Please don't let their kindness while you're spending your hard earned money with them fool you!!!! I was a patient of theirs for approximately 18 years. They had had me on high doses of opioids and opioid pain patches. I just recently had to move out of state when I did they would not cover my care to detox me off of the patches and pills or cover me until I could get established with another pain management in the state I moved to. I really wondered why this place had such bad reviews because my first few visits were good. After 12 years of daily pain from a nerve entrapment, I finally got some pain relief, if you can call tramadol relief. But it was better than nothing, I had a diagnosis and a hope of a cure. I have attended every appointment. I did the nerve block even though it was apparently just a diagnostic thing and not meant as a more permanent pain relief which is what I was told. Eventually I tried to schedule a nerve ablation and was informed that even though my insurance would cover it (I confirmed myself) that they did not charge insurance. Well, I don't have a grand laying around right now so I said it'd probably have to wait but I'd talk to Dr. Kaplan at my next appointment. At my next appointment, I waited nearly an hour past my appointment time, finally spoke to Dr. Kaplan and was told that they did indeed charge insurance and she would take care of it. I heard nothing from them for a week.... And then they finally called to schedule the procedure.... The day I was in the hospital with my husband who was having extensive surgery on both hands since both his thumbs had somehow become detached. I called them back two days later and said I needed to find out what the recovery time was because I now have a husband with no working hands which means I really need to be fully dinctioning until he heals in the next 4-6 months. I never received a call back and kind of forgot about it because taking care of 6 kids and a farm while teaching with no other adult help has had me a little busy. And today, I get a letter stating that I have been dismissed from the clinic for "failure to comply". Comply with what, exactly? I came to every appointment. I followed their drug instructions to the letter.... In fact, I didn't even get the last medication refill because my pcm told me she wouldn't treat my anxiety/ptsd if I continued to take it. So what exactly did I "not comply" with.... *follow up* They have called and claimed I popped positive for methamphetamine at my appointment on 11/18.... But showed nothing for the benzos I'm prescribed and take regularly. And then, after said supposedly positive test which was in November, they still put in a refill weeks later for pain medication. I was also prescribed promethazine on 11/12/25, 6 days before this drug test. Guess what pops positive for methamphetamines..... Promethazine. I know this because the exact same false positive happened with the exact same medication in 2022 when I was giving birth and I still have the signed affidavits from two pharmacists saying that it did.... A 36% chance in fact, higher depending on the manufacturer. And for the record, I tried to tell them at that appointment that I had been prescribed new medication and it wasn't in my file and they said it didn't matter. Sure. This kind of unethical bs is why chronic pain patients kill themselves. This clinic makes military doctors look good and I didn't even think that was possible. 13 years of absolute misery, finally get something resembling help and a hope that the problem can actually be fixed and they snatched that away because they're too damn stupid to log medications or know that medications can cause false positives. But no, clearly I woke up one day at the ripe old age of 43 and just randomly decided to do meth while knowing I was going to be drug tested. Absolute morons. Apparently in TN you don't get pain relief unless you want to run the risk of one of your allergies actually killing you by not taking your medication. 1 of the WORST offices I have ever had to deal with. If I could give them a zero or minus I would. One hand doesn't know what the other hand is doing. Went to appointment and was told they classified me as a new patient ( which I wasn't), and Doctor couldn't see me and needed to reschedule my appointment, ( I live 1hr away from site). Had my procedure done and called for a follow-up appointment and I needed to be seen at the Chattanooga office, day of appointment and they call to tell me I have to go to the Ootawah office which I told them it was further then my insurance share ride could go. This Doctor nearly killed me using meds in pain pump that were not FDA approved. I ended up in a coma and now have to deal with brain damage due to O2 deppervation. Stay away from Doctor John Blake! TL;DR Find another place. They don't care about their patients, just the money. You likely won't see any Dr., but the NP's instead & they don't listen to patients. I'm also not sure how organized they are or truthful in their patient visit charts because on my first visit I saw Holcomb & we discussed physical therapy, but I told him I've done that 3 times so I have the exercises at home that I can do. We decided on having facets injections. If those worked then we'd do the ablation, the scheduler would call me, & that was the end of the visit. He left the room, but popped his head back in to hand me a brochure on a Spinal Stimulator & said Dr. Blake wanted him to give it to me. I casually said "Oh. I don't think I'm there yet." bc why go through another surgery when there are less invasive options? A week passed & no one called me to schedule the injection so I called them. The woman told me that no procedures were noted in my file. She read the notes to me & all the notes said were "patient refused physical therapy & spinal stimulator." she said there was no mention of the procedure. I am in no way exaggerating when I say the spinal stimulator wasn't even a conversation, it was a brochure handed to me through a cracked door. I also didn't refuse physical therapy, I said I would do it at home. Absolutely inaccurate. I also stated I'm there for alternative treatments & if I ever need pain meds; not noted either. She said she'd call back, never did. 4 weeks later, the injection follow up visit, but I never had it since no one called me. Day before the visit I had rcvd an authorization approval letter from my insurance approving the injections, so I took it with me. It was with different NP, I can't remember the womans name, but she's absolutely the worst! She interrupts you while explaining the issue so she never hears the whole problem. She said I didn't sound like I was a good candidate for the ablation & instead tried pushing gabapentin, cymbalta, lyrica, & topamax. I explained I just got off of Gabapentin and Topamax & they didn't do anything. I tried Cymbalta for anxiety, but I didn't respond well to it because I can't take SSRI's or SSNRI's. I agreed to try Topamax again, but asked her to check for an interaction with a other anxiety med that I just started and it was amazing for me. It interacted so I declined. She then proceeded to shame me for not wanting to take medication that messes with my mental health & when asked why I declined the Cymbalta I told her bc those meds make me suicidal. She asked how old I was, I told her 36 & she got a cocky attitude & told me the suicidal risk only runs from ages 18 to 25 or some age younger than I was. I said "Oh yeah? Because 2 suicide attempts from taking similar meds only happened 2 or 3 years ago; that's how my psychiatrist & I know I can't take that family of meds." She continued to be condescending about me not taking her pill options w/ zero concern for my mental health. Despite not believing I was a good candidate, she made a note for someone to call me ASAP to schedule. 3 weeks *after* visit #2, someone FINALLY called to schedule the injections. I told her my actual back Dr. is going to perform the procedures because it took them 7 weeks to even call me. She NEVER noted that in my file FYI. I was treated so poorly by NP #2 & the massive amount of incompetence makes me extremely uncomfortable to have anyone there touch any part of my body. I didn't plan on ever going back. Visit #3 came after a stent was put in my left common iliac vein bc it was 90% supressed & the stent was pretty close to the hardware from a spinal fusion I had in 2017 that I was still having pain from. The pain was excruciating because of this & because of the stent stretching the vein. The pain lasted longer than the 3 days worth of pain pills I was allowed to have *per the government* & I am on Plavix now so that limits a lot of medication I can take & I can't have the ablation done until I'm off the Plavix in 90 days. The Vascular NP suggested I go to pain management for meds since their hands were tied & they called for me. I got the NP from visit #2. I tried to explain about the stent & the hardware, but she looked at me like I was crazy when I told her the stent was placed in my lower back since, well l, that's where the suppression was. I gave them pictures that the Vascular Dr. took during surgery that shows the before stent placement and after stent placement and ya know what's in the picture too? The hardware from the fusion because it right next to it! She didn't bother to read the Vascular report or anything, she didn't believe me, & would only give me 4 Tramadol (10 mg hydrocodones worked, Tramadol is a step up from aspirin) & NSAID patches that insurance probably wouldn't cover. I was livid. It was their fault I was in so much pain bc if they had done the ablation in a timely manner then I may not have been in that much pain. I declined the pills left. read more

I was 10 minutes late for my dr appt due to a wreck on the interstate. The last 3 times I was there I had to wait up to 2 hours to see the PA for 5 minutes. I will not go back to a drs office who does not care about their patients. I even tried to call but they don't answer their phones until 8. Wasted trip. Will not go back. read more
