Staffing appears to be understaffed, with some dedicated caregivers and some who seem to be there to act as prison wardens, or sanitation workers who regret their status in life. Case managers are clipped, and not easy to communicate with, and my spouse was seldom ever offered insights by real nursing staff. She was quickly fitted with a catheter, apparently to save staff from having to help her to the bathroom and back. Physical therapy was hit or miss; in the first two days she was eligible, and was skipped by the staff. One of the physical therapists was probably 100 pounds overweight; and the physical therapy she did receive was minimal at best. The physical therapy department is not open on the weekends, so Saturday or Sunday PT is not a possibility. Food was passable to worse. My spouse was in a shared room with another client, a woman who loved to have her TV on late into the night with the sound at 100 DB - never did Staff come by to suggest the sound be turned down - giving my spouse earplugs which were little help. And result of the substandard nursing environment was that we checked out early, to the chagrin of several on the staff, who said "we were leaving against medical advice". We were pledged a referral for home-based nursing, but it took several phone calls and several days for Staff to file that paperwork. Overall grade, D. We would not recommend this place.

    Impressed. We all…read moreknow the stereotypical negative reputations that go along with care at Skilled Nursing Facilities, but this place actually cares! My husband was transferred here from a rehab facility to receive rehabilitation for a below knee amputation. He was at deaths door, practically, when he was admitted. Incredibly weak, out of it, at times lethargic from the care he had just escaped from. At American River Center, it's lively, clean, the staff is exceptional, and above all the physical therapy has been the best my husband has received the whole year of this newly experienced amputation ordeal. Who ever oversees the management of this facility, my hats off to you! It smells GOOD in here, they take care with organizing the menu, they provide weekly newsletters with the menu to their patients, and you can make requests to the menu. They allow family or visitors to come and go. The staff is very friendly and very attentive. The facility is always being maintained whether it's inside or out. I really just don't have enough nice things to say about it here. By the time my husband was discharged he was able to walk with assistance of a rolling walker. Compared to when he arrived needing 3 people to help him out of the car, there was literally no need for help upon discharge. Physical therapy helped him reach his goals. I am so grateful as his caregiver! Above all, I felt comfortable knowing he was in good hands and never worried about his cared. This is saying alot for SNF's!!! I'd give more stars if I could, but 5 it is!! Thank you American River Center!

There is a clear disconnect between the kitchen and the other staff at the moment but the rehab. staff is mostly very good and as they don't have long term care they do seem make a genuine effort in that area at least. I'd recommend using it if the need arises, and keeping a close eye on it while you do.

    Absolutely horrible! Worst place to send someone needing…read morecare. Staff is inexperienced doctor does not care for patient, staff are rude and slow patient I'm concerned with has gotten a horrible life threatening rash and bed sores on a water restrictions yet has gained over 40 lbs and is getting worse! Sent to this hell hole by Sutter Roseville. Good luck trying to talk to someone there put on hold 44 minutes then disconnected. On hold again 22 minutes and the staff can't even answer the questions! Horrible horrible horrible place! 0 stars is what they should get and the place shut down! DO NOT SEND A FAMILY MEMBER THERE!!!!!

    I was there twice for a total for close to a year, looking over the other reviews I wanted to add…read morein my account: I HEAR that this place has a contract with Kaiser, and they seem to get the most difficult and undesirable patients. What the person said about the woman who walks around in and out rooms day and night is true and she steals food constantly from rooms and from the snack trays, those trays are for after meal snacks and she steals them, I guess that explains why I haven't gotten mine sometimes. Yes the staff knows and sees but you can't do anything about it, you can yell and scream at her, doesn't do any good. It wasn't unusual for someone to park their wheel chair in a hallway and start screaming and yelling for about a half hour, then move to another section. Overnight screaming was frequent and sometimes sounded violent many of my roommates didn't seem to know where they were and why, some went through mood swings depending on their medication level. Parking is atrocious between visitors and transports blocking all the spaces... good luck! The food was worse than normal my roast beef dinner had a half eaten chicken wing bone in the middle. I tried to save it but a CNA threw it away before I could complain to the kitchen and when I did of course they didn't believe me. I ordered out a lot I had never met the Doctor until months later on a Saturday when he was like running from room to room asking you a question and before you could bring up a concern, he was out of there. Yeah it's hard to get someone to answer the phone and if the receptionist is gone...forget it the person who does barely knows how to work the phone. Okay I may need surgery and despite my misgivings I may go back Why? Because the nursing staff is really nice and works hard despite the shortcomings. The therapy staff is tops with some of the smartest people I ever met, I often wonder why are they at a place like this or is they aware of the talent they have? Over the years I've been bounced around to a few places and I'm sorry to say it doesn't get much better than this

    I just wanted to follow up on my first review of 6/17/26. My father has now moved out of his…read moretemporary home at Mission Carmichael after a roughly 90 day rehabilitation. Not only was his rehabilitation very successful - going from transferring only by lift to minor assistance and assisted walking - but he genuinely appreciated the staff who I believe made a big difference in his ability to regain more independence. I would like to thank a number of people and I am sure I will miss some but most everyone was helpful and encouraging for him. Tim made him feel welcome from his very first day and always followed up any issues. Raul who handled his PT was extremely encouraging for him and made a huge difference in his daily progress to walk. Miriam and Sandy who handled his OT were equally encouraging and always came up with different ways to do things. Ana brought him coffee and uplifted his spirits each morning. Manjit and Keziah handled his medications and were very kind. Hanah and Theresa were two of many CNA's who were very helpful and would periodically check in with him. Vladimir and Lowell were "gentle giants" who assisted him. Emily always seems to be helpful during lunch. Tina the front desk receptionist was always cheerful each morning. Lilian (the case manager) organized his discharge working to make a smooth transition into his next home - she was invaluable in educating me on medical issues and looking out for his best interests! They all worked as a great team (I know I have forgotten some people and I apologize to them) to create a successful rehabilitation. It is not easy doing what these folks do but somehow they kept their own spirits up as well as his and he will miss all of them.

    I am changing my review due to an experience with their business office after the fact. Turns out…read moreafter all was paid by insurance she owed $150.00. She received a bill at some point which she probably opened and didn't mention. The last month I didn't know my mom had progressive dementia along with other issues that have placed her back into another skilled nursing facility. What happened to really upset me was receiving a certified letter addressed to her with the $150.00 past due notice. To me this is ridiculous waste to send to people on Medicare and who are disabled. I spoke to a person in the business office explaining my mom's condition, my disappointment and to direct ALL COMMUNICATION TO ME. I thought it was pretty clear that day and I wouldn't get a call until the next month for payment. I got a call one week later asking where to send the statement. I don't like writing reviews like this one but this goes beyond what I consider poor business practice and a waste of facility money.
