The resulting costs is to be an 80/20 split with patient cost of course 20%. The problem comes in if they (provider) exceed the Schedule Of Benefits as outlined by the insurance company, and they routinely do (it's all about profit). They start services BEFORE the insurance has fully verified coverages. This is where the problem comes in. The insurance responds to the submission that something isn't covered for whatever reason, they're not covering xyz, and Loudoun Periodontics tells you "oh, the insurance didn't cover this because..." and want you to pay the outstanding claim. I'm going through this exact scenario. My total bill was $2684, my cost $659.80. After services were rendered, they submitted to the insurance company (I thought they had a pre approval) the insurance denied the claim stating they performed a service they can only perform every 2 years, and it's only been 1.5 years. Now they want me to pay the entire $2600! Stay away from this group. You have been warned!

The office is about 10 inconvenient miles away, he's not always been in my dental plan, so I pay for the privilege of going there. It's worth every penny.
