Was transferred here to learn how to rewalk again. I had read the reviews before being transported here so already I was skeptical, but my case manager said there wasn't another option based on age (I'm in my 20s) and I'm sure my insurance. Good things: PT Team was great! Was able to get me walking again after only a week of being there! Sarah and Melanie were always so patient with me and listened to what I needed. Marsia my nurse was the best thing about this place! She advocated for me and pushed for me to go back to the hospital when I was having complications from my medications and the doctors didn't want to do Jack! Okay things: Most of the CNAs were nice. The younger ones tend to be the nice while the older ones can get impatient and short with you. But don't rely on a quick response, 30 minutes to respond to a call light! Bad things: No one checks on you! You have to request to have your sheets changed. Doctors only show up once a week when they want to to check on patients and you can't request any medical needs like labs till they are there! And even then they are likely not to do anything! If Pat comes into ur room when you have a call light on don't even bother asking her for anything, she won't do Jack and and say you can do it yourself even when you literally can't. Even if you get sick right in front of her she can't be bothered to help. Make sure you take your shower when you're told to otherwise you'll be thrown under the bus! To top it all off food is crap and give you like 15 minutes to eat and if you're not done then they will just leave your tray till the next meal time. Rooms are also very small and no real privacy! If you have other options choose another place, but if this place is your only option good luck to you. This place needs a lot of improvement! read more

On 2/1/2026 a Patient expired at this location. On 2/2/2026 I called for the Family to request copy of patient's records at 10am. I was told by Jenny that Records Department Manger had only been employed a week this location and wasn't there today. I requested for Administration to give me a return call. At 4:49 pm on 2/2/2026 I didn't receive a return call. 6 hours later so I called again to no avail. On 2/3/2026, I called again at 1pm requesting to speak to Records Manager. At this time Jenny told me Records Manager was in meeting all day and couldn't return my call. This is ridiculous and so unprofessional. The family is desperately requesting records for their loved one. This is requarding a precious like that was lost and facility is displaying NO concern or consideration for the family. Is this the type of facility you would trust your loved one at?? Think twice!!! read more

I read the reviews from 2024 and nothing has changed, over crowded, unfriendly, down right unclean and feed my dog better. I really do not want to put them under the bus but they should not be in business. I would do as the Indians did and just walk into the hills . read more

I would not wish this place on my worst enemy. Grandmother was sent there for rehab, and aside from the 15 minutes of PT and 15 minutes of OT she was either kept in a chair or bed the entire time. Three people to a room, it smelled and honestly management could not get their act together. Worst part was, two hours after arriving they lost her hearing aid, and magically two and a half weeks later it showed up. They said it was found in her dresser drawer which had extensively been searched prior to this by myself and two others. DO NOT PUT YOUR LOVED ONES THERE read more

I am writing this as a grieving, furious family member who watched Palomar Heights Post Acute put profits over people. This place is chronically overcrowded -- they cram patients into rooms like Tetris pieces, with no regard for dignity or basic safety. That overcrowding created a welcoming environment for COVID to spread through the facility and sicken multiple residents. Medically necessary care is routinely neglected. My grandfather never received the follow-up he needed to recover; his health deteriorated while he was there, and he died years ago. Now my uncle is a resident, and I am terrified they will let him slide the same way -- not pushing him to meet physical therapy goals, letting him lay there and just rot away, not giving him the care required to get him back on his feet and home to his family. Palomar Heights: change your ways. Hire competent, compassionate staff who will actually deliver required therapies and treat people like human beings, not beds to fill. Do your job -- stop putting numbers ahead of lives -- or we will pursue every legal remedy available to hold you accountable. Don't fail another family like you failed mine. read more

If I could do no stars I would. I was sent there from Scripps and it was the most horrible place I have ever been to. I was dropped in my room they gave me a tv test and a Covid test and was gone. I saw no one for damn near three hours. I had a room mate that was screaming for me to go to her so she could see who I was. I stayed wrapped in my clothes on my bed afraid to touch anything. I was trying hard not to go to the bathroom but when I did I walked in and the floor was sticky. There was a box on the ground which you were supposed to put your used TP at. I assumed that since the sign read no toilet paper in the toilet, YUCK. 2am and still no meds or insulin. Then the roommate starts yelling for someone to change her. That was it for me. I got up started wandering the halls where there was an ever present yelling and screams. I finally found someone and said I wanted out of there. They said oh no you can't do that. I asked again is there anything keeping me here? They said if leave it will be under AMA. I said fine get my papers cause I'm leaving in ten minutes. Seeing that place in the rear view window I knew I had dodged a big bullet. Never send any of your loved ones here. They will die of neglect and the smell is out of this world!!! read more

Palomar Heights Post Acute is like a second home to me. I've been staying here after my hospitalization and its been great; the people are friendly, the care I've been given has been great. The location is perfect with close relatives living nearby within walking distance so I have frequent visits and being able to do physical therapy also helps with my recovery. read more

No stars. DO NOT send a loved one here. This place needs to be shut down and the staff banned from being responsible for any persons care. They are negligent, rude and do not inform the family of important life decisions. The facility smells and the rooms are overcrowded. The staff is unkind to the patients and leave them unattended including important medications. I cannot stress this enough SHUT THIS PLACE DOWN ASAP. read more

Love I was sent over there from UCSD after getting total knee replacement. I was there for a total of 12 days during my time there I met a lot of staff and a lot of nurses there they went above and beyond with helping me knowing my situation with my knee and everything and still taking the time to take care of their other patients, I appreciate them very much, and also with the rehab that I was receiving there at the facility. They are my time there I realized how good of people these staff members are, going to miss you guys. Thank you for everything you did for me. read more

Very bad service, patient was not taken care of and when confronted many times we were told we are worst complainers, talking in asian language/ laughing while patient need changing they go take a break and truly ignored patients needs on many occasions with witnesses, when confronted they don't show compassion at all do not go there will end up in hospital due to poorest of service. read more

Over the last year, both of my parents have stayed at Palomar Heights on multiple occasions. The Staff and Administration are wonderful. They have shown amazing care for my parents and maintained a stellar line of communication with me. Kudos to PHPA!!! read more

My mom spent a month here, she hated it! Anita, her case worker was very unprofessiona, she never returned one call or voicemail that I left her...not one! She never faxed over the paperwork for my mom to get physical therapy and never ordered her wheelchair. Clearly, it's all about the money at this place. read more

The only thing nice about the whole facility is the view. Each person pushed their work onto somebody else, even taking up to 3 hours to get to the restroom. Physical and Occupational therapy didn't see me but 5 times in a two week period. My insurance payed for therapy for two weeks but I received no such thing. It was noisy 24 hours a day. I wouldn't go there again unless I was moving to a facility to get on hospice to die.I went there to learn to walk again and without the medicine they were SUPPOSED to give to me, left in an ambulance. Thanks a lot Palomar. They also lost most of my clothes in the laundry and now I must buy a whole new wardrobe. How could they possibly get 5/5 Medicare rating? Medicare must not be looking with open eyes. read more

My husband was a wheelchair patient who had fallen and broken a small bone in his neck. After five days in the hospital he was greatly weakened and not able to transport himself from wheel chair to bed, toilet, etc., so we took him to Palomar Heights rehab to regain his strength, after visiting several other rehab in the area. He was put immediately into diapers and no effort was made to help him get to the bathroom, nor was he provided with a wheel chair for nearly a week. Rehab was spotty due to staff vacations over the holidays, and as he got weaker he resisted therapy when it was offered, and no attempts were made to encourage him . Three weeks after he was admitted he passed away from undiagnosed urinary tract infection and renal failure, sepsis and pneumonia, none of which were ever tested for or diagnosed. All this from a little tiny broken bone. We sued and won a judgement, which it doesn't look like they're telling anyone about. read more

I'm a daily visitor except when there's a covid breakout.my wife is a residence at the rehab.I really appreciate all the friendly and hard working staff.The nurses and cna's are very professional,they took care of her with tlc,also rooms,bathrooms and hallways are spotless.kudos to the cleaning staff,they are doing an outstanding job. My Dad spent 2 months in this facility recently. As skilled nursing homes go, this is the best one that I have come across so far. My Dad has been in 2 other ones this past year. My Dad shared a room with one other resident. He had a big window in his room that had a view of the mountains. He also was able to open the window for fresh air which really helped. The nurses Jocelyn and Rose were the most attentive. Jocelyn always took time to help me and bring me up to date every time I called or stopped in. Rose kept a good watch on my Dad. In the beginning I was concerned about my Dad getting discharged to soon. The business Manager Isabel kept her word and allowed my Dad to stay under "custodial care" for a few weeks. I gave a 4 star instead of a 5 star as I think that the different departments could be more informed about what's going on in different departments. read more

I'm writing this yelp review to other families when they have to move there loved ones to an after care rehab from the hospital. My grandpa had been at the facility for about two Maybe three weeks when I finally had the chance to go visit him I was shocked ! First off, let me ask a simply question .... if you take care of someone and you help them urinate everyday because he had no catheter... would you notice if that person was not peeing anything but two drops and within 4 days all the sudden your patient looked like they were days away from giving birth with twins ? Would that make any person go wow you need to go to the hospital now and we need to get you help ASAP?! Cause last time I checked males can't give birth ! Anyways this place is a joke -basically not trained correctly and should not be taking care of patients in acute rehabilitation center. The staff were like oh yea your his granddaughter .. yea I have noticed he is not doing as well as last week....haha ummm hello people has anyone not noticed my grandpa is about to give birth ? His stomach is so extended and bloated he looks like he's gonna pop and die right in front of me ! So I told my mom either you call 911 right now and get my grandpa to ER or I'm gonna do it ! These people are just gonna let him die ! He's just a patient to them .. he is everything to us and I won't let him die like this, my grandpa is the most awesome person I know who has always brought joy to my life So that's what we did ... and it saved his life ..ascites and fluid building up in the abdomen wall as well as bladder obstruction and major infection of his entire system,...so thanks for offering to take an chest X-ray and wait it out all weekend trying too give him laxatives to poop! My father was placed here for physical rehab after being hospitalized. He wanted out immediately, and I could see why when I went to visit him. The rooms are tiny, with zero privacy and about 3' between beds. It's very difficult to visit with someone in their room, since there are no chairs. You have to ask for one, and if you're lucky, they'll remember the request after 15 minutes. Some rooms have three beds to the room. There's a "privacy" curtain between beds, but it's open on top so the roommate's TV flickered all night on my dad's face. My father complained that the staff suffered from the "left hand not knowing what the right hand was doing." Quiet building? Not even close. About every 10 minutes, a shrieking alarm would go off because someone opened (or left open) a wrong entry door. Yeah, what a view from his room...of the hill with weeds above the Walmart parking lot. "Luckily" my father was out after 48 hours and returned to the hospital. When released to rehab a second time, we outright refused the dumpy Palomar Heights. The place was not conducive whatsoever for my father to heal. My mother in law was sent here after her release from Palomar hospital. The very first thing I noticed was the strong smell of urine in the hallway. Second thing I noticed is how tiny the rooms are and how they put 2 to 3 people in the tiny rooms with absolutely no privacy. I don't know how anyone can relax and heal in this depressing rehab place. The care givers look overworked and depressed as well. If the hospital recommends this place for one of your love one's, tell them NO THANKS!!! and look for a better rehab facility. read more

We were fortunate to find this lovely place after my sister's mother in law needed some rehab. The caring staff was so accommodating! The location was ideal for us as well. It's in the perfect location near shopping centers which made it easy for us and our family to visit. The rooms are cozy and the big windows allowed for plenty of sunlight to brighten up the atmosphere. I would give it 5 stars if the place would be remodeled. It's functional but it definitely needs a little face lift. read more

Don't go here for any reason!! The staff doesn't bother to follow doctors orders, the place is not very clean, they don't bother to listen to their patients cares or concerns. God forbid if you have a wound that needs a bandage change or your pain meds...good luck getting the staff to respond to those requests. Even a basic preference of a food request was completely ignored and disregarded. Pay for better care (really just care) any place else if you need it. read more
