This place was recommended to me from the hospital my mother was being released from for sub-acute rehab services. I went and toured the facility. It looked and seemed nice to the naked eye but the bogus business practices showed me what time these people are on. Its all a money grab and no integrity whatsoever. This facility has rehab and longterm care. While I was there I asked about longterm care because I knew the next step would be to bring mom home and attempt to care for her or place her somewhere that can care for a dementia immobile patient. So I give the hospital coordinator the green light to work with the ALH coordinator. ALH sends me a 15 page application to see if my mother will qualify for medicaid or financial assistance (She has medicare - private insurance). The application wanted all of my mother's account numbers and amount in them, they wanted me to sign over her SSA checks directly to them, and set up AHC payments for her care all before ever seeing a price list of any sort. Context, I told these people up front that we would be paying out of pocket because she has assets and will not qualify. That was met with MAJOR resistance and because of it, they held up the hospital discharging her to their facility for rehab. Again, the application is for longterm care, insurance would cover the rehab. So for over a week Im going back and forth between the facility and their billing office in NY simply requesting a fee list of proposed services to cover longterm care so I can budget what expenses will look like. They repeatedly told me they dont have a fee list (I sat on the phone with them while somene named Layla argued with her manager about fees). Note that with each person I spoke to, the basic daily room fee changed 3 times proving my point of wanting a simple list in writing. I explained multiple times that Im not showing them my mother's assets without knowing what their fees are. What's protecting us from them just making up new fees when they see what she has? These people told me oh we wouldnt do that. Guess they thought I was stupid because I refused. I asked multiple times, why they would treat a self-pay patient like this. "That's just our policy." Scammy to say the least. ON WHAT PLANET does a facility not have a set fee list of services. They finally sent something over with TWO lines on it. Nothing speaking to what else would be needed on a day-to-day basis, just room fees. BOGUS. So again, I refused to send the application back, and they refused to accept my mother for even rehab. Insane!

Unfortunately, I received no response, no returned phone call, and no follow-up despite my efforts. This lack of response only reinforced the concerns our family experienced throughout my grandmother's stay: poor communication, lack of accountability, and disregard for the concerns of patients' families. It is deeply disappointing that even after publicly acknowledging my review and inviting further discussion, there was still no effort made to genuinely address what occurred or communicate with our family. Families dealing with the illness or loss of a loved one deserve compassion, responsiveness, and transparency. Sadly, our experience continued to fall short in all of those areas.

She rang the nurse bell and they told her stop. my mom was in pain at the time. se3cond issue was with the same attendant week later. my mother was in pain and needed to be attended to for using the bathroom. They did not change/clean her up until over a hour later. I reported this to a supervisor, and they said the person would be reprimanded and would not be back the next week. That person came in that next night and asked my mother did she need any help. My mother said she was in pain and had hard time breathing no one helped her. my mother had to call me from her phone and asked what to do. She called 911 and first responders took her to the hospital. The Nurses were good and vey profession and on their job. The attendant and supervisor are not. The place seems somewhat clean. Your love one's needs might be on the back burner with some attendants. Seems like they want you and your loved one's money but don't want to treat them like they would their own family. Since then, I have been looking into another rehab and senior place. She will not go back there and will never if i can help it.

It was time to get serious help. I was falling 4/5 times a week and calling the fire department to pick me up off the floor. After serious research working with social workers, we discovered Autumn Lake.Spa Creek, 10 minutes from my family. The setting was beautiful, and I was instantly attracted to the place. I immediately contacted my social workers to find out how to become a lifetime resident. Come to find out that after providing this place with appropriate documentation and filling out forms needed, they evaluated me for residency. After some days waiting (not long) during hospitalization, I was accepted! I was transported to Autumn Lake via ambulance and immediately shown to my room. I was welcomed with OPEN ARMS! Every single staff member i met greeted me and offered their help whenever I might need it. All residents said hello with smiles. My room is spacious and most of all, SAFE...for me with an electric wheelchair. I met my sweet roommate, perfectly matched for me. Most of all, I got to meet Alish, one of the social workers instrumental in my acceptance. Gave her my thanks! Signed a lot of complicated paperwork, but Zi in the business office made everything clear...she explained it all. Then I met Rebecca (Unit Manager) who told me how the unit I was living in works and really presented everything clearly. If I need help, she was quick to say she's always available. Then I met Jallow, the nurse that I could contact for room/floor related issues, and of course any medical questions that I had. He was personally responsible for my direct care. I quickly came to find that he is a special soul, Jallow. You constantly hear his name called out loudly throughout the halls, and he always answers loudly and comes to your room immediately. I've NEVER EVER seen a person being CONSTANTLY PULLED IN DIFFERENT DIRECTIONS consistently respond with bright and knowledgeable care every time. WITH JOKES AND LIGHTHEARTED words that always LIFTS YOUR SPIRITS. Wow. He is AMAZING. And he manages the aides that work under him with confidence and brilliance that inspire his leadership. Cherlita is my favorite person on my care team. I get to be taken care of by her at least 3 times a week and she. Is truly a standout performer. I need lots of very personal care and she always goes the extra mile! It's INCREDIBLE how CLEAN she makes me feel...she handles many, many patients at one time - a typical night includes 19 patients , with call lights going on constantly for other rooms needing assistance urgently. I don't know how she does it. Her bedside manner is incredible, maintaining speed with excellence...and with a smile! Next, I'll tell you about the Therapy Department. I began therapy right away upon arrival. I met Ashley, Katie, Cesar, Harry, and Frank right away and they wasted no time in creating a therapy plan for me, and discussing my goals. A progress report was created. I want to stand. That is my goal. They are helping me realize that goal with carefully planned exercises. Working very closely with Katie, I'm working on foot and leg strength. Very difficult with loss of movement, but she is finding a way to somehow do it through hands on therapy, and strength building exercises. Ashley, Cesar and Frank are all working with me on upper body strengthening exercises and I've been able to enjoy visible results already. Autumn Lake is a clean, comfortable and very safe refuge for those that require rehabilitation and those who need a long term forever home. I was SO lucky to get admitted here.
