On December 23, 2025 GF sent him to RI Hospital. While there the doctors said Hospice was the next step. He entered Hospice on December 23rd and passed away December 27th. On December 26, we cleaned out his room and notified the director that he was near the end of life. This was all done prior to 12/31/2025. In January, GF reached out requesting payment for the month of January 2026. I explained he passed and was paid in full through 12/31.2025. GF said they needed 30 days notice if moving out and therefore $11,000 needed to be paid for January. I said I do not think that should apply to this situation as GF sent him to the hospital and there was no way we could have predicted his passing and given 30 days notice. GF said they would consider that and get back to me. I never heard back and I assumed it was resolved. Well I was wrong. On July 14, I got a collection letter from an attorney looking for payment. Really?1? The Director never called me back, I just received a collection letter. In my opinion this is nothing more than a money grab! $11K for never living there in January! Plus I know for a fact that his room was occupied in January by another person. Talk about double dipping! GF is a business and there goal is to make money. I am in the process of getting my attorney involved and notifying the RI AG's office of my experience and GF's policies. I was convinced to stay the last five days so I could be discharged with the so-called home care package of in-home follow up therapy visits for my recovery because if I had left early, I would've gone home against doctors orders and been denied that Health service. I don't even know if it was worth it at this point because of how it felt to be in there... I just experienced another terrible episode of bad Healthcare at the Elmhurst facility at 50 Maude St. in Providence but as bad as that place was, Lincolnwood was much worse. Stay far away.

We transferred my dad from another assisted living to St. Elizabeth's a little over a month ago. Every time I come here, the staff is always smiling & friendly, the place is clean, & my dad looks well cared for. Administration is very helpful as well & very prompt handling any concerns. My dad likes the food too, which is great! He barely ate at the last place because the food was terrible. The building is in the old side. Seems to be a bit of delay transitioning from heat to AC when the weather warms up, but I'm told this is common for most facilities. It's been maintained well, but the windows definitely need to be replaced. Besides that, my dad's only complaint so far is the room size, but he opted for the smaller room & it IS bigger that the room he had at the other place; larger ones are available. The have a lot of places for family to gather. They also have 2 kitchens the residents can use to cook/entertain, a small gym-like room, a chapel, snack area, 2 sunrooms, a salon, & lots of TVs if residents want to get out of their rooms. And other rooms that I know I'm forgetting. Very happy so far.

The great thing is if Mom needs more or less care, she does not have to move! The staff just conforms to her needs, so her room is her room. She has resided at Smithfield Woods for over a year, and we are very happy with the care she is receiving. The place is more than clean- it looks like a hotel or condo and has modern rooms with spacious bathrooms/walk in showers, kitchenettes, A/C, beautiful windows/views, several dining rooms and common areas/reading areas, activities daily, and a full dining room for 3 meals a day. For an extra fee, they have a med tech who will give our Mom her meds (she has too many to keep track of herself). They always have one or more RN's on staff, a maintenance person if you need help putting up a picture, and activity staff who check on the residents frequently during the day to encourage socialization. They have cleaning service who keep our Mom's room neat and clutter-free since she uses a walker/wheelchair. The office is always open during the day if you have a question. Now I am particular about floors/smells, and being a nurse myself, I appreciate the cleanliness and fresh-smelling air of the hallway and common areas. The aides are so friendly and let me say- we all know how understaffed this whole state is when it comes to nursing facilities! The aides go above and beyond for their residents here. Our Mom is safe here and that is the most important! I tell many friends and family about Smithfield Woods for their elderly loved ones. Hopefully many more people will go take a tour and see how nice it is here!

Additionally, whenever I visited my grandmother, she was either in bed and/or wearing the exact same outfit I saw her in last. The final straw was that my grandmother was knocked to the ground by a CNA when trying to transport her from her bed in the middle of the night. If you know someone that is a resident here, check on them often and randomly.

In the time I've worked there witness patients with bed sores, ants in the patient's room and had to pull ants out of patients hair, out of their beds. The only patients that got the proper care were once that received visitor daily. Watch your family members carefully, they don't receive the care that they're supposed to when you're not around. I'm not saying this because I'm not upset I left this place I had a hard time looking the other way under these conditions.A CNA I don't like the fact that I had to show quality care to certain patients and not the others. As a CNA my job is to show quality care to all my patients and I couldn't work under these conditions at this place. I believe the state should check into this place, without their knowledge so they don't have the opportunity to just clean up knowing the state is coming. This is my opinion on this job and what I've seen and what I've learned from this place. Plus most CNAs are not formally checked on their bci.
