The kind that unfortunately you spend the rest of your life dealing with in one way or another. In retrospect, things could have been much worse, but when the girl standing next me during a tennis doubles team swung a little too hard, both of my front teeth were compromised. I was overjoyed when I finally got a dental implant in 2016, restoring a smile that I didn't love for the previous for 14 years. Then late last month, I bit down on a nice piece of pork loin, and my crown fell out. Since I originally had the implant done in NYC and I have since relocated back to the west coast, I reached out to my general dentist in the Bay Area to the most competent prosthodontist he knew. The good news from my local prosthodontist was that only the screw had broken, not the implant. The bad news was that he didn't have the training or tools to remove the broken screw. The material and precision required made it a challenging task, as my implant system is 100% zirconia, meaning both the screw and implant are ceramic. Even a slight error in retrieving the screw could fracture the entire implant, necessitating a complete redo. While in the dentist's chair, I frantically Googled who could extract broken dental implant screws for my specific implant system (Z-systems). When most results were research papers and YouTube videos aimed at other dentists instead of the normal marketing language, I realized I was in deep s***. Fortunately, I stumbled upon a blog post by Dr. Mastrovich, detailing a case study with his technical assessment, algorithmic approach, tools, and techniques used, and evaluations of different approaches, especially those from previous extraction attempts by other dentists. When I got home, I researched further and discovered that Dr. Mastrovich has a 25x microscope, has handled 1300 of these cases, is well-versed in many permutations of the problem across various implant systems, and stays updated on the latest developments in the field, including an AI-based implant identification app mentioned in one of his recent blogs. I also noticed a Nikon D850 setup in one of his office photos. As a hobby photographer, I recognized the weight of that camera, which told me Dr. Mastrovich doesn't compromise on his tools. I reached out immediately to book an appointment. A few days later I learned that the prosthodontist in the Bay Area had given me the wrong specs! I didn't have a screw but an abutment! Dr. Mastrovich independently discovered this by contacting the Z-systems manufacturer. I knew then and there this was the right best person for the job. Long story short, I had the abutment retrieved a week ago successfully, with no damage to the implant. I also learned that Dr. Mastrovich has taught himself to make many of his tools. Over the past two decades, I've encountered many dentists, some of whom are long-time professors at prominent dental schools like UCSF and UCLA. It's clear to me that Dr. Mastrovich operates on first principles and is a pioneer in his field. In my view, he is someone who teaches those who teach others. In an effort to replace the abutment and crown post-extraction, I've also reached out to over 30 dentists across the US and Canada. I could probably write a blog on the comparative differences between dental offices, but one surprising observation is the frequent lax attitude towards HIPAA. The number of times I've seen prominent dental offices using @gmail accounts for patient communication or back-channeling patient records without explicit consent is shocking. When you're dental implant needs a tap and die set! Had implants done in 2009 by another doctor. Hard to say if it was the material or work but the bolt that is screwed into the implant (think of a nut) broke. Implant is fine, but bolt holding the tooth in broke, very odd. Dr. Mastrovich clearly doesn't drink, his hands were crazy stead to back out a screw that but buried in my jaw and less than a MM. Also this broke off 5yrs ago so it had all kinds of build up calcium build up. Wasn't cheap at $880 per hr, but he's the Mcgyver of dentistry, making his own tools for unique repair. He had it done in less time than quoted which saved me money. Hope to never see him again:) But if I have any other problems he's the man. BTW, very friendly and communicative staff. Off is immaculate, you can tell the man is OCD as he washes his hands and dries the sink after, exactly what you want when you have an issue like mine. read more

Dr Mastrovich did what no dentist had been able to do with the issue I had with my dental implant during the last few years - he took out the screw that was stuck in the implant. He did it with the special tools he had made for this specific job! As a result, he saved the implant, which is a huge deal, as otherwise it would have required to be extracted, followed by new bone grafting and re-implanting (people who have gone through the process of getting dental implants would know how much time and pain and expense that entails).... My general dentist couldn't get the abutment off of my implant so he referred me to a prosthodontist to get it off. The prosthodontist told me he had only done a couple of these but he would be "very conservative" in working on it in case he wasn't able to get it off. I was desperate to find a solution for an old implant that needed a new crown and abutment. I had been to four "implant specialists" who literally told me they could not help me! I was on line searching and found Dr. Mastrovich. It appeared that maybe I had actually found a real specialist. I had an initial virtual appointment to describe my issues and I then went in for my first appointment. I knew from that first appointment that I was in the best hands! Dr. Mastrovich knew everything about anything having to do with implants! He had equipment that no other dentists have to determine what brand of implant I had and if he was not able to identify the brand, he was able to make the abutment that I needed!
