Highly recommend this nursing home to anyone that is considering. My grand ma was referred here by hospital in Santa Cruz for additional rehab and nursing needs. The nursing team and rehab team were very kind and patient with my grand ma. I really appreciated that. They constantly gave me update what was going on and it was very easy to communicate with all the team The facility was clean. Any concerns I had it was swiftly handled by the team. I say this with great sadness My mothers best friend Sandra was a patient there for some time  she passed away I was told that the workers there threw her things outside for "more room" ? but was left outside  and completely destroyed by the weather I went after for my mother to see if we could collect maybe anything left behind that they didn't return. When I got there and came inside there was no one to greet me so i asked a nurse, she told me look for some girl named Cristina from activities the nurse said that she would be able to help that they were outside. When I went to look there was a lot of people in scrubs smoking marijuana pens and laughing I can't imagine staff working high around residents even worse nurses going in taking care of patients high I do not recommend anyone send a loved one here to get better read more

Poor quality of care by the Physican in charge and his nurse practitioner. Never saw the physician and NP stopped responding to my requests. Went to the hospital for development of PE after surgery, then 2 additional times I told them to send me to the hospital. First time, I developed a UTI, then returned to rehab; second time, I developed a kidney infection and was admitted to the hospital for 5 days. Told the hospitalist when I was ready for discharge, to find me another rehab as I refused to go back to this facility. Found another wonderful place in Los Gatos where my care was continued. Would not recommend Pacific Coast Manor to anyone needing rehab or otherwise. While there are a few good caregivers, the majority are not. Whichever government agency that oversees care in nursing homes/rehab facilities should investigate this facility. read more

I give Pacific Coast Manor 5 stars. It has been such a transition for my 96 year old Mom who is here for long term care. It was a hard decision but ever since I first step foot into the building, I was greeted with warm smiles, respect and the place was clean, smelled nice and the staff answered all my questions. The care is attentive and I feel she is in a good home for her next phase of life. Mallory and Rafael are dedicated to what they do and it shows. They are professional and compassionate. All the employees get along and have fun together, you can tell the environment is positive and cohesive. I highly recommend this facility if you're in need of long term skilled nursing or rehabilitation. read more

My wife has been here for four days now because she had hip replacement surgery.When she calls for help to either get out of bed to go to the restroom or she needs her meds or wants to get in her walker or wheel chair or she is in pain the response is 20 minutes later or even three hours later help actually arrives.Not very professional. One time one of the aids Tore off he band aide that was on her surgery .Complain to staff several times same results.Her 96 year old roommate ask for help in getting out of her toilet and finally my wife yelled for help and then they finally came.During the day time i noticed it's very loud ..Wouldn't recommend this place for anyone. read more

My mom went to this rehab to recover from her hip. It was a nightmare for her. She has dementia and put her in a room, which was loud chaos going on in the hallways. Nobody coming when she had to go to the bathroom. She called me 17 times during the night and then she called 911 to get help. I had to take her out the very next day. This was a very bad experience for my mom and she ended up back in the hospital in the Bay Area to finish her recovery before she came home. read more

Having broken my leg earlier this year followed by surgery then rehab I wasn't sure about Pacific Coast Manor for rehabbing my leg as all the reviews I saw were of the terrific care for elderly patients. I found out soon enough my answer. Other than the round the clock care for my well being I found the Physical Therapy department better than I expected. Thanks to Liz, and Ed I was able to go home sooner than later. I would high recommend PacManor for rehab following surgery. Night shift medical staff checked in on me as well throughout the night. This place is such a gift! After my mother-in-law had hip surgery, she spent a day and night at a different rehabilitation facility and it was awful. We had her moved here and it was a completely different experience. The staff are so caring and attentive, always making time for people and ensuring that everyone has what they need, which is not the case at other places. Plus the environment itself feels so welcoming and home like, with pretty areas to sit in front and in the courtyard. My mother-in-law was here for almost a month, and we will be forever grateful for the wonderful care that they provided. read more

I have been told by THE EMPLOYEES of Pacific coast manor, and Dominican hospital, that PCM is the number one place to go, BUT, I have been there recently and I have spent much time on 2 different occasions at Santa Cruz Post Acute and I want to say, with experience, that Santa Cruz Post Acute is NUMBER ONE! I had to sign myself out of Pacific coast manor against medical advice, AMA after 14days. Terrible food, and not a good place to stay. I hope everyone will hear my thoughts and heed my advice. This 5 star review applies strictly to the people who "walk the floors" from the housekeeping staff to the nurses and to the Physical & Occupational Therapists. It does not necessarily apply to the doctor or administration about some of whom I have serious concerns. The people who walk the floors are almost universally kind, encouraging, hardworking beyond belief and well beyond what should be necessary. Over and over again during my almost 1 mo. stay here, I talked to employees who had signed up for a double shift, 2 eight hr. shifts, one after the other. They walked in with a smile and 16 or more hrs. later, they left with a smile. The reliance on this, or even the expectation of it, is inhumane. The explanation I received for volunteering to work this many hrs. could be summarized as, "This is the only way to give the patients the care, attention, and affection they need to recover." They are amazing people who deserve my respect and deep gratitude. I also enthusiastically extend my gratitude to Braden, my PT, and Katie & Nihada, my OTs. Behind the façade of tough taskmasters are 3 kind, loving, and extremely talented people who share their hard earned knowledge with their patients. Their goal is that, through therapy, the patients will be able to leave Pacific Coast Manor as quickly as possible. I can't imagine where I would be without their help. I will always be grateful. Within the administration, I do give credit to Social Services. Any lack of a prompt response to my concern can easily be explained by saying that they have too small a staff for the volume of patients. My PCM doctor was supposed to see me within 24 and 48 hrs. after my admission. I saw him briefly on my 4th day here. I did not see him again for 3 1/2 weeks. That was yesterday when he came to say that I was being discharged then. I told him that I wasn't being released yesterday. I will be discharged day after tomorrow. In closing, I have a question. Who is/are responsible for the admission of patients with serious dementia? Patients who have to sit in the hallways where they must be constantly watched by the nurses, CNAs, aids?! Patients who sit for hours starring blankly at the floor or walls, but who suddenly stand up without the support necessary for their safety and who run the extreme risk of falling? This facility is obviously not equipped to adequately help and protect these patients. God bless the nurses, CNAs, and aids. They try their hardest which just adds to the burden they already have, which contributes to 16+ hr. days! With 5 stars at the beginning, I end with no stars at the end. I have been to PCM twice. Their staff are absolutely THE BEST. Their administrators, on the other hand, routinely violate patient rights that are guaranteed by the Code of Federal Regulations. Also, their physical therapy program is woefully inadequate. read more

I got here from the VA Palo Alto at 5 pm, 5/12/22. They were supposed to order my medication & it's now 10:08 pm, and no medication. They said they haven't come in & won't be in for another few hours. WTF! I just had my left hip replaced on Tuesday, & I'm in pain. Oh well, they are in the halls yucking it up! I was incredibly fortunate that my nearly 90 year old mom was placed at Pacific Coast Manor (PCM), nine months ago after a fall. I cannot find enough words to express my gratitude for the incredible care and kindness they have given to my mom during her stay. Everyone from the people at the front desk, the cleaning ladies, care teams, activities leaders, placement leaders, to the management, nurses, and doctors. She thrived there and left in better condition than she had been in for many years. She was groomed, nourished, treated, and much stronger than when she arrived. She can be difficult and has a lot of particularities, and they always found ways to work with her. The staff at PCM have big hearts even with the constant demands they are under. I have so much admiration and respect for them and all that they do. The facility is always clean and buzzing with positive energy and activities. I highly recommend PCM. read more

We hope to never have to use a rehabilitation facility whether it's for ourselves or our loved ones but life has its ups and downs. During the past month we ended up having to take my father to rehabilitation after an emergency stay at the hospital. We were lucky enough to have found Pacific Coast Manor in Capitola. My first contact was with Rafael and from the beginning he was very helpful and very nice to talk to. Along with everyone else that we had to talk to from Cinthia and Shannon and everyone else I am forgetting. But in general all the staff is great. The facility was clean and maintained very nicely and the girls in the front always greeted us with a smile and were very helpful. My father had a little over 6 week stay for rehabilitation and was treated very nice. They made sure to the last day that he was ready to go home safely and healthy. We are so grateful for everything they did for him and the support they gave us during the time he was there. I told Rafael in the nicest way possible I hope to never see you guys again but if we do you guys will be it. So if you ever find yourself in need of rehabilitation for yourself or a loved one please do call them first. You will be treated like family. i recently had the occasion to start inquiring for an assisted living facility for my mother. i was referred to pacific coast manor from a friend. i called to inquire about details and was fortunate to have misti cox be on the receiving end of my phone call. misti took time to discuss the facility and iit's merits in detail with the patience of an angel. i had so many questions !!! she took her time and addressed all of my concerns. when our family does make a decision, pacific coast is first on our list. if misti is an indication of the staff, i am sold!!! what a kind, helpful lady! she is a rarity in this fast paced world.!!!!! read more

My Mom was in this rehab facility and I thought everything was going almost smoothly (had a pin put in her hip) and she had developed a U.T.I. and was receiving antibiotics, when she had to use the restroom more frequently the aides would scold her (during the late evening hours) saying that she just called them. My Mom would have been released prox. Oct. 30/31. Now it is Nov. 16 and we are still in rehab at another facility. As a result, my Mom fell and broke her hip bone, because of their non-attention (not exactly known what time of day it happened) and she was readmitted to a trauma center as her next surgery of her leg was considered, "complicated'". Dr. Potts, in charge of her care,(Pacific Coast Manor) did not "disclose" the actual reason for her return to the E.R. but the E.R. doctor did disclose her return was because of a "FALL". read more

I probably lost count of the multiple admissions my sister, Carolyn, experienced with this facility. Skilled nursing facilities are not known for being " well liked". We just happened to " win the lottery" with this one, this place in time. Rochelle needs to be acknowledged here for going above and beyond with requests on my sisters' behalf, that would have most admission staff wincing. We had made several requests over the many months. Each time our wishes were granted. The staff here cared so much for my sister, in the end, many of them were seen dropping in to say "Hi".... I highly recommend this facility. I have had the privilege of working at Pacific Coast Manor for four years. The dedication and compassion of the management and care staff is superb. Situations are not always easy to manage, and solutions are not always in direct sight but the staff works hard at problem solving and making sure that residents are safe and loved. Pacific Coast Manor is the best skilled nursing facility in the county for a reason, and their report card to the community shows their accomplishments. I had to have a couple joint replacement surgeries and was unable to care for myself after surgery. A nurse at Sutter recommended Pacific Coast Mannor as a recovery place. I'm in my 30's so I thought I would feel out of place....boy was I wrong! From the moment I got there I was welcomed by not only the staff, but also the patients. There are two beds to a room which worried me at first, but it turned out to be an amazing experience. The staff are all so professional and caring. If there are any issues they are corrected immediately. The facility is very clean and even the food isn't bad. I was actually sad to leave because everyone from the administration, the nurses, the CNA'S, the physical therapists, and even the other patients were so kind and made you feel like family. If you're looking for short term or long term care, for yourself or a family member I highly recommend Pacific Coast Mannor! read more

In just 9 hours I was fed a hamburger helper horrible meal. Did not get pain meds "on time" twice in that short period. After TKR surgery I was abandoned on a walker ...nobody around for 20 minutes after waiting 20 minutes to get to the bathroom. I had to beg someone from the other wind to help me into bed. My service light was on and aids were walking by but didn't even bother to see what was up. Then the medication guy wouldn't even help me get into my bed....somebody else's job he essentially informed me someone would be there soon....NOT! I finally called friends at midnight to get me the hell out of there! Wouldn't put an enemy in this place and it is supposedly the best there is in Santa Cruz...disgusting. Felt like I was in a war hospital waiting my turn. Absolutely horrible! read more

although very reluctant to go into rehab after my back fusion I found it necessary. I was pleasantly surprised by the level,of care available. I received physical therapy and occupational therapy every day and it was a tremendous aide in my recovery. my favorite part of the experience however was the staff. to a person they were always cheerful and smiling. even though understaffed like everywhere else these days they did their very best to accommodate patient requests. I was there a month and I grew fond of most of my caretakers. I saw day to day what a difficult job they have and their continued good humor is inspiring. the nurses too were efficient and timely in dispensing meds etc and with a smile. it is an institution noisy with poor food but the staff makes it as pleasant as possible. hopefully, I never have to go to rehab again but would certainly go back there. I can honestly say this was one of the worst facilities i worked at as a CNA. The overworked shifts and favoritism that happened here was unreal. I would be left alone with a whole floor as other CNAs would go outside to the smoking area and completely abandoned the lights. I would get pulled to assist residents by my RN with a big attitude because the lights would be going off, knowing i was the only one on the floor. All this also at the peak of the pandemic. The residents where always my concern. If my family member was left there i would make sure to go on night shift to check on them as much as possible. read more

I took my mom straight here from Stanford on 1/30/2020 after a knee revision surgery. Mom could not walk on her own and needed a wheelchair AND a walker, so the helper they had left us out in the freezing windy cold while they took 15-20 minutes to locate the items in need. During this time, mom had to go to the bathroom REALLY bad (she did tell this to the girl BEFORE leaving us to find the items) Mom had a total panic attack while waiting. Finally we get in, get her room, and there's 2 ladies in there making up my mom's new bed. Mom still has to go to the bathroom. The one lady says to both my mom and I, "You two..OUT--We trying to make your bed". I got pissed and said, "hey! look, she HAS TO GO to the bathroom". Then everyone leaves! Where's the help? She can not do it on her own! UGH, so I had to flag someone down and get them to help. Then, (I wasn't there for this part, but mom told me) One night (right before she was ready to be discharged and go home) a night nurse, whom she can not remember her name-something like Lola or Lulu.. came in to her room totally sicker than a dog. Hacking and coughing and just being pure gross. Waltzes up to mom, WHILE PICKING HER TEETH -no, no gloves, no no washing her hands after. My poor mom got immediately sick right after that. Then, she (mom) gave it to my brother, and myself. What kind of circus are they running there? I do not recommend this joint. Disgusting. read more

Supposedly Pacific Coast Manor is the best skilled nursing facility in Santa Cruz. I stayed there for almost a month in the rehab section. The staff are very friendly and hard working, but as for probably most skilled nursing facilities PCM employees are definitely overworked and understaffed. I wouldn't have been able to survive on my own at home after my surgery, and wonderful Rochelle bent over backwards to ensure I had a bed (and my own room at that)! The help I received by nurses and aids was amazing. Big thanks to nurses: Tanya, Beverly and Somer and aids: Joy, Ann-Ann, and Rachel. I did have to advocate for myself quite a bit to ensure I was receiving the care I needed. Chelsea, my OT, was a godsend for helping advocate for me. I can only imagine what it would be like for patients less clear minded than I am, who cannot advocate for themselves. The physical therapy team dropped the ball and discontinued PT the second I could walk unassisted, when I had many many other issues I needed PT for. I must say that the current doctor at PCM is a complete buffoon. I was carelessly misdiagnosed with pneumonia when in fact I had a dangerous infection in my abdomen, that required hospitalization and IV antibiotics. I had ZERO symptoms of pneumonia and EVERY symptom of an abdominal infection. The facility wasn't as clean as I would have expected. I had to bring my own garbage can so that soiled wound dressings could be thrown away instead of piled on the floor or my bed. I had to ask that cups of my abdominal drainage not be put on the windowsill or table I ate at. They need a better system for wound care. Perhaps a cart with a sterile surface? read more

Transported there from Stanford after one week having a lumbar fusion. Each department at Pacific Coast Manor does not communicate well to each other. You are aware before you go the facility you share a room but what you are not aware of is you share a toilet with 4 patients. These toilets were not sanitized enough. The place was under staffed. 20 minutes for a button pushed in the middle of the night only to then have a nurse come in scolding you for her having too many patients. Worst of all this is one of the best places offered. Insurance pays them big bucks! THE FOOD IS BAD. Some of the staff like Carlos are kind, compassionate and do their job well. PT is limited and can be handled to be a personal challenge from a staff member. WORST OF ALL IT IS ONE OF THE BEST OFFERD. Sorry PCM this is my story. read more

Both of my grandparents and a grand aunt have lived here. The staff is nice, professional, and knowledgeable. They took great care of grandparents and my grand aunt. This facility does have activities for the residents. If I need to be in a nursing, I hope it's this one or one as great as this one. The parking lot is small so visiting might be a problem if you are driving. A great place if you need a nursing home. read more

Most of the staff seem like they are actually happy to be at work, and I appreciate the generally upbeat, positive atmosphere.
