having to be transported via medical transport which was extremely painful for her. We asked for meds for 45 days so that would give her time to heal some more before we had to find her a new doctor. Nurse (Supreet) said that the doctor would only prescribe for 30 days. Ok, that would buy us a little time. They ended up giving us only enough for 3 days without even telling us. When the pharmacy called the prescribing doctor, he said he couldn't extend the prescriptions since she had already been discharged. As we were waiting for med transport, PT refused to even teach us how to transfer a newly bound to wheelchair person in and out of bed until they realized how unsafe that would have been to discharge her, even though they had known two days prior that she would be discharged. My elderly mother received great care from the staff and Administration during her six week stay. The Martinez Facility was one of only two care facilities in the County without and Covid cases among patients or staff. Although, at the end of the day, the most important issue is care and respect of the patients, I gave 4 out of 5 stars rating due to a non-working, almost non-existence phone system. During Covid 19, where patients cannot have physical visits from family and friends, it is important that they are able to stay in touch by telephone. read more

When looking for a place for my sister in law to go, we were very pleased with Legacy. The staff here was very nice, and caring. The nurses who took care of Tara were the best. We couldn't have asked for a better place for her. They were gentle, caring, and listened to our concerns, wants, and needs. As soon as we walked in, they greeted us with a smile, and a hello. That is so important, when you know your loved one will be spending time time there..especially their last days. Our family is grateful for them, and how they treated Tara. Not only did we feel their genuine care and love, but so did Tara, and that was the most important. I highly recommend this place to anyone, and wouldn't even mind staying there myself one day if needed. read more

I feel very fortunate to have had my clinical's done here at Legacy. I am in the CNA program at American College of Nursing I graduate next week on May 9th. I feel that I am more than ready to take the American Red Cross state test on May 20th since I have had so much experience here. The first day I walked in to Legacy I have to admit I was very nervous to actually be hands on and have to perform ADL's for the residents. I was encouraged by the CNA I had that day to just jump right in. I just cleaned the residents the way I had learned from my CNA school book and then perform other necessary daily living tasks. I had gone from feeling uncomfortable to comfortable working with the residents in one day! Today was my last day and I was solo on cleaning my patient while the CNA was watching. The CNA's provided answers to all my questions and were always very helpful. The CNA's here do all the work by the way. The deserve to be more noticed for their exceptionally hard work and dedication that they put in to their job. There is many different residents in this facility and the all stole my heart! I admire each and every one differently and I am going to miss them! I left an application on the administrators desk today since today was our schools last day being there for the clinical's. Lets hope I get call to come in for an interview! My mother, at age 92, has had to go to nursing care facilities on 3 other occasions for respite or PT. Legacy, has by far, been the best facility. She was admitted to legacy after a minor fall that resulted in a fx'd thigh. Initially needing bed care for the fx, then PT to regain transfer and wheelchair mobility. Staff was vigilant and comprehensive with their level of care. A high level of expertise, combined with compassion and kindness. Dr Henry and the staff were always available and answered all our questions and concerns thoroughly. Loni and the PT staff were excellent at there job, I was present for an early PT session and was very impressed with their professionalism, patient communication, and PT skills. Loni even traveled to mom's assisted living facility to discuss mom's care plan. We were never ignored, never spoken down to, always accommodated to the best of the facilities ability. The entire team, from admin, to nursing, to the covid doorkeepers, were polite, professional, and most important, enthusiastic about their work. I'm actually still here at legacy. when I got here 6 weeks ago I couldn't walk or barely stand. Now I'm walking again and better every day. First let me take you on a virtual worded tour of the people that have helped me. When you walk in you're greeted by Ivy. Ivy is happy every day and always doing 10 things including multiple conversations. As you turn the corner there is Pamela in admitting and Gwen in marketing both are always willing to help anyone. Next is Vanetta and Christine the administrators, I have needed them and they have always been here to help me with life issues.Ive never had such personal treatment, like being given phone numbers in case I need to talk when they aren't here. WHO does that. Family does that. Next is Anna and Lovie (aka Luigi!). Both are as sweet as can be and they want to make sure that you're comfortable. Next is Mike, if you like football and the RAAAAAAIDERS then your good. Next is Irene the activity Queen! she always puts together a wide variety of theme parties as well as daily fun. Tell her she's a butterfly (she'll get it). Then next are my rocks George and Jimmy. These are your social services people. They make sure everyone is OK when you leave and your always welcome to come in and sit and just vent. Seems like they are friends rather than staff. Nurses and cna's Vanessa (she can Sing!), Lulu, Ivan (she cares about everyone, so helpful to me.), Manjeet (she's like my caring sister) the nicest lady I've ever met, Arcelie, Carla, Manjeet, and the rest, sorry if I missed anyone. The physical therapy crew. Irwin, Maribic, and Tony (Mr.Miagi), he is always pushing me to do better and I need that. Housekeeping staff is always cleaning. The place is near spotless 24/7. Food crew makes REAL meals, restaurant qaulity and Jennifer always makes sure you eat what you want weather your allergic or just don't like it. So what more can I say but that is that I had no family coming here but now I have not staff but a lifetime of FAMILY and friends. I would recommend Legacy to anyone. I'd say if you have a loved one that needs care, bring them here where everyone smiles. Trust is hard to come by and when I first got here I wanted to leave but and things came together. It's been a blessing to me. Thank you for giving me hope again. I feel like I have my life back. read more

I have a friend in this nursing home and visit her frequently. It's horrible. The food is nearly inedible. The place is understaffed so it is hard to get the attention of a CNA, let alone a nurse, if there is a problem. Today my friend told me that they are out of two medications she must have EVERY DAY. She is on a waiting list for 2 other places. I hope a place comes up before my friend dies of neglect. My advice is to avoid this place at all costs. I have no idea why there are so many positive reviews. But I am in touch with my friend everyday by email and I get the blow-by-blow account. I don't know about the other people but my friend is not senile so she is alert to what is going on. Some of the CNAs and nurses may be nice but that's not the whole story. The admin has been cutting back more and more, even since the last reviews in June. It wasn't too bad when my friend first came there several years ago, but 3 admins later, she's VERY sorry she did. read more

My mother was being discharged from the hospital after suffering a stroke and I had to find a rehabilitation facility in a very short period of time. At the very last minute I went to Legacy Nursing and Rehabilitation Center without an appointment. I was greeted by Rose Giron, Admissions Director. She could not have been more welcoming. I was extremely stressed and needed a rehabilitation facility asap - in fact my Mom was scheduled to be discharged from the hospital at 4:00 p.m. and I showed up at Legacy at 2:30 p.m. Rose gave me a tour, assured me Legacy would be happy to welcome Mom as a resident and helped me facilitate her transfer that afternoon. Legacy is very clean and the staff is wonderful. They went above and beyond to make my Mom and me feel comfortable. If you're looking for a good rehabilitation center - Legacy is great. From the staff to the residents, Legacy was a blessing to my Mother and to me. I can't say enough about how much I appreciate all they did for us. I can't say enough about this facility! I brought my father here from a manor care facility in long beach where he was being mistreated. After a long drive from the Long Beach area to Martinez, the Director greeted us at our vehicle and immediately referred us to the hospital. It appeared to the director that my father had scabies and she did not want to jeapordize the other patients. After a 7 day stay I discovered that not only was my dad infected with scabies but was also malnourished and severely dehydrated. Legacy assured me that my dad would have a spot at their facility when my dad was stable and healthy enough to return, and they kept their word. My dad spent 2 years at this facility and he was always clean, dressed, up and out of his bed and well fed. My dad did not have the ability to clearly communicate, but that did not stop the staff from talking to him and engaging him. My dad had also lost the ability to feed himself, so the staff would make sure to feed him and keep me informed of any changes in his eating habits. Let me just say, Legacy does not have all the bells and whistles, but if you want your loved one to be well cared for and to have that personal touch then I highly recommend this place. read more

Legacy is a very clean family oriented facility. Vanetta was the one who helped us get my mom in there. My mom was hesitant to go to a rehabilitation facility again because she had been to one before but once she got to legacy everyone made her feel at home. The facility is very clean and she said the whole staff there was so kind and respectful. My mom also said the food was really good there as well. My mom got to come home yesterday thanks to everyone at Legacy for working with her to get her health back. I appreciate everything you all have done for her. I was a patient at Legacy for two months, but I feel "patient" isn't the most accurate description. I think the word "guest" is more appropriate because that's what I felt like. I can't say enough about the entire staff there. The nurses, CNA's, physical therapists, dietary manager & staff, administrators & custodians were extremely friendly, cheerful, kind, caring, responsive, and knowledgeable. By the time I left, they were more like friends and family than Legacy employees. They truly care, work very hard, and do their best. I felt I was in good hands when I was there and I will be forever grateful for the excellent care I received. read more

My husband and I went to Legacy Nursing and Rehab to have a tour for my mom who is 87 yrs. old. I called to set up an appointment with them, the receptionist was very friendly and I am so happy that she approved my schedule. My husband and I came to see the place but we came pass our appointment schedule but 1 lady who seemed to be going home managed to give my husband and I a 15 minute tour. The lady who gave us a tour was very informative, she explained everything that we need to know from our mom's rehab progression to her daily activities ,her nursing needs and expense that her insurance will have to pay on a daily basis. My husband and I are glad that we came and met Ice. She is wonderful! The Nurse's in every station are friendly as well. If my family decided to transfer my mom back down here, I would definitely love to see her in Legacy. read more

I just completed my 4th stay at Legacy Rehab center. I keep coming back because it is such an exceptional nursing center. I was in this time to recover from major surgery and as usual the care I received was unmatched. In total I have spent over a year at Legacy so I am somewhat of an expert on the Rehab Center. I have spent time in another supposedly high rated nursing home and it didn't hold a candle to Legacy. The function of a rehabilitation center is physical rehabilitation and this place has an excellent physical rehabilitation center attached to the facility. It has an expert State Certified staff headed by Myla and including , Irwin, Tony and Marivec who all work with the patients every day, from young to very elderly with amazing expertise and patience. Marivec especially was one occupational therapist I was always glad to see each morning. She was always cheerful and was willing to work with me not when she had time but whenever I needed to. The PT staff at Legacy had me from flat on my back to walking again inside of six weeks which was no small task considering the degree of my disability. They also have an exceptional nursing staff at Legacy who constantly watch over their patient's overall health and promptly respond to even the smallest of health problems that might arise. The need for a band aid or a headache was not too trivial. I was particularly impressed with the kindness and caring of three of the nurses on the staff there, Vanessa, Zaharah, and if you want to see a stunningly beautiful nurse, take a look at Carla. Besides being really pretty Carla is a very competent RN. All three nurses displayed particular attentiveness and caring above and beyond the call of their regular duties. And then there is the CNA staff that the patients are most closely exposed to from day to day and who can't be praised too much. These Certified Nurses Assistants scurry from one room to the other all day long tirelessly tending to the patient's wants and needs and bringing them coffee whenever they ask for it. I have to mention a few of the CNA's who stand out. Myrna, Mandeep and Arcellie who are particularly sweet and attentive and they keep the patients clean and comfortable every single day. Another CNA Manjit Kuur deserves special recognition. She is a consummate professional and attended to me all evening long day after day like she was my own mother. And then there is Lulu who was so professional and sweet and attentive to my needs I pleaded with her to marry me several times. I could not believe her energy and tireless ability to do her job all day long. It was obvious she loved her job. Legacy is such a well run facility, the housekeeping department, the laundry and the kitchen all run smoothly every day. All departments down to the very capable and disarmingly cute receptionist Ivy Francisco who greets guests and visitors with a smile every day as they come in and directs them where they need to go. Ivy was my go to girl whenever I needed something. She was always willing to help me any way she could by printing out a document for me or finding someone who I have lost track of among the 300 plus people there. She seemed to know everybody and was always there to help anyone. Employees like Ivy are the reason why Legacy rehab hospital is such a great place whether you need a facility to recuperate after an injury or a safe place to put your elderly father that you can trust completely and know he will be safe and clean every day and well fed and offered activities. There is an activity room where the patients can play cards, bingo, board games, do crafts and artwork and participate in group singing and many other activities. I couldn't recommend Legacy Nursing and Rehabilitation Center more highly. read more
