    My mother was a resident at this facility in November of last year, where she ultimately passed…read moreaway due to dementia and other serious health conditions. During her stay, the quality of care was inconsistent. While a few nurses went above and beyond in caring for my mother, and for that my family is truly grateful, many others appeared unwilling to do more than the bare minimum required of their job duties. Thankfully, my siblings and I were able to step in and help fill the gaps in care that we felt were lacking. Toward the end of my mother's life, there were times when it felt as though she was being ignored rather than compassionately cared for. What was most upsetting was the way our family was treated after my mother passed away. We were not given adequate time, privacy, or respect to grieve before her body was removed from the facility. That experience added unnecessary pain to an already heartbreaking moment for our family. In addition, Deborah, the Director of the facility, was consistently rude, unprofessional, and argumentative in our interactions with her. There was also an incident involving another patient, where decisions were made before the full situation was properly investigated, resulting in my mother being moved to another side of the facility. Based on my family's experience, I would not recommend this facility to others seeking compassionate and respectful care for their loved ones.

    I'm not sure why so many complaints, but I froze my account due to covid and limitations and I have…read morenot been charged a dime since 4/1/2020. I've been a member since they opened in 2008. It's actually not just a gym, it's a wellness center and is not for everybody. I toured the new Health-quest and it was nothing compare to Synergy. I toured the new In-shape, and I walked in and walked out!!!! The towels they give you for showers are rags that I wouldn't even use to wash my car with!!! I tried the $10 a month gym in River Park and well , that's why it's $10 a month. Synergy is the cleanest and most sanitized place to work out that I've ever been to. It's part of the Queen of the valley , so it is constantly mopped, wiped and dusted and sprayed and cleaned, around the clock.... and that was before Covid!!! The class instructors and staffs are the politest and most courteous and patient and fun and so are the members. If you're not happy working out at Synergy, Well, you're just not a very happy and satisfied person, and that , nobody can fix but yourself....... try other places for a day or so, and you will appreciate Synergy more.

    After being closed for two weeks for improvements (we were still charged for our entire membership…read morefee), I returned to find that I am disallowed from using the new equipment without setting up a 45 minute training session. However, there are no appointments available. For this month or ANY future months! Beyond this, part of their ' improvements' included removing the only machine that can be used for lifting heavy weights for glutes. I hope management sees this and can explain the logic in removing the only machine available to build strong glutes, which inform so many other functional areas of our mobility. Please explain how this is an improvement!

    I would like to say how much I appreciate Meadowood Nursing Center in Clear Lake for taking such…read moregood care of my mom. She was recovering from a fall, and she had a great team helping her, the nurses, and PT, and staff were very kind people that helped her get back on her feet. Christian was the first person I spoke to at Meadowood, he helped with getting her admitted and he always answered the phone if we had any questions. And Peggy in the billing dept. was awesome, she was very helpful with all the paperwork, and she is very resourceful and said if I had any questions in the future that she would help anyway she could. I really enjoyed working with Francine the case worker, she gave me progress reports on my mom, and I appreciated all the work she did. Nurse Taylor is amazing, she went the extra mile when I needed some help, and she explained in detail how my mom's care was going. Nurse Wendy was the first nurse that helped my mom, and she was lovely and was so happy for all the progress my mom made. And a big shout out to the people in the dining hall, Kyle and Barry were so sweet with the residents I watched them talk to all the residents and they were very caring and funny and walked around to make sure all the residents were happy and taken care of. Even offered my husband and I hot chocolate. I am writing this review because my mom told me how nice and caring everyone was to her at Meadowood and that they were always patience with her which means so much to me, and I knew I had to write a big thank you to them. People that work at nursing homes work very hard and sometimes do not get the recognition they deserve. There are so many people in nursing homes that have special needs, and I saw firsthand how hard everyone at Meadowood work and I just want to tell them how much my family appreciates them.

    Elder Rights | False Imprisonment | California Law Meadowood…read morenursing center Clearlake California. This statement is being made to document and publicly address serious civil rights and elder abuse concerns involving Meadowood Nursing Home in Clearlake California, and the response of local law enforcement on January 28, 2026. My family member was admitted to Meadowwood voluntarily. He is not conserved, not under court order, and not subject to any psychiatric or medical hold. He has been there for months wanting to leave. On January 28, 2026, he attempted to leave the facility a right explicitly protected under federal and California law. That same day, he underwent cognitive testing and passed with full capacity, demonstrating that he was alert, oriented, and capable of making his own medical and personal decisions. Despite this: * Facility staff physically blocked exits * Staff placed their hands on him and pulled his arms to prevent departure * Family members attempting to assist were interfered with * Law enforcement was called solely to prevent him from leaving They have locked doors preventing family members to get in and for him to get out. At no point did Meadowood staff present a lawful detention order, conservatorship, or physician-initiated hold because none existed. Under California Penal Code §236, false imprisonment is defined as: "The unlawful violation of the personal liberty of another." False imprisonment does not require locked doors or incarceration. It includes physical restraint, blocking exits, intimidation, or coercion. Under 42 CFR §483.10 (Federal Nursing Home Reform Act) and California Title 22, residents retain the right to: * Leave a nursing facility at will if not legally restricted * Refuse treatment and placement * Be free from physical or chemical restraints * Be free from coercion, intimidation, or retaliation * Maintain privacy during visits A nursing facility does not possess authority to detain a competent adult simply because it disagrees with the resident's decision. Administrative inconvenience does not override constitutional liberty. When Clearlake police department arrived, family members cited California resident rights law and Ombudsman protections. These were dismissed as "not true," and my family member was compelled to return inside the facility. This response is deeply concerning. Under Welfare & Institutions Code §15610.07, elder abuse includes: * Physical abuse * False imprisonment * Neglect * Mental suffering * Deprivation of rights Law enforcement intervention that facilitates unlawful detention even unintentionally may enable elder abuse rather than prevent it. Training deficiencies in elder rights law place vulnerable adults at risk and undermine the purpose of protective services. During his stay, our family documented the following concerns: * Severe understaffing * Termination of the facility physician with no consistent medical oversight * Repeated requests for UTI testing ignored for more than two months * UTI confirmed positive once finally tested ( with threats of us, bringing him to the ER ourselves ) It is well established in geriatric medicine that untreated UTIs can result in delirium, sepsis, kidney failure, and death. Additional observed conditions included: * Lack of privacy during visits (staff monitoring) * Prescribed medications not consistently administered * Unsanitary conditions and persistent urine odor * Excessive delays responding to call lights * Residents discouraged from walking in favor of prolonged wheelchair confinement * Meals served cold and appearing below nutritional standards These conditions raise serious concerns regarding compliance with state licensing and federal participation requirements. Facility staff repeatedly told my family member that if he left: * He would lose Medi-Cal eligibility * He would be personally responsible for payment This information is factually inaccurate and appears to have been used to discourage discharge. Threatening loss of benefits to prevent a resident from leaving may constitute coercion and financial abuse, both prohibited under California law. Elder rights are not optional. They are not discretionary. They do not disappear at the doors of a nursing facility. A voluntary resident is not a prisoner. When facilities restrain, intimidate, or misrepresent the law to keep residents from leaving, that is not "care" it is unlawful confinement. This statement is made for documentation, accountability, and public awareness. Families deserve to know their rights. Residents deserve dignity. And institutions entrusted with elder care must be held to the law. Every staff and CNA involved should lose their licenses and never be allowed to have elderly in their care again. This is abuse!

    Dr. Jacqueline Weisbein has a very aggressive bedside manner. When she came in the room (first time…read moremeeting me) she sat down with her arms crossed and said, "So what's wrong with you?" I just had major surgery to have stage 4 endometriosis removed from multiple organs. I even had my appendix removed during the extensive abdominal surgery. She seemed uninterested in my post-operative report. My insurance company dropped me two days before the surgery and she yelled at me to stop crying and told me to get insurance. I did, & then her office was very rude to me over the phone & they didn't even call to confirm my appointment beforehand. I have over 20 years of medical trauma from being misdiagnosed with depression and anxiety when in reality I have had one of the most painful conditions. Dr. Weisbein's demeanor completely changed when her next patient (an older male) walked in and she quickly changed her tone and said, "hello my love!" to him. Bottom line- ladies!!! Don't go to this woman. She's mean. *I almost forgot to mention, when I told her I was in a car accident, she said it was against her policy to talk about more than one injury at a time. *Go to Dr. Eric Grigsby or Dr Melanie Johansson if you need real help and you are a woman. They are experienced and non-biased unlike Jaqueline. *Thank you for your apology for your aggression & I hope you do better for future patients. I am finding better/more experienced/better equipped doctors for my needs.
