The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. They want to help and truly care. We are lucky to have them in Claremont.

What a job. You never have to return telephone calls. If this facility cannot return a phone call, I am suspect of what type of medical care they are giving to their patients. If you do have family there, and have issues, please call the LA County Ombudsman's office at (800) 334-9473. You can also contact the Medical Board of California www.mbc.ca.gov to file a complaint. Do better Pilgrims Place!

Let me start…read moreoff with my analogy of this place....Hathaway is like a big ship and the manager, therapist, and counselors are busy all day rowing to keep the ship moving, while all the techs are busy patching the holes to keep it from sinking from all the holes that the owner Dina is causing. There is a very high turnover rate with counselors the longest one lasted 2 weeks and have gone without a counselor at times and therefore had no groups. We were bored most of the time and we ran out of essential food items for three days such as milk, eggs, butter, and bread. Then the food was locked up and rationed out like we were prisoners. Most of the parents had to bring in food. Not to mention the food that was purchased was low quality and many of us had to purchase our own food but were not able to get out often enough to keep stock. Staff were so busy driving the ship and being overworked that they could not keep up with the schedule or meet our needs. The staff nurse made several mistakes with the medications and sometimes worked up to 24 hours straight. Even in the midst of all the chaos, disruptions, and lack of structure I was still able to get what I needed for recovery due to an awesome therapist and counselors working hard to give me the tools I needed to recover and helping me to apply them. I had to work hard independently by doing homework and reading the material that was provided by the therapist and counselors. I tried to communicate my frustrations on several occasions with some changes made that didn't last. They don't practice what they advertise on the website. So do your research before checking into a rehab facility especially ones that cost as much as this one did!

I live in hacienda heights, and I had to go to West Covina to go pick up my mother. I could not be there instantly I told the male nurse over the phone. You need to train your staff to speak in a more sensitive way. Especially after a bad news had given to the family. "I am so sorry your dad has passed, however; we do have policy the body has to be removed in x amount of the hours. Please drive safely to get here asap when you can." This was the way, your staff should have spoken to me, 2 years ago. I did not complaint before because I was in total shock! Do Not take advantage of people's grief and depression again! I see a lot of work as they're trying to make improvements. For a visitors, employees, and residence of this facility. I had a question where my friend was located at. They were very quick to look in the computer and guide you which way to walk closer to the room. Smiles on each of the workers. For they truly care of what type of care they give to each resident that lives there. They work with post acute patients that come out of the hospital. Claremont Heights provides physical therapy so the residence can be more independent so they can go home quicker. The beautiful activity room was wonderful to see as this is a heart of each residence, getting better and stronger and feeling loved. They provide the medication each residents needs. The check-in kiosks is so easy to understand and use. Once you walk in the lobby, the staff is very happy that you were there very kind and always has a smile on their face. The only downfall I noticed is the facility. Some rooms have cracked in the ceiling and in the wall. There needs to be a custodian team to keep the rooms swept and cleaned to whether you'll find trash on the ground. To all the nurses that work there the CNA's that work there the kitchen crew that works there. I am so thankful that every resident has a smile on their face, knowing they are safe and well loved. There are vending machines and some courtyards so the residence can buy their favorite snacks. The roses and the colors of the gardening are just so beautiful. It feels like you were at home. When you walk into the lobby, there is music being played. And there is a TV screen and it shows the most amazing colors of every season like a picture window.

    My family had a very difficult experience with Pomona Vista Rehabilitation Center. My mother was…read moreplaced there following hospitalization and required very specific medical care, including strict dietary restrictions related to a feeding tube and recent surgery. Unfortunately, there were serious communication and care concerns during her stay. Despite repeated efforts to inform staff about her medical restrictions, she was given food that she was not supposed to have. This led to complications, including her feeding tube becoming dislodged and requiring an emergency hospital visit. Even after the hospital provided updated instructions, it felt like important information was still not being clearly documented or followed. In addition to the medical concerns, it was very difficult to get timely responses from staff, and we often felt that our concerns were not taken seriously. This created a great deal of stress and worry for our family during an already challenging time. We ultimately requested a transfer to another facility closer to home so we could better advocate for her care. I strongly encourage families to stay very involved, ask detailed questions, and closely monitor care plans if their loved one is placed here. I am sharing this experience so that other families can be informed and prepared.

My elderly family member was admitted here short-term. Within the first 18 hours alone, we experienced multiple alarming issues including missed medications, repeated unanswered call lights, transportation failures, unsanitary conditions, etc. SOME issues during the stay were: - My family member repeatedly sat in feces for extended periods waiting for assistance after pressing the call light that resulted in skin rash. (Unanswered call lights were a HUGE issue at this place). - There were flies in my family member's room, they gave my family member half cooked meat (pink inside [NOT STEAK]), & there's strong foul odors throughout the facility. - Dialysis transportation coordination was HORRIBLE to the point where family member almost missed the dialysis appointment twice because they did not coordinate the rides properly at all. I have no idea how this place is still open & running. I've informed Kaiser that they should take this place off of their list. Weeks after my family member was discharged from this place, I spoke with a different social worker and this social worker informed me that "Many families have complained about that facility." YOU HAVE BEEN WARNED.
