I have been to Dr. Pechak's clinic regarding a gum treatment. The treatment included 2x surgery's. Both surgery's were quickly done in a very professional manner. Dr. Pechak and the Anesthesia team (Donia) provided wonderful and professional work. I can truly recommend the clinic and give 5x stars with no doubt. Do not go to this clinic. They take $$$ and do not complete work paid for. Not an issue with Dr although he does not appear to be accountable for mismanagement. Horrible experience with office staff. Understaffed. All interactions included complaints about lack of staffing in both offices. Overcharged.Treatment discontinued without completion of work. Office front desk incompetent, unprofessional and unethical in their practice. Surgical staff is good. The list of grievances with this staff are too lengthy to list. They are incompetent and dishonest. Melissa is incompetent, rude and lies. Maya is arrogant and utterly lacking in compassion or knowledge as to how to deal with patients. Quality of care would be positive if they completed treatment that was promised and paid for. Dr. Pechak does not talk to patients. at all or take responsibility. Apparently, they had 3 workers who left suddenly November on extended "medical leave". They seem to be unable to find competent employees. Their have a revolving reception staff with only Maya and Melissa running both offices. Beware of this office. They are neither ethical or honest. They charge you before you have any treatment. I'm not sure what happened to this place, because I've been going here for years, but the last time I showed up I was informed I had an outstanding balance due. When I asked what this was for I was told that the last cleaning was $100 more than I was charged at the time of service. I was also informed that I owed money for multiple dentist exams. When I told the receptionist I hadn't seen the dentist in years he seemed confused. The only comment made was that there were no notes in my chart from the dentist so maybe I was right. Then I asked if they could look up my insurance and they said only if I had my card on me. I have had dentists look this up online before so I was shocked at how cold and unhelpful they were. When I finally got back to the hygienist I asked if I could speak to a manager. Someone came halfway through my exam and said they didn't have time to meet with me, but would audit my account and get back to me. When I checked out the front desk had somehow managed to find my insurance without a card. There was no apology for misunderstanding. No explanation for the multiple exam fees I was going to be charged that never happened. Nothing was said about why the last cleaning wasn't collected in full at the time of service. 2 weeks past and I didn't hear back so I called and asked for a follow up. The same receptionist called me back and said I owed $200 for an exam fee back in 2024 with a dentist who ended up cleaning my teeth. I had no idea I was being charged for an exam at the time, nor was I told after the visit. I'm unsure why the lack of transparency around fees and why they are deciding to charge me now for something that happened over a year ago, but needless to say I won't be returning. I encountered false representations and a complete lack of professionalism throughout my visit. I will be filing a formal complaint. I find it very surprising that a clinic as established as this one doesn't have a process in place for when the insurance claims are not processed properly. How do they not know? And once it's been brought to their attention, it seems they don't know how to properly resolve it. It really is not that hard. Making honest mistakes is one thing, but not admitting or owning up to it is truly despicable and in bad faith. Really makes me wonder how many other patients had to pay cash price for services that would've been covered if insurance claims were processed properly. I had to find this out myself about my claims. When I called the clinic I was told that the billing team would refile the claims. I followed up weekly for about a month with the office staff giving me obscure answers. I ended up calling the insurance myself to find out what the issue was. I also found out that the clinic did NOT resubmit any claims as they said they would during this entire month of my back and forth with them. I had to call the clinic back with the exact instructions and ensure they have the billing team cc me when it's done to eventually get the claims reprocessed correctly. Two months passed where I did call the office once to follow up on the reprocessed claims with no further updates from them. I eventually saw through my insurance portal that the office had overcharged me for the procedures and owed me money after insurance payout. Because of how unreliable my follow-up calls with them have been, I waited until my final in office appointment to ask about this. After my appointment was done, I was taken into an office space with Melissa. And this is where it gets crazy. Melissa printed out some paperwork to review the billing with me, and in the end said "yep you're all good to go!" I pointed at the final balance on one of the papers and asked whether the -$800 balance shown is money owed to me. Melissa immediately says no, they're waiting on some resubmitted claims since they always resubmit claims twice when they're rejected and pointed at a random obscure line item. Lady please. Look at the amount of work I went through to get them to resubmit my unprocessed claims. And now she's claiming that they reprocess denied claims twice? Don't make me laugh. This clinic simply does not have the capacity nor the good intention. Melissa was also unable to answer when the claims were even resubmitted and told me to wait an arbitrary 2 weeks for an answer. Mind you it had already been 2 months since anything was last processed through insurance, and 3 months since my last procedure with them. At one point Melissa even tried to take back some paperwork she gave me and said they were irrelevant. She also tried to gaslight me by telling me multiple times "I don't know why you're so mad! Your insurance did so well! You don't owe us anything!" When they literally owe me $800 after insurance payout. Hello?! She ended up taking me to her computer outside in the hallway by the waiting area to print out some more things to show me. The paper she printed also showed a -$800 balance and she legit said "oh I don't know why this is here" like it misprinted and she's confused. At this point we were in a mild shouting match and the whole office could hear. I kept calling her out on her BS and she would just continue to lie to my face. I honestly felt like I was gonna have a stroke if I kept talking to her so I left. I ended up calling my insurance a few more times in the upcoming weeks and confirmed the claims have already all been processed and there were NO refiling of any claims as Melissa had claimed. I called the office back, and they were eventually able to write me a check for the $800 they owe. TLDR: AVOID THIS CLINIC. The office staff/billing department is incredibly incompetent and dishonest. Melissa is a gaslighting and deflecting pathological liar. If you do go with this clinic and anything goes wrong, just know that no one will let you know. No one will call you back even when they say they would. When prompted, they'll tell you they're on it but nothing would be done. You will likely be overcharged for the service so be vigilant. Keep a log of all your calls/encounters with them and know the names of every person you speak with. Be prepared to advocate for yourself because this clinic will not. In addition to the $800, I would be down a total of $1700 with this clinic if I did not double check all the claims myself. Dr. Keswani was amazing and I would trust her with all my perio needs. But the physical and mental stress of having to deal with the dishonest billing practices and the fraudulent, incompetent, lying office staff does NOT make it worth it. There are plenty of other perio clinics with great reviews, please do yourself a favor and avoid this place. read more

This place has such a HORRIBLE quote/refund handling process where I'd like to give it 0 star if possible. This review is not for evaluating Dr. Pechak's skill since I decided not to use this service due to extremely vage quoting process. I have taken an evaluation visit to this implant center for tooth extraction and implant quote in first week of January 2025. Dr. Pechak did x-ray scan, took some photos of my mouth and sent me to the front desk to discuss about Quote. I was charged around $200 for my evaluation visit which the front desk told me could be refunded after insurance reimbursement. I was given several combos of treatment plans which includes 1) Deep oral cleaning 2) All wisdom teeth extraction + desired tooth extraction 3) One side only teeth extraction + desirect tooth extraction 4) Remaining wisdom teeth extraction 5) Follow up visit X-ray cost + implant cost. None of these quote came with any itemized dental code or itemized price quote for each work. It only shows price for the whole treatment package in total. When asking the front desk to provide an itemized list for quote, they simply refuse to do so unless I could pay the full quote amount and lock my appointment. At this point I decided to walk away due to their bad attitude to push away customers. I have revisited Dr. Pechak's office at end of January to print itemized receipt for FSA record keeping. The receipt has shown that Dr. Pechak owed me $160 after insurance reimbursement on that specific date. Ever since that day I am calling Dr. Pechak's office weekly to ask for my refund. This business always reported they need internal approval process or they need decision from financial department to prepare for a check. I have not yet seen any sign of refund in 2 months of time. I decided to file complaints to 1) Better Business Bureau 2) California Dental Board to escalate the matter. If I still cannot see refund in a month, I am going to take legal action against this business. For anyone seeing this comment. Avoid this business at all cost if you don't like vague quoting, over paying for tooth extraction&implant procedure. Do not even take the evaluation visit as it's extremely hard to get any money back from them. read more

Tuesday, 23 Sept I experienced a tooth extraction by Doctor Perchak. It was the most painless dental procedure I have had. His staff made me feel comfortable and well taken care of. About 6 PM that evening, after the sedative wore off, I felt no lingering pain, a slight headache was it. I would definitely recommend the Perry and Implant Center for any dental issues you might have.This was the first step towards an implant and I look forward to that being completed. read more

Dr. Pechak is a very good Dr. you will get the top of the line care, but it's going to cost you arm and leg. The office management will charge you $$$$ with hidden fees and over the estimation cost. That includes FRAUDULENT CHARGES. I tried to challenge that with insurance, but the office response was full of lies. They will not return your calls to refund over pay after EOB settles. They keep ignoring my multiple requests to meet to go over the math of unpaid balance. Very convenient when collecting the full payment before the procedure. Be warned! The photos and X-rays taken during the exam truly help understand what Dr. Pechak saw and why treatment was being recommended. Dr. Pechak is the one of the best periodontist in the Bay Area! He always provides very professional implants and gum treatment plan. He did multiple (5+) implants, bone grafting and tooth extraction for me, all of them had a quickly heeling process and best results! Last week I called his office about my gum feels a little uncomfortable, Clarissa quickly squeezed me into Dr. Pechak's busy schedule just one hour later, and when I went to office, Dr. Pechak immediately took a full mouth X-ray and did a throughly examine in a short time, also provided me prescription also told me which mouth rinse I can use. After 2 false starts with other providers I ended up in Dr. Pechak's office where he quickly diagnosed the problem and created a plan for treatment. He came into the office extra early the next day to patch me up so I would not miss my trip. After I returned the office worked with my not very flexible schedule to get the job finished. I had my initial consult with Dr. Pechak at his new office at 500 S. Murphy. It was a beautiful facility with exceptionally punctual, organized and professional staff. From the moment I contacted their office, to my "new patient forms" via email, reminder calls/text, and my in-office check-in, it was very pleasant and easy. Every person on his staff clearly has a role. Dr. Pechak has a very nice bedside manner, explains concerns and treatment thoroughly, and is patient with your questions. I have consulted with three other periodontist through the years and had two previous gum graphs. Dr. Pechak is not only professional, he is quite skilled. He did my first gum graph in 2000 when he worked on Stanford campus. I remember he called me that night to check in that my post-procedure healing was going well. I sadly didn't have records and couldn't remember his name for my next gum graph 10 years later. When I recently found him again with my new dentist, I was thrilled. He proved to be all I remember from 20 years ago; very kind and professional, and up to date with the latest knowledge and technology. I am so relived to have my gum care back in his hands. It took me many years to *finally* book my full-mouth, gum grafting procedure. Dreaded the surgery, but Dr. Pechak was amazing. He made it as painless and pleasant as possible. Able to get it all done in one (albeit long appointment) and now almost 3 weeks after surgery, the results are incredible. And by closely following Dr. Pechak's post-surgery care procedure (including the StellaLife VEGA products), my recovery is moving along rapidly. I would strongly recommend Dr. Pechak for his periodontal expertise and skill, but also a gentle chair-side experience. My only regret is I didn't bite the bullet and get it done years earlier! (Btw, I found the entire team in the office including reception to be very kind, responsive and customer-focused.) read more
