Last place you want to go. Nurses are nasty and do not know what they are doing. They let my grandmother sit in her fenced for 6 hours before they changed her. The PT came to work with my grandma and couldn't because she was still in her pajamas and shit filled diaper. 2 weeks in this place and she was dead. Do not send anyone you love here. Also her crystal vase was stolen, but I'm much more upset about them killing my grandmother. read more

We always pray for a place that people would care for us temporarily. With caring staff helping to dress us if necessary.

The time came when we had to make a decision on what was best for our mom. That decision was a Nursing home...but it's mom and we wanted the best for her and chosing the Wartburg made the decision easier. Top quality care in an amazing setting was everything we'd prayed for...speaking of 'prayed' for a facility with a real church and a community of older citizens enjoying each other's company. I live less than 2 miles away and I'm always 'dropping' in and I'm impressed with the care and attention our mom is receiving. The Dr's and staff keep us WELL INFORMED about our mothers health, Dr's appointments, weight, and have identified a meal plan that has stabilized her weight, that previously was dropping dangerously fast. I have to say that I was a bit worried about leaving my 95 year old aunt at the Meadowview at Wartburg but these poor reviews have been completely unfounded. First, the admissions person, was so kind and helpful throughout the entire process and even after my aunt moved in. John, the administrator, took the time to personally visit my aunt to check on her just a few days after she arrived. He even took the time to tape up a photo of my aunt's mother when she couldn't find her own roll of tape. A big deal when you are 95 and like to keep things in order. Moving her out during Covid was so heartbreaking but my mom has Alzheimer's and being the sole caregiver for both was proving difficult because the two didn't get along. Luckily I am free to drive by and waive and drop goodies off at the front desk. And once we get our vaccines we can safely visit. Still heartbreaking not to have her under the same roof and scary with what has happened since March 2020. I have to say the entire staff has been nice and accommodating. Sharon helped her get on her church Zoom meeting. They have provided assistance with things that I forgot. Audrey at the front desk was always kind and helpful when I had questions. Maria made admissions, a very scary and stressful time, a welcoming process. Her and Jackie kept us laughing the whole time which took away from the sadness. Little did they know my aunt and I were in tears just moments before. Grateful for the two of them is an understatement. My aunt has said that even though she is sad about quarantining in her room every single person she has come across was very nice. I had hoped that was the case because during our initial walk through I got the same response from another resident who had been there for four months. I can't wait for my aunt to get out and explore the property because she is a very social person and covid provided a ton of isolation for a lot of people. Dr Dava even spent extra time speaking with my aunt and is arranging to have someone make sure she is happy during this transition. He even took the time to call me and review her situation and changes in medication that were necessary. It was such a comfort for her to be able to see a doctor and not have to travel to midtown Manhattan. The whole team is amazing. I really cannot say enough good things. It's only the second week but the first week is always the hardest. I have a business and I am taking the time to post because it is hard to have a love one leave home to get care from strangers but at Wartburg they have truly proven that strangers are just friends we haven't had the time to get to know. So I publicly want to thank each and everyone of you for caring for my aunt in the manner in which they have thus far. read more

After surgery for a broken ankle, I was brought to the Wartburg Rehabilitation Center. For the next two months, the registered nurses, CNAs and Physical Therapists gave me excellent attention and care. The nurse's aides made sure I was comfortable and well feed. The physical therapists built up my upper body strength while my ankle healed. Once the surgeon gave permission to put weight on my foot, the physical therapists instructed me on how to walk again. All in all, the care I received was the best. read more

My brother in law spent almost a year at this facility following a fall last October. He died this morning. We are his only family and we live out of state. While he was a private pay patient they called all the time. Nurses, doctors, rehab, everyone. Then after paying over $100,000 privately, when Medicaid/Medicare took over the calls stopped. The communication to us as his only family members was nonexistent. He developed pneumonia, a collapsed lung and COPD under their care and we were only notified when they transported him to the hospital. When he returned from the hospital I called at least every other day to get updates on his condition. All I would hear is "Mr. Roger is doing fine." And then I would ask them to have his doctor or the nursing supervisor call me. In the past two months I have called almost every single day with a request for the doctor or the nursing supervisor to call me and I did not receive one return call. Not one. No one - not a nurse, the medical director, the nursing supervisor no one called us to update us on his medical condition after returning from the hospital while on a bi-pap machine. Until today-----when they called to tell me that he had passed away. I would not leave my worst enemy under the care of this facility. I sincerely mean that. read more

My Dad is suffering from dementia and has been a resident at the Waltemeade facility on the campus of Wartburg Skilled Nursing in Mount Vernon, NY, for 9 months as of the date of this review. The nurses, aides, social workers, therapists, cleaning crews, kitchen staff, spiritual counselors, and administrative employees have been very caring, friendly, and helpful. The meals are specially made for my Dad's condition, and are varied and well prepared. Precautions are taken to ensure the health and safety of the residents. It was a difficult decision to have my Dad admitted to a nursing home, but our family feels confident that he's getting appropriate medical care, physical, occupational, and speech therapy, as well as entertainment activities. The importance of our religious beliefs are honored. I believe my Dad is still with us due to the attention he receives and the exemplary care provided by everyone at Wartburg. Visiting hours are generous and the campus grounds are beautiful. My family has no hesitation in recommending Wartburg Skilled Nursing facility for anyone requiring such care. Frank V. If I could give this facility no stars I would. Care is neglectful. My father needs assistance to eat . They leave the tray and later come back and take it away. I have gone and he was sitting in a soiled bed. I notified an aide walking by because there was no one at the nurses station and she said someone will be in. 45 minutes went by , no one appeared. read more

If I could give less than 1 Star I would. Had a loved one in rehab there very recently and was disgusted. The outside looks great. It looks like such a nice place. But don't be fooled. First - The PT staff was great, dietitians as well. However, the nursing staff was HORRENDOUS. Most of the nurses were so far removed from the patients. I will say there were a couple of exceptions. But just a couple. I did witness 1 nurse leaving a patient in a bathroom for close to 45 minutes. Disgraceful. Most of them had terrible, disrespectful attitudes. The opposite is needed when working with elderly people. A level of compassion and respect is required. I was hard pressed to find that there. It's a shame. I did hear once upon a time it was a nice place. That is clearly not the case anymore. Wartburg- pay attention to your nursing staff. read more

Just finished very successful in-patient rehab for hip replacement at Wartburg. Can't say enough good things about the care I received. The P/T and O/T programs were excellent, pushing me just far enough to improve daily. Assistance from aides was almost always timely and delivered with care. My only little niggle-some pretty funky food but always a willingness to provide me with alternatives. read more

A loved one was in the Wartburg 4 months ago after having been hospitalized for heart issues. The nursing staff and the physical therapy staff were extremely helpful. However, there are some serious lapses in the care and discharge process that could put you or your loved one at risk. Similar to the review by the doctor, leadership/doctors at the hospital are not good at working with patients and families to prepare for discharge. Multiple times we requested to discuss discharge plans, as well as request lists of medication so we could ensure at home. Every time we were told you'll get that information when you're discharged. This leaves no time whatsoever to arrange for in-home care as needed and to ensure that lifesaving prescriptions are available at home. One would hope that the social worker would be an ally in the process, but requests for help were usually met with hostility and condescension. With a few key changes, the Warburg could be quite good, but the overall attitude towards patients and families keeps it in mediocrity. read more

My mom was in the Wartburg for 4 days this past August 2019. It was HORRIBLE!!!! I could not wait to get her out of there and I did. First let me start by saying she was there for rehab in building # 14. The building is old and needs a face lift. Every person who came into her room never said hello or who they were what they were going to do to her except for rehab and the dietitians They never came into the room to clean it. My mom had a brain bleed so she could not really talk or mover her right side. They were supposed to turn her every 2 to 3 hours and they DID NOT. When it was time to change my mom the aids would tell her turn to the side when they new she could not. The doctor ordered a nebulizer treatment for 7 days she never received it, they gave it to her room mate and took my mom name off the machine. I had a fight with the night nurse who tried to OVERDOSE my mom with her seizure medication, she told me if I did not like it to take care of my mother myself, and I can tell her supervisor and I did. That same nurse was talking on her cell phone while giving the patients their medication and writing her evening report for the next shift. The next day I wanted to make sure the day supervisor new what happen the night before, she told me to meet her in the lobby to talk and that is where we spoke. I was upset because she did not follow any HIPPA laws. The aids were constantly playing on their cell phones when sitting with the residents in the day room /TV room. Every time my mom would call using her call bell they would make her wait and sometime they never even helped her when she need help, they would say what do you need and then ignore her. My mom cried and begged me not to leave her, she was too scared to be alone with them. My family was not happy. read more

My elderly mother went to the "skilled" nursing unit for rehab after hip fracture surgery. The first night, the nursing manager decided she had an upper GI bleed and wanted to send her back to the hospital or bring in hospice. He based this only on a dirty diaper, and it was WRONG, but it sure did cause a lot of angst and confusion. The dinner that night was fried fish on white bread with french fries. The next day, the staff neglected to change her diaper all day. The room was dirty - huge pile of ?? stuck to the floor. The staff never noticed that she wasn't eating anything. The only saving grace was the occupational therapist. I got her outa there as soon as I could. read more

If I could give zero stars, then I would. My husband stayed at their rehab a couple of years ago. Although it is a very pretty and modern facility, he was not given good care. He was left in a wheelchair most of the day. His PT included putting velcro weights on his ankles which gave him a pressure sore (bed sore) which took months to heal. On top of that, they did not want to give him a home prescription for oxygen which he needed 24 hours a day after he was recovering form pneumonia. I eventually was able to get the oxygen prescription after meetings and much stress . I was treated very unkindly by the woman who was in charge at this facility. I would never recommend this place to anyone. They did not even provide a diabetic for my husband so they just gave him insulin ( which he never needed before). On his last day here, they left him in the lounge area with an empty oxygen tank and he was near collapse. He was hardly able to walk and we hadn't received the walker that was ordered. We had to literally beg them to borrow a walker to get him home ( we returned it the next day). read more

Please note that Wartburg has many misses. They DO NOT offer a diabetic diet preferring to pump their residents with drugs rather than give them the proper food. The food is terrible and is served on paper plates with plastic forks. After dinner in Memory Care, they are brought to the TV room where they're placed and left until it's time to go to bed. Most of them are sleeping because they're offered no stimuli. It has been observed that staff are not kind and just want to go home; unfortunately, the residents' care is below par. The psychiatrist is always on vacation, but when she comes in she prescribes psychotropic medications whether or not indicated. This is a terrible place and I would have given it no stars had that been a choice. BEWARE!!! read more

Both my mom and mother inlaw are currently at Wartburg for rehab. My mom has a room at the new facility and my mother inlaw in the older building. Both have received excellent care and are thriving. The staff has been attentive and accommodating. Both enjoy the meals. The grounds are beautiful. I refrained from giving the facility a 5 star rating, as a bathroom light has not been fixed in a couple of days... I can be forgiving in this area as I manage a large facility and understand that some things may take more time to correct. All in all, my mother will require additional surgery, and I will be requesting Wartburg for her rehab. read more

My mother entered the Wartburg's Meadowview's memory care facility in 2013. She passed away in 2019 at age 99. During her time at Meadowview she was treated with respect and love, and given the dignity that a person in severe dementia deserves. With minor exceptions, I visited my mother every week, and found the staff at Meadowview, and their dedication to the residents, to be simply wonderful. They prepared the food, made up the rooms, took care of residents' health and hygiene, and made sure that she was always dressed in a dignified way. In emergencies they always did the appropriate thing. They were always there for my mother, day and night. Doing all this, continually over the years, is not easy and simple. It takes thought and effort to make this work for the residents, and this staff pulled it off. In all of these years, everyone that I interacted with was totally dedicated to the welfare of the residents. My deep thanks to everyone at the Wartburg and in Meadowview. read more

A once great assisted living and rehab maybe gone sour. The people who sell you on the place are not the people who take care of Mom. We found $11-dollar-an-hour, resentful, sometimes just plain mean care staff. No training for those not naturally kind and caring, although there are some of those superb people still there. Evasive and negligent nursing. Out of touch administration. We had to move out at age 92, no easy process. read more

My recent stay at Wartburg for short term rehabilitation was wonderful. After total knee replacement and three nights in the hospital I was very happy to get settled in at Wartburg. After the initial flurry of intake activity, I was able to settle in and get comfortable. The decor is pleasant. The staff is superior. Any request that I made was met in a timely manner. They are friendly and attentive. The therapy area is bright and clean. The therapists are wonderful. I felt that I was with kind and caring friends during this stay. I was able to return home after 2 weeks. I can't say enough goods things about Wartburg and would highly recommend it to anyone. read more

I spent almost two months at Wartburg subacute rehab after ankle surgery. I have the unique perspective of a physician. The room was Spartan but kept clean. The grounds are well maintained BUT if you are unable to walk you are limited to a small patio area. There are virtually no diversions provided to your day except for an occasional bingo game and the occasional singer. There are no arts and crafts offered as part of occupational therapy. A clergyman came to my room two or three times to offer communion - he should have first checked my religion (I'm Jewish). You are asked the day before to choose the three meals and you can also order from a wide range of choices BUT they sometimes get it wrong and the hot meals were usually cool by the time they brought them to my room. I thought the rehab was good until I came home and had a home rehab that was much more carefully tailored to the requirements of my orthopedic problem. I also found out that one of my weekend therapists was not a rehab therapist but rather a rehab therapist assistant who knew next to nothing about external fixation and did not identify herself as an assistant. Only then did I realize how skimpy in time and attention is the inpatient rehab. Although my occupational therapist was very pleasant and we spoke daily, one is sent home without anyone in occupational therapy doing a pre-assessment visit of the home environment - this is DANGEROUS! I needed to quickly install grab bars in a bathroom, order walkers for each room including a special narrow walker needed to fit through the bathroom door and I ordered a scooter to get around my coop building. Diabetic needs are not well provided - there is no diabetic diet available and they frequently overlooked checking my fingerstick glucose before meals when they should have administered insulin. Pain medications are given promptly. A major deficiency is an inadequate number of nurses. The admissions office tells people that the nurse-to-patient ratio is 1:7 but they include nurses aides in their math. At night the ratio is in the 1:20 or 1:25 range which is inadequate and DANGEROUS. Indeed, the nursing stations in the rehab building are completely empty at night - no nurse, clerk, etc. The nurses are generally pleasant and competent but have such a large patient load that they must spend virtually all their time administering medications. The discharge process is disorganized. On my scheduled discharge day, the unit clerk and nurse were not aware that I was leaving and prescriptions and a copy of the medications and lab results were not ready. To inform me of my impending discharge they gave me a form intended for Medicare patients (I have private insurance) and the telephone number to appeal the discharge was therefore incorrect. I had an external fixator on my leg after surgery - the rehab people and nurses were not familiar with care required. The wound from the fixator were not properly cared for and I required an extra surgery to remove pins from infected pin sites. When I came home the care by home nurses was much more careful and effective in preventing infection. Indeed, the wound care could only be provided on most days after 11PM - I was told I could have it at 4AM as the only other choice - this the result of an inadequate number of nurses. It's sad that despite all these limitations Wartburg rehab remains one of the better inpatient subacute rehab facilities in Westchester. Final warning - make certain that you are getting a room in the newer rehab building rather than the older building where you may have to share a room. read more

My father is in the new rehab building, after hip surgery. The facility is beautiful but BEWARE. Do not leave anyone you care about alone. The people that work here are nice but there are not enough of them. Time for pain medication? They told him they ran out, can u believe it? He called me frantic at 5 am and I had to run over (30 minutes away) as I could get no one on the phone. Upon arrival they said "yes the drugs have not been delivered today". After several calls and following the nurse on duty around for 39 minutes he received it. If I did not get in my car and get here he never would have gotten it. We called for assistance to go the bathroom, per protocol. No one came in 15 minutes. I went to find someone, they were busy. He was mortified he was going to poop his pants, so I stole a walker from the room next door and took him myself. They came an hour later. We were warned about the lack of staff here, do not come here unless you can spend most of the day with your loved one. You need to go over everything, everyday. Especially medication. I feel for these people that work here, they are doing the best they can, in very poor working conditions. Don't drop your attention for a minute!!! read more

Stay away from this place! They have a bunch of thieves on their staff. My aunt is a nun who receives $600 a year for her service for God. She has not taken a vacation in 3 years so she saved $1800. She stayed with a friend one night and came back to her room to find her money gone. They went through her personal belongings and now she feels very violated. The supervisor will not do anything to make up for this because he says they have a safe in their office for people to put their money in. However , my aunt does not want to give up all her freedoms since she is of sound mind and body. Her convent simply shut down and the diocese put her here because she has vision problems. Many of the other residents are complaining about theft too. They are predators of an aging population. I advise everyone to put cameras in your room since the staff can go in and out of your room all day. No safety. read more
