I am extremely disappointed with my experience at this dental office. From start to finish, the…read morelevel of professionalism and patient care fell far below what I expected. Communication was poor, appointments were not handled efficiently, and I felt my concerns were dismissed rather than addressed. The staff appeared disorganized to my case, and the overall experience left me feeling frustrated and undervalued as a patient. What was most concerning was the lack of transparency regarding treatment recommendations and costs. I did not feel that adequate explanations were provided, making it difficult to make informed decisions about my dental care. I originally came in for a routine cleaning about a year and a half ago with no pain, sensitivity, or concerns regarding my teeth. During the visit, I was told I had a couple of cavities that were not a major concern, but I agreed to have them treated anyway. Since that treatment, I have experienced constant pain and sensitivity that I never had before. I can no longer comfortably bite into foods as simple as chicken without pain, and drinking anything cold causes significant discomfort. What should have been a straightforward procedure has turned into a year-and-a-half-long ordeal. Over this period, I returned multiple times to report the same symptoms. Rather than seriously addressing my concerns, I was repeatedly told that the work had been performed professionally and that the discomfort would eventually go away. Days turned into weeks, weeks into months, and now more than a year later, the pain remains and has only gotten worse. The treatment was redone five separate times, yet the issue was never resolved. Despite acknowledging the ongoing problem by repeatedly redoing the work, the conversation than became that there was no need to continue and that "these things happen with age." This explanation is difficult to accept considering the pain began immediately after treatment and was never present beforehand. What has been most frustrating is feeling dismissed throughout the entire process. During my most recent visit, after more than a year of consistently reporting the same symptoms, I felt as though my concerns were not taken seriously. At one point, I was even left with the impression that the dentist questioned whether the pain I was experiencing was real. No patient seeking help for a persistent issue should be made to feel that way. I have placed a tremendous amount of trust in this healthcare provider. I trusted this office to improve my dental health, and instead I have been dealing with pain, sensitivity, repeated appointments, and unanswered concerns for over a year. I want to make it clear that this review is not directed at the dental assistants or "most" of the front desk and scheduling staff. In my experience, the majority of them were kind, professional, and genuinely seemed to care about helping patients. My concerns are specifically directed toward the individuals responsible for handling my ongoing care. Is what made this such a frustrating and uncomfortable experience.

Everyone including Dr. Sklenicka where very nice, answered all my questions and listened to my concerns during my visits. I had a consult at a different oral surgeon several months ago and just wasn't comfortable when leaving so I never went back. Consult and surgery all within a week which was very convenient for me and my work. I tend to bruise easy but had little to NO bruising on my face, some bruising with the IV(to be expected). I iced my face to help with the swelling but also had little to no swelling! Took antibiotics and Ibuprofen as directed so I truly wasn't in ANY pain just some muscle soreness. Never woke up during procedure. Although I do suggest they should manually weight patients rather then just asking. All in all, I am very very happy with my experience with this office and my results from having all 4 wisdoms teeth out. Don't be a chicken! It's a relief to have it finally done and over.

Following my consultation, I was provided with an estimate of over a thousand dollars. The office correctly charged the contracted in -network rate for the office visit and X-rays, but charged me the out of network, max price for the 2 wisdom teeth extractions. I immediately questioned the amount of the tooth extraction because my dental plan provides an approximate range of $170.00-$250.00 for this service, at 80% of covered charges. The staff member acknowledged that the estimate was likely incorrect and instructed me to speak with the front desk personnel on my way out. I proceeded with the extractions and, upon checkout, spoke with the front desk regarding the charges. I was informed that this was the rate they charged for tooth extractions, despite North Florida Oral & Facial Surgery being an in-network provider under my dental plan. When I questioned this, I was told, "This is how we charge." Therefore, it is unclear why the office applied the proper in-network pricing to part of the visit while charging me the maximum out-of-network rate for the tooth extractions performed during that same appointment. After recovering from the procedure, I contacted my dental insurance and was informed that I had been charged incorrectly. Once the claim was finalized, my dental insurance determined that the in-network contracted rate for all the services was much less than I had paid and I was owed over 800.00 in a refund for overpayment. According to my dental insurance, payment was issued on May 29, the Explanation of Benefits (EOB) was sent on May 30, and the payment check cleared on June 1. On June 10th, I participated in a telephone conversation involving my dental insurance, myself, and a representative from the oral surgeon's office. During that call, the office representative stated that someone would contact me by the end of the day regarding the EOB and the refund owed to me. I never received that call. On June 11, I contacted the office again and was told that a refund check of over 800.00 had been mailed on June 5. I was instructed to allow seven to ten days for delivery. I waited until June 24 to allow sufficient time for the check to arrive, but I still had not received it. I left a message for the billing department and then called the office and spoke with the office manager. During that conversation, she stated that the refund had been approved on June 11 and mailed on June 12, contradicting the previous information I had been provided. When I informed her that the check had still not arrived, she stated that she would contact the billing office and call me back. As of the close of business today, June 24th, I had not received any return call. It has now been twelve days since the refund check was allegedly mailed, and I still have no resolution, no refund, and no consistent explanation regarding the status of my funds. SECTIONS 456.0625 AND 408.12 OF THE FLORIDA STATUTES ESTABLISH REQUIREMENTS FOR HEALTHCARE PROVIDERS TO REFUND PATIENT OVERPAYMENTS WITHIN 30 DAYS OF DETERMINING AN OVERPAYMENT HAS OCCURRED. THESE LAWS APPLY TO LICENSED HEALTHCARE FACILITIES AND INDIVIDUAL PRACTITIONERS WHO FILE CLAIMS FOR REIMBURSEMENT WITH GOVERNMENT OR PRIVATE HEALTH PLANS. Based on the facts described above, I am deeply concerned about the billing practices employed by North Florida Oral & Facial Surgery. At a minimum, the conduct appears deceptive, evasive, and inconsistent with the obligations of an in-network provider. I WOUND NEVER RECOMMEND THIS PRACTICE TO ANYONE! This review reflects my personal experience and is intended to help other patients avoid similar billing and insurance issues.

On August 20, 2026, my wife arrived for her scheduled dental appointment at 12:30 PM because she…read morehad been experiencing pain and discomfort in her tooth. Approximately six months earlier, she had been advised that she might eventually need a root canal and was prescribed ibuprofen to help manage her discomfort. During today's follow-up visit, my wife explained that she had begun experiencing discomfort again and simply asked whether she could be prescribed ibuprofen. She specifically requested ibuprofen--not narcotics or any controlled substance. Unfortunately, according to my wife, Dr. Elaheh Habibi's response and demeanor made her feel as though she was being treated like someone seeking drugs rather than a patient seeking appropriate treatment for dental pain. This was completely unacceptable and deeply concerning. There is a significant difference between appropriately evaluating a patient's request for pain management and making a patient feel judged, stereotyped, or accused because they asked for a commonly prescribed non-narcotic medication. My wife deserved to be treated with dignity, professionalism, and respect. I work in the medical field myself, and I understand the importance of responsible prescribing and the need to protect patients and providers. However, professionalism also means communicating with patients respectfully, listening to their concerns, and avoiding behavior that can make a patient feel stigmatized or humiliated. I believe this incident warrants serious attention. No patient should leave a medical or dental office feeling as though they were treated like a drug seeker simply because they asked about ibuprofen. I am requesting a formal apology from Dr. Habibi and an explanation regarding how my wife was treated during this encounter. I also intend to pursue the appropriate channels for reporting this incident to the applicable licensing and regulatory authorities so that the matter can be properly reviewed. This is not about demanding medication. This is about demanding respect, professionalism, accountability, and appropriate patient care. I will not tolerate my wife or any patient being treated in a manner that compromises their dignity. I expect this matter to be taken seriously and addressed appropriately.

The crown repeatedly fell off, and during a follow-up visit I was informed that the underlying tooth structure could not adequately support a crown. After learning this, I sent a formal written request for a refund and received no response. I also called the office twice requesting to speak with office management and never received a return call. As a patient, I expected professional communication and accountability, especially after paying a significant amount for treatment that did not succeed. Because my concerns were ignored, I reported the matter to my dental insurance company for review. While treatment outcomes vary, patients deserve timely responses when problems arise and reasonable efforts to address failed treatment. Unfortunately, the lack of communication and refusal to address my concerns has left me with no confidence in this practice. Their office manager or financial person will not return my calls, and I have been professional in the handling of this matter. Management resolved the issue at hand. They were professional in handling the situation and everything was resolved.
