We have worked with many staff members over the years and every one of them has been passionate about the work they do. We are completely confident that she is receiving the best care available. There were so many decorated pumpkins to see and it was all due to the residents, staff and visitors who took the time to provide them. Bottom of the barrel Healthcare…read more So after being stabilized at Roger Williams's hospital, I spent three weeks in Elmhurst nursing home to recover from pneumonia. However, the lack of proper basic hygienic care and the mistakes they made with my medication, as well as the maltreatment by staff and the incompetency by CNAs, nurses, nurse practitioners and administrators led to my being rehospitalized with three secondary infections - some of which were serious. After my first week there I wanted to get a transfer to another facility and started gathering testimony from the different professional caregivers (non-staff) who work at other facilities, most of whom confirmed to me that they feel helpless at addressing these problems in this facility... They take for granted that the Marquis group corporate owners, will not allow them to get either the staffing or the supplies they need at that place to provide proper care, and after talking with different residents, I can see that both subacute care patients as well as long-term residents have the same complaints... I regret not documenting accurately enough to file a lawsuit, but I am filing complaints with the Rhode Island health department and publishing reviews everywhere I can online to let the world know, this place is BOTTOM OF THE BARREL as far as nursing homes go... what is taken for granted there is that mistakes (some of which will be life-threatening) WILL be made and the general attitude of staff is "not my fault, not my job, not my problem". The State needs to investigate how they operate that facility, they need inspections and auditing... The national Health review board has already removed their rating. There are 200 beds and nowhere near enough staff and a lot of serious medication errors and total lack of competency going on there.

The CNA needed to clean him up after he vomited. She had me leave the room and shut the door. My husband informed me that she was saying horrible things to him such as "who would want you. Look at you. You're useless". As he's telling me this he's crying. He thankfully only stated 24 hours because he needed to be transferred to the hospital. My heart hurt for him. This will be reported to a higher level. This place should be closed down.

I have no complaints about the CNA's, the nurses, how I was treated during intake, moving to another room, how they treated my roommate when he was brought in... They were so welcoming to him because he'd been there before ... So that Nursing staff and the PT/OT people were terrific. Everything about my experience there, including the food was great, and I would recommend them to anyone needing short term or rehab care. I cannot speak about what it's like to live there long-term I did not see that part of the facility, I was on the first floor ... That being said, I have a major complaint with the social worker/case management worker who completely BOTCHED my transfer to the Westview nursing home/rehab in Warwick, which is 45 minutes away and also run by the same company that runs overlook. I was never notified when I would be transferred, or what time, and nobody visited me to discuss discharge and go over medication and paperwork, and prepare me for the type of transfer. This was a huge problem for me. I am unable to breathe sitting up in a wheelchair for more than a few minutes. I've only ever traveled by ambulance from house to hospital from hospital to facility and from facility to my house in the past. Always on a stretcher. So when a lady in a soccer mom minivan showed up in 5° weather and they told me suddenly "OK get your stuff together. It's time for you to go!!!" ... I was shocked and upset. They had told the lady coming to pick me up nothing about who I was and what I needed and transport. So she was helpless. They were extremely incompetent in failing to prepare me for this journey, failing to explain to the driver what she would be dealing with concerning me as a patient, and not providing me the care I needed to be safe and secure and medically supervised in the transfer. What was so frustrating was with no notice and no time to get ready, they expected me to sit in the wheelchair in 5° weather in a T-shirt and no socks, and no coat while that lady fussed around with the van moving stuff around to make accommodations for my wheelchair in the back of the van, which she had to push up a steep makeshift ramp into the back of the minivan, and then get tied down with both side and rear doors open, so she could get in from the back and also from the side, not to mention find a place to store the oxygen bottle and make that safe to travel, all of which caused me to be freezing cold and lose my breath and have my heart rate accelerating into high tachycardia, bpm range - and there was no help by the facility staff - it was the worst experience of transport I've ever had and they were just un concerned and nonchalant about it. The lady who finally secured me in the minivan was not a nurse or have any experience that way she was just a driver. So I was freaking out and unable to get the care I needed, which is to stretch out and let my heart calm down before we start driving in traffic and I start being jerked around by the normal movements of a car in traffic... truly a horrific traumatizing experience, but I considered to be seriously lucky to have made it to Westview, despite tachycardia and high blood pressure and extreme anxiety during the whole journey. So I give them a four star rating for the great care of the staff and the great set up and the great vibe of the short-term care, rehab floor... However, my main complaint is revealing - both the the social worker and case management lady failed me big time in a way that put a serious risk to my health by screwing up how I got transferred to the other facility in Warwick.

She is in the advanced stages of ALS and requires a fair amount of attention -- she cannot feed or dress herself, and she is difficult to understand because the disease affects speech. But she is usually pleasant despite all of this, and I think the staff appreciates that. The caregivers are mostly very good, kind, and attentive people (a few aren't -- mostly the night crew, but I guess that's not surprising as not a lot of people want to work the night shift). But trying to get cooperation from the kitchen is just flat out impossible. I could actually give North Bay four stars if not for the really poor kitchen situation. The kitchen says it will take special orders and it does. But strangely, it's standard operating procedures that seem to throw the kitchen for a loop. For instance: - They asked my mother what she likes and doesn't like. One of the items she dislikes are mashed potatoes. The little card that comes on her tray says she doesn't want mashed potatoes and yet at EVERY DINNER for the past five months, she receives mashed potatoes - They sometimes forget to bring her a meal. I mean, really. How hard is this? There's a finite number of rooms requiring meal service and those rooms need to be serviced every day. Yet there doesn't seem to be a delivery plan. - Variety is completely lacking. Because my mother is in the advanced stages of her disease, swallowing is difficult so her food must be pureed. Lots of things can be pureed and yet they bring her some tasteless chicken (and by the way, they never identify what they are serving her) at 90% of her meals. - For weeks, she indicated she could not have milk products. They continued to bring her milk for many weeks. They still continue to bring her desserts such as puddings that are milk-based. - It is imperative that she has soup to help moisten even the pureed food. Yet the kitchen forgets to include soup in her meal about half the time. Asking for it once the tray is delivered takes about 40 minutes. Completely UNACCEPTABLE! My family has complained repeatedly about how poor the food service is here but nothing changes. I don't know why. If your loved one is particular about meals or needs special accomodations, I would strongly urge you to look for a different retirement facility. This one is untrainable.

I could leave feeling she was safe and well cared for, which was so important to myself and our family I would have given them five stars if not for the financial/management people who are another story. I could name names here but I won't. Suffice it to say that they don't return calls or fill out paperwork in a timely manner or correctly. I also had a situation where they wanted to put my Mother into the rehab unit at one point which really was not warranted. They were trying to get approval from Blue Cross. I asked to be called back on the Insurance status before she was transferred as I did not approve of this transfer without coverage. They never called me back. She was transferred. I went in to sign the code status papers to find out that they wanted me to sign other paperwork because they did not get approval. I was furious to find this out and that they wanted me to sign these papers. After speaking to someone there I was assured that I would in no way be responsible for the nearly $300. per day. It was not my debt. Needless to say I got billed for it. The bill came in my name. It took many phone calls to get them to stop billing me. Then they started billing me for back rent even though they were well aware that she had been approved retroactively for Medicaid. It took many more phone calls and letters. Hopefully this has been resolved. Time will tell.

He was so depressed being there that he stopped eating and starved himself to death. He had a stroke and he was almost non verbal but I understood him. so prior to not eating he was in a private room and without telling us or even explaining to my father what was happening they came in, collected his belongings and wheeled him down the hall to a shared room as he's saying "this isn't fair this isn't fair!" (he couldnt get any other words out). He stopped eating that day realizing that all of his rights and dignity were gone. This place left irreversible scars on my family. As he's dying we had to beg for him to get into a hospice room and all they would say was no..with zero reasoning. One day a nurse came in and said in front of my father that we could take him to the hospice room so he could die with dignity and we could mourn our father in peace. We cried we were so happy he finally was going to be able to be in a room that the neighbor didn't have the tv blaring and joyous laughter and conversation while he's literally dying. his roommates family was walking into our side of the room to use the bathroom all day. 5 minutes later administration comes over and says "we can't do that you were misinformed". Again, no explanation and again my father had no rights. This one lady didn't like my mom because she called her out for The poor care he was receiving and she called the shots so she took it out on my family, my father in particular. I walked in on a Wednesday. he was practically fallen out of his chair, I asked for him to be put in bed and that would be the last time his feet touched the ground. Our hospice nurse ordered meds which no nurse at the facility followed up on. We did. He needed comfort meds he was in horrible pain. He FINALY got his first dose and how relieved we were that this beautiful man was finally out of pain...they skipped his next dose "oh I thought he got it"-how dare you. He woke up from sedation in writhing pain. I watched helpless as I never left his side except to sleep. Admin knows exactly who I am and I never left his side. He died in my arms within two days in a room only a curtain away from a stranger with a tv blaring next to us. He deserved so much more. SHAME ON YOU ADMINISTRATORS. You know who my family is and you should be truly ashamed to even be in the business of caregiving because that is not what you do and that is not what you did. I hope you're haunted by the deaths you've so deeply aided in. There is so much more and most of it people wouldn't even believe, but I can't relive all of these events I only hope to help someone else. Spare your loved ones the pain of this. Nice Than Most…read more Sadly, I had to move my mother into this facility. Seeing that she has been in and out of several assisted living facilities in the past year, I have to say this is the nicest of them all. Clean, decent food, bright colors, and modern. They could have a couple more CNA's on per shift but for the most part they do a decent job. It's the best alternative for a bad situation. Thanks
