After watching a couple of videos regarding Light Adjustable Lens, I opted to have this done. I opted to do monovision so that I would never have to wear glasses again. I would hopefully see both far and near. I had the surgeries done about a month apart at his surgical clinic. The surgical clinic's staff creates a very clean and comfortable environment and I felt at ease. After the surgeries and after "locking in" the final adjustment, I now have 20/15 vision in my dominant eye, and I can read the smallest print on Dr. Kootman's card. It is amazing. Then I see in my Medicare claims afterward that they indeed did successfully bill the same service to my insurance. That is double-billing. They got me for $200 while paid also by Medicare. Their explanation is that it was two different services because they claim there was a skin-tag among the keratoses growths that were frozen. To them, that justifies a $200 cash charge. In my opinion, this is unethical and profiteering. I reported this to Medicare as improper billing and opened a complaint/appeal. I have since moved to another practice that operates ethically and honestly in their billing. Be very careful with this dermatology practice and pay attention to all the bad reviews as they exist for a reason. Be sure to heed these reviews and not the reviews over on G, because at G, businesses have methods to delete reviews they dont like, which is why you'll often see nothing but 5 star reviews over there while the reviews here are poor. I am going to take this business to small claims court for recovery of the improper cash charge. I requested the name of who they would like me to subpeona to Justice Court, as I must have a person's name who will appear to represent the business, and of course, they just give me the big run around having me call another number who told me to call back to their original number, finally getting called back by a rude manager who continually spoke over me and refused to give a name to have my process server present my subpoena to. This is nothing more than obstruction. So, I will subpoena the apparent managing partner, Dr Anthony Petelin, and will lodge a complaint at the Arizona State Medical Board.
