This review is of the Springdale Village post acute rehabilitation facility as there are several different types of Springdale Village facilities in Mesa, Arizona! I can't say enough good things about our experience at Springdale Village Post Subacute Rehabilitation Facility where my sister was recovering and receiving physical therapy from a broken leg. The facility is clean, well-run, and filled with people who clearly care about their patients. I especially want to recognize Robert K., my sister's case manager, who went above and beyond in every possible way. Robert was an absolute blessing during a very difficult time for our family. He worked so hard behind the scenes to make sure everything was in place for my sister's care and discharge plan. He was always professional, organized, and on top of every detail but what truly stood out was his kindness and compassion. Robert is truly amazing at what he does, and it's clear that he genuinely cares about his patients and their families. We are incredibly grateful for the level of support he provided. Thank you, Robert K., and thank you to everyone at Springdale Village for making such a positive impact during a challenging time. Highly recommend this facility! read more

My mom went to Springdale village on 6/13, we arrived approximately 5pm. My mom suffers from dementia and stole incontinence as a result. She did not get her night meds until after 11pm and then didn't even get all of them. There was one cna and one rn for 28 patients. It took well over an hour for anyone to come help change her when she had an accident and the man in the room next door was yelling all night long. Had my Dad not been there (all night in a chair) I'm terrified of how we would have found my mom in the morning. We were promised all light and roses and it was the most horrible experience, we removed my mother first thing the morning of 6/14 and the staff had no concern about this whatsoever. I will not recommend and will be lodging a complaint with the state licensing and Medicare Monday morning. Cannot even give this place 1 star. DO NOT SEND your loved ones here!!! read more

My mom stayed here for 4 nights after a recent hospital stay. The building is older, although I think they may be renovating it. Her room was very warm the whole time and they didn't/couldn't adjust the temperature. I ended up bringing her a small fan to try to help. The nursing staff is kind, but many of them seem to be a bit incompetent. She's on oxygen, and needs to wear a bipap at night. The nurses that were there for the first two nights didn't know how to use it, and even when I tried to show one of them, she shook her head and was unwilling to try. She didn't get her meds until the evening of the second day she was there, and the schedule for giving meds was inconsistent. The physical therapist she saw was wonderful. The food was just OK, but she didn't really get to choose anything from a menu. One woman came around on Sunday and took her order for Monday, but the rest of the time she just got whatever they gave her. I tried calling the front desk to arrange transportation for a doctor's appointment, but the woman said to call back later. When I did, I left a voicemail, but nobody ever got back to me. I ended up taking the day off to take her myself. I was able to get her discharged after 4 nights because I just didn't feel confident that her needs were being met there. When I arrived to pick her up, several people rushed in to get her to sign some documents that were supposed to have been signed upon her arrival. Looking at older reviews, I think that this place was probably great in the past, but the current management could benefit from tightening up some things with staffing/training. read more

This place doesn't deserve any Star- If you want your loved one or friend to just be a number and not take care of here go ahead take them - but if you're looking for proper care - DO NOT take them here. Banner Baywood, sent said patient (Retired RN and a Veteran) to this property for PT & OT care. Patient was placed in room at approximately 4:30 pm. Patient never received a call button, never received any CNA or LPN/RN assistance. When night time came she was yelling for someone to help her for over 2 hours, when person finally showed up she was told to "Shut UP there are patients sleeping." Here is a list of things that did and do go wrong: 1- Patient is Diabetic- they are liberal so they do NOT have diabetic foods. The foods they serve are high Carbohydrates and loaded with sugar. Patient can't eat the stuff - patient is well aware of her diet requirements and keeps her sugar counts in tact. 2- PT tone 3 hours a week - patient had PT three times in 9 days and very little OT. The PT as I witnessed it was a total of 10 minutes. Patient will never get better with 10 minutes. 3- Showers are offered twice a week per their statement- showers are NOT offered you have to demand a shower, patient arrives on Wednesday evening and had one (1) shower in 9 days! 4- Staff writes things into the notes that doesn't take place. Such as - patient had a BM CNA had to help clean patient after BM. Patient NEVER had a BM in the time patient was there. Patient and I made mention of it several times. Patient was given suppository but NEVER had a BM. Yes, records state she had multiples - but she NEVER did. 5- Staff states that patient was given medications, and that sugar counts were taken, and insulin distributed. Today patient NEVER received her insulin before leaving this property. 6- patient can't get out of bed on her own, there for she couldn't get to the sink to wash up, brush her teeth or anything each day. They do NOT care. 7- patient had a DR appointment- facility was to transport patient to the DR appointment. Transportation showed up to get her however it was too late for her appointment. In other words - they didn't set transportation up for a 30 minute ride. Patients appoint was rescheduled. They once again set up transportation to be transferred at the exact time of her start of appointment - We had to demand that patient be picked up 50 minutes before fore her appointment to arrive on time. 8- Social Workers - it's a pure JOKE, they are no help, are rude and if you want anything done you have to stand over them and watch to make sure things got done, like a transfer to a better Rehab Facility. 9- here is a real kicker the discharge paperwork- transfer came to pick up patient, RN came with BLANK paperwork and asked patient to sign the paperwork. Patient said it's all blank, RN said yes that's correct, I will fill it all in and give you copies. Patient refused to sign the paperwork until it was filled in. RN filled in the paperwork and then brought it to be signed, (did I mention patient is retired RN and a veteran) patient looked at the paperwork and said this isn't true. Patient asked me to sign said paperwork and write after signature - inaccurate documentation- disagree with information. Patient just want to get out of this hell home of a building and supposed care facility. Save your loved ones and friends DO NOT LET BANNER send them here! This visit for a relative was not good. They are short staffed and not able to keep up with patient care or physical therapy needs.

My 71-year-old wife had a stroke July 2022.I had her moved three times from bad skilled nursing homes. This place has pretty much seems to have thugs running this nursing home in the Administration. Not the employees that help take care of the patients. Yesterday, I was called into a "meeting" in an office due to my negative reviews which made the little man mad, and he told me he is Evicting my wife. said he is giving a 30-day notice to move. Very touchy man. After about 10 months of being there, my wife has a severe rash on her whole body due to them changing detergents in the laundry. I asked them about them changing back or trying a different detergent. They TOLD me no. Well, my wife had her dentures either stolen or tossed out by their staff... She has lost over 80 pounds since before Thanksgiving! Then, an Aide grabbed my wife's throat one day giving her meds. Bruised it badly and scratched my wife. I went straight to the Lisa's office, she's the Social Services lady to talk to her about all of this. See, I called Mesa Police to report what happened and an officer came to her room to interview my wife. Called in a Crime Scene photographer and took photos of my wife's throat. I made the Administration mad and they are "thuggish" in regards of anyone asking difficult questions about their neglect and coverups of it. I am tired of my wife being mistreated. The night nurse at the time kept tossing my wife's meds on the floor while my wife is asleep. My wife couldn't have even reached them. Another thing I went to talk to them about... Don't know where yet, but I am transferring her someplace else. We've had enough. read more

Springdale Village is a rehabilitation and retirement home, primarily for the elderly. Like many others they offer full care, assisted living, and independent options for residents. After my mother-in-law continued to have health problems and injured herself in a fall once again, she was assigned to this location for full-time care. Compared to other places, this one appeared very clean and well maintained. Much of the decor and flooring seemed new. It wasn't grungy and didn't smell bad like some other rehabilitation homes. While the staff was a bit rough around the edges at times, they still got things done efficiently and met all requirements. This location was no different from others in that the staff seemed stressed and extremely busy all the time. However, I attribute that to the fact that sick people can be quite needy and some demand constant attention. It seemed like they were keeping pace with all of the demands and needs of the residents, including my mother-in-law. This is not a bad option if you need to go to a place like this, or are looking for one for a loved one. Plus, it is cost effective as they accept most kinds of insurance. read more

I chose Springdale because of it good reviews. It would be the first time I was intrusting my disabled adult daughter to a faulty. It was for seven days. I was not happy when I picked her up. Her teeth haf not been brushes or her hair. I could assume that she had not gotten up, because her clothes had not been worn. She had scratches on her face. She has MS and dementia. She remembered nothing. And her sling went missing. The facility did take the responsibility for it. read more

SHAME ON THESE THIEVES!!! My MIL was a resident here and the care was just "ok". Now, sadly she has passed but maybe for the BEST! We just heard on the news that the rent for this place will be increased 92 percent within the next 6 months and many residents are being forced out since they are on a fixed income that does not accommodate an increase like this. Springdale Village was great for my dad who was in the hospital recently for covid and UTI which caused extreme weakness. They were able to get him walking again, up to 100 feet. PT and OT staff were great and worked him hard. He actually liked the food and he is a picky eater. The TV did not have the basketball games on TNT so we had to bring an ipad in for him. The aide Janice was the sweetest hard working gal I know. She was there with him almost every day. The social worker Lisa was very helpful to transition him to the next level. The wound nurse was great (although not her fault he got a heel abrasion some how). We would have kept him there for long term care but it was 25 miles away from our house. We elected to move him to an assisted living home near us, although it is not working out well. He is regresssing already, so we are looking into a different care facility near our home. Dad was begging to go back to Springdale. He really liked it there. read more

I stayed here for 4 weeks because I broke my ankle. I would definitely recommend this place. The people are very nice and take good care of you. Facility is very nice and clean. Plus the food is good. If I have to stay in rehab again I would definitely go back there! read more

I cannot say enough about this wonderful facility and the people that work there. My grandmother lived at the Springdale village for 10 years and from the beginning Gidget assisted her with her independent living. Making sure that all her needs were met. Then when she needed to move to assisted living Felicia continued to work with our family to make sure that my grandmother was watched over. Ron, the Med tech and all of the medical staff always gave my grandmother the best care. And recently, when she was moved to Hospice during the final days of her life they treated her with such care and respect. My grandmother truly enjoyed living at Springdale and I'd recommend this facility to anyone who wants to feel comfortable that their loved one will be treated with the best care. read more

I thought this would be a good place to go, boy was I wrong. I came here because I did something to my hamstring and couldn't get around by myself. Well, I might as well get around by myself because it takes them forever to answer the bell, so I stopped ringing it. I get frustrated and then I finally going to the bathroom by myself. First when you come here, they isolate you for 14 days. They barely ever come in to check on you and they show no compassion. It's really hard having no human contact for about 14 days. read more

The PT was Excellent and Christina and Nancy worked with my husband to get him up and moving and making sure he had exercises he could do on his own. My parents have lived in the independent living area of Springdale Village for 7 years. They absolutely love it, here. They have many friends and enjoy their company in the beautiful diningroom, or during bingo, or crafttime. The special concerts they bring in are fun and enjoyable. The staff work at involving the residents in the fun. They have taken very good care of the independent living environment and knows each person by name. As with all facilities, you have great apples and then there is the one that spoils it for all. The pandemic has made life very difficult for everyone. Learning the new requirements from the government and the CDC, was quite the challenge. The allowing or not allowing loved ones, from the facility, had no bearing on the professionalism or their seemingly lack of care. The rules passed down to them were put in place for the best interest of the resident's health. Also, the high pay people were receiving on unemployment, caused a low number of people in the hiring pool. This resulted in incredible, undue stress on the staff. Since the unemployment has finally come to end, I am hoping that the employment line will increase and allow for much needed help for the worn out staff. Yes, the food was challenging at times, but they have been working on it, and are doing a great job! Other issues have come and the wonderful staff stepped up to the plate to make sure the concerns were taken care of. With more than 300 residents, I have seen the staff go above and beyond, many times. Please remember, the staff are few and you are one of so many with needs, where each need is never the same. Yes, I would highly recommend Springdale Village. My mother loves it so much that she is responsible for nearly a dozen new residents over the years. read more

My grandmother was in Springdale's care for 5 days and now we are in the hospital and she is fighting for her life. They were unresponsive when we called to confirm her medication was given and to check on her. When she finally was released she had a fever of 105 and then when we get to the hospital find out she is SEVERELY hydrated and was not properly fed!!! If you care and love your loved ones DO NOT ALLOW THEM TO TAKE CARE OF YOUR FAMILY!!! read more

If I could offer no stars I would! My grandfather was in this facility and the care was horrible! The nurses never answered the call light. My grandfather waited so long for someone to answer the call light as he had to use the bathroom and finally got tired of waiting for someone on the way to the bathroom he fell and ripped open the skin on his arm, when grandma and I got there he was on the floor half in the restroom and half in the bedroom all grandma saw was the legs on the floor and panicked had it not been for the small voice we heard saying "Nurse, Nurse" we would have thought he was dead! I went in search of a nurse but there was no one in the surrounding nurses stations or in the hallways! I finally got a hold of the med nurse in the third station over where the double doors were shut and for good reason when you open the doors it smells of feces and urine and I almost vomited at the smell that wafted me when I opened the door but once I explained to her that my grandfather was on the floor bleeding and we don't know for how long she ran and got another nurse and wrapped his wound.... They would not call the ambulance to have him transported to the hospital said they had this handled.... the second time we had an incident where his O2 stats were so low he was babbling nonsense like a two year old the nurse refused to check his stats in front of us and said he did not need a concentrator for O2.... that was the last straw we called the paramedics ourselves and when they arrived the nurses tried to turn them away had I not been waiting for them to explain to them the situation they would have left at the nurses request however I threatened to sue the ambulance company if they left and they finally agreed to take him to the hospital where grandpa was immediately admitted and hooked up to an O2 concentrator after a few hours on the O2 he finally became alert and responsive and was speaking like an adult again. But because of the lack of O2 for so long his condition has deteriorated to the point of having to send him to HOS for comfort care. I wish I could give 10 Stars. The staff at Springdale are WONDERFUL!!!! I can't say enough of good things about them. It is everyone that works there. They are so friendly and make you feel like you are part of their family. They are warm, kind, and bend over backwards to help you and your love one that is now living in their facility. Gidget and Mindy in the Leasing Dept. are beyond Fantastic!!! They are very Professional but also very down to earth. They know their stuff and they are very friendly, knowledgeable and take care of any issues we might have. They are about making the residents as happy and comfortable as they can. They bend over backwards for their residents and their families. My Mom first moved into Springdale In February of 2010 and moved out in February 2020 to move in with her sister. She moved back into Springdale on June 21, 2021. Besides Gidget and Mindy, I want to give a Big Shout out to Felicia. She has been a great help with getting Mom moved in and all the paperwork that needs to be filled out. Any time I have a question, she has an answer. Whenever I call her or email her she always response back right away. I met the Med Techs Tina, Rita and Ron and just fell in love with them. They have been taking care of my Mom now for almost a week and we love them and so does our Mom. I met Stephanie and Bianca and they too are so friendly and have told me to let them know if they can help in anyway!!!!! I have a lot of friends that have elderly parents and I can't say enough of great things about the Staff at Springdale!!!!! read more

Please don't place your elderly parent in this rehab center. The nurses and CNA's were awful. The things I saw actually frightened me.

My father fell and needed physical therapy to be able to return home. We weren't given much notice from the banner hospital to find a place to bring him. At first this place seemed perfect. The first person we talked to was so helpful and facilitated the move easily. That was the only time that was positive in dealing with this place. Getting any updates or information was difficult and not accurate or helpful. I understand that things are challenging at best during the pandemic, however, customer service and patient care should not be lacking. I was advised that I would receive updates following the PT meetings on Tuesday afternoons- I was never once called, I had to call them repeatedly to get any information. then when I reached someone I was told that they scheduled to do a conference call with me at 1130- i was not asked if this time worked for me nor was i aware of it till i repeatedly called in for information. I was told one thing by social service and then received different information from one of the therapist who was very nice and explained the reports and the actual condition of my father and what was truly going on and what his progress was. This place is awful. They caused extreme undue stress to my family in an already extremely stressful situation. They left my mother crying every night, frightened of what was happening and what she would need to do for my father for care. She has a heart condition and was recently released from the hospital following another heart surgery one year after a valve replacement and triple bipass surgery. No service that is supposed to be a resource for families and a care facility should cause harm in the manner this place does. Would never recommend anyone go here. read more
