Upon leaving inpatient, I transitioned to sober living, which again was safe, enjoyable, and much needed. From the executive director all the way down, the staff genuinely cares about saving lives and doing God's work. This team is on the firing lines daily, and they know how to reach those that need it. From Reception to the PTAs to the PTs, all services were the best one could expect. The receptionist team of Vanessa, Morgan, Gabby, and others were always all smiles when you come in. The professional knowledge and expertise of Samantha, Steve, Charlie, and Erwin were evident in the care I received. The PTAs (Lauren, Caylee, Christian, and Josh) were very patient and informative in assisting me with the exercises and stretches I needed to get done. I am generally pain-free more days these days. Well done everybody! If I missed anybody, it was not intentional!!

They are all caring, understanding in all of the different ailments from all kinds of different patients. I want to thank Jacalyn, Jim, Claudia, Max and Ranon for all of the therapy and helpful ideas to continue my own therapy at home. The facility is very clean and doesn't have any kind of smell to it. I like the fact that they have ice water available all the time and staff will come around and ask if you would like ice refill and even a water refill. I would recommend this place for injury rehab and for your elderly loved ones.

My mom had dementia, fractured her knee and had to be taken there because I couldn't care for her…read more24/7 and don't have the means to have a nurse at home. -My mom's clothes got "lost" day 1. I dropped them off, name on each piece, including a small blanket. I visit the next day and she's wearing someone else's clothes. Her blanket no where to be found. When I went to pick up her clothes after her passing, they tried to give me a bunch of stuff that wants hers. And I only got back one shirt and 1 pant. Out of 4 tops and 3 pants I had bought her. -The place stinks -Nurses were good while we visited, other than that they didn't help her. -She got mrsa, within a week of her being there. No one noticed it until they had to call 911 to take her to the hospital, almost a week later. -Lost like 15 pounds in those 2 weeks. She wasn't eating and they didn't tried to get her to eat. -I asked all the questions, they "tried" to communicate, asked me different things she liked to eat to get her to eat, or liked to do so she can have some distractions. But nothing went any further than that. -Half of the time I visited I found her soiled. I visited every other day, as I was taking turns with my brother. All of this because we don't have the means for the good places. If your loved one doesn't have a voice, avoid this place. Because they won't take care of them at all. It was the worst experience for all of us. *The people who can speak up for themselves, were ok. This was mid-January 2026

My mom needed some rehab after a total hip replacement and we found Country Manor La Mesa. She was…read morethere for 5 days to strengthen enough to be safe to come home. I was impressed with almost all of Country Manor and wanted to ensure a recent review was entered. I am a nurse and have a "good", "bad", and "ugly" way of looking at skilled nursing facilities. I'm very happy to say that I only had 1 item in the ugly category, very few things in the bad, and numerous things in the good category! We'll start with the ugly and work our way up since I think most people are looking for what to be warned about in these reviews. The one "ugly" item is that there were no cleaning wipes to be used in the restroom. Mom was in a semi-private room and the bathroom is shared between two rooms (4 residents). There were no cleaning wipes in the bathroom despite me asking at least 3 times. She needed a commode the first few days and after that I was there with her most of the time so I was sure to clean the bathroom toilet with soap before and after she used it. As far as "bad", we struggled with the staff's mentality of short-term rehab post-operatively. We expected she'd be there only a week or so until she could get strong enough to be safe at home and so I wouldn't need to lift and move her myself. I had to constantly remind them of this as they would refer to her stay as "in a few weeks...." or "eventually, you'll be able to.... so you can go home....". I told the care team in our initial admission meeting that our goal was to go home on x day, which was 7 days later, but no one seemed to recognize that this was very possible for her. Another item in this category was the 'stacked' therapies. Almost every day, the OT would come in to do work with her for about 40 mins and then the PT would immediately come in afterwards. It made my mom even more tired and in pain than she started out and made it difficult to show that she was improving enough to come home. The final thing on this list was the follow-through of the staff to the call light. The aid responded very quickly (within a few minutes), but if it was medication needed, the nurse didn't get the message. This happened on at least 3 occasions and we'd have to call two or three times. I'd also add to this list that the staff should wash their hands in the room prior to doing assessments and treatments so the resident and family can see them doing it. They'd use the hand cleaner outside the room, but it's good practice to let us see them washing. I also would recommend that there be a written 'welcome' pack to residents and families that includes things we weren't aware of, such as meal times, shower days for the beds, how to request an alternative meal, and the times that the staff change shifts. Finally, I'd recommend white boards installed at each bed to help orient residents to the day, date, and who the staff are for that shift. It would also help to state goals for the rehab patients and be a place to write questions to ask or put reminders. For the "good" list, I can't say enough about how friendly everyone was! They treated my mom very well and were patient with her when she couldn't hear them. I was not surprised by the staffing as this level of care typically has one nurse with an aide or two caring for a high number of patients. They were still generally quick to respond to our requests for ice packs, snacks, ice, water, and food requests. Special shout-outs to OT Tara, Rosemarie from marketing, Dr. Abesamis who listened to us and our plan, Tish the Activities Director (sorry we never had a chance to teach you MahJongg), and PM nurse gentleman whose name is escaping me. I felt the facility was very clean, didn't smell like either body fluids or antiseptic, the food was very good, and the grounds were immaculate! We really enjoyed being able to walk out to the patio area and sit in the sun! There was plenty of parking while I visited each day and it is close to the Vons in case I needed to pick up anything. The birds in the lobby were great and mom loved going and watching them each day. There were a lot of activities although mom didn't participate in many. A very lovely place for a skilled nursing facility! I'm so glad that mom was there! Thank you, everyone!

A few weeks after she was admitted there, my Dad also needed skilled nursing. The admin folks at Country Manor were so kind that they figured out how to put both my Mom and Dad in the same room even though rooms are not normally mixed gender. Both of our parents received great care throughout their stay. The staff and admin, almost without exception, are very caring, considerate, and kind. They helped us through the death of my Dad and continue to care for my Mom.

This facility will not accept a motorized wheelchair in their facility, even if we said it would just be parked in their room so my loved one had a proper and safe place to sit that can be adjusted to their needs. After hearing this we never admitted our loved one. Care at home was a much better and safer option for us.

After…read moreover a year of making calls to try to move her to a closer facility for our family visits, Claudia was finally able to make it happen for us. She and I had been in touch weekly for over a year and she never lost her patience with me or got annoyed when I called the way other places did. Instead she was apologetic when there wasn't an available bed. The admissions staff was very caring, detailed and informative from the beginning till end. We stayed in close touch with Crystal through the care conferences, when my grandmother had to be sent to the hospital and back, along with when she had to go on hospice. Crystal is kind and considerate. The staff took care of her through her occupational and physical therapy sessions with Alicia and Melody. She was able to self feed and sit up right in a wheelchair again. They got her moving more than she did at her previous SNF. Her dieticians Zakirah and Yesica provided a great diet for her that she enjoyed until she couldn't anymore. I let them know she loves her sweets and they helped make it happen. Her last bites before she passed were pudding and Ensure fed to her by her nurse supervisor, Vivian, who took extra special care during the last days. They built a connection because Vivian spoke Vietnamese to her so naturally our grandma took a liking and was more responsive. Vivian even took the time to stay a little later so she could feed breakfast to her. Dan (charge nurse maybe?), was very detailed, gentle, and graceful in his responses when I called to ask questions about her status. And when he had to break the news, I felt the empathy in his voice over the phone. Her other RN's and CNA's (I'm sorry I don't remember everyone's names) including Adona, have all been a lovely presence in her final stages. My grandma always appeared very clean and fresh and lovely at every visit. The janitorial team is fantastic at what they do too, keeping the facility clean and spotless. Protective PPE is always organized and right outside the door for easy use. It doesn't smell like a SNF. Her last SNF didn't smell clean. Highly recommend this facility and their staff! Thank you for all you have done to take care of her with care, respect, and dignity till her last breath.

They are under LOTS of lawsuits & they violate lotsssss of labor laws. Do NOT send your loved ones here if you love them, this facility will not extend their time here with us.
