    PLEASE STAY AWAY FROM THIS FACILITY!!!!!! We had my mom go here for a respite visit (5 nights) she was admitted June 3rd, 2021 at 11:45am. The following morning June 4th, 2021 I called the facility to check on my mom when the intake coordinator stated "we have more paperwork for you to sign" ok cool I'll just pop in and visit my mom while I'm there. Dear lord.... When I walked in roughly around 9:15am I found my mom now comatose, laying in her own feces now dried to her and still in the same diaper she was admitted in. (I packed different colored diapers for her stay) her catheter hose was twisted shut, the humidifier was placed back on her oxygen machine after they were told to not put it on because my mom had lung cancer & there was SERIOUS concern of aspiration of fluids, my mom had not been administered any of her medication the first day she was there, I tested my mother oxygen levels and they were low 70s. I immediately went to the nurses station to ask them to explain to me why my mom was being neglected and they all just stated at me like I was speaking a foreign language. I called my hospice team and requested and emergency transport from the facility. As soon as I went to walk back down to my mom's room 2 nurses showed up with a sitting shower chair (my mom was bed bath only AND comatose) I told them to get out. Then another nurse showed up to try to change my mom. I told her: "Get out, I trusted you to take care of her when I wasn't here & clearly you cannot do your job. I'll change my mom myself" I refused everyone who came to the room until my transport team arrived to take my mother home. At the point where my transport team and I went to wheel mom out the nurses tried to tell me: " you can't leave with her, her oxygen is too low" my response was: "trust me, I know! And you only know because I did your job! Can you tell me how long my mother has been deprived oxygen?" They had no answer. We got my mother home.... She died less than 8hrs later on June 4th, 2021. This has been reported to the California department of public health. The San Diego county ombudsman, and I've contacted an attorney. I have witnesses, and even photographic evidence. My mother didn't deserve this & neither does your family member.

    Wow. This is the first time I can say a place and the team has gone above and beyond for their patients and visitors. I am so glad I took a chance on this facility after doing my due diligence of reviewing all the highly rated rehab facilities in Escondido. I live in Rancho Bernardo but my mom lives in Escondido. The drive alone to visit my mom was worth it because I knew she was in great hands. Thank you to all the staff. The process was smooth from check in to check out. Thanks everyone!

    My sister was admitted to Valle Vista on June 25, 2021 and my family removed her the following day. She was transported from a hospital to Valle Vista to regain her strength following brain surgery. When she arrived, they had nothing in place for her. (NO pain meds and NO "food" for her feeding tube) They weren't sure if those items would arrive before morning. The charge nurse hadn't shown up for her shift so the nurse at the desk was edgy as they were scrambling to get my sister's room set up. It took three attempts with different oxygen machines before one worked. (Faulty machines....outdated electrical outlets.....or both?) Because of "policy" a family member wasn't able to stay the remainder of the night in her room to see if she was safe and being attended to. Besides the lack of medical care, the room was run down and depressing. The wall behind one of the beds had been repaired but not repainted (big white patch). The remote to adjust the old bed was yellowed and dirty. There was an old fashioned fluorescent light hanging on the wall next to the bed but the string (attached to a little chain) to turn it on was at the foot of the bed. How would a person who is rehabbing possibly reach it? I couldn't stomach going in the bathroom but another family member did, and suggested that I avoid it. While looking around at the faculties I couldn't help but think, "With all of the job availability in the health care field, who would choose to work here?" And then it all made sense! Ugh!

    Great facility with helpful and hardworking staff. Really enjoyed visiting before the pandemic. Lovely view out the dining room too!

My first pick, for sure. A family member has been here for nearly one month due to an illness requiring the level of care provided at a skilled nursing facility (SNF). I've visited at least 5 to 6 days weekly. Every visit I am more and more confident/pleased with the level of care and cleanliness here. You enter the bright/light building and are warmly welcomed/greeted; currently there's fun, festive decor through out. The holiday decor is a great addition, nice touch to bring some holiday cheer. There's a beautiful library/room for residents/guests, not quite complete, but nearly ready/renovated. The entire property is slowly being updated and renovated and it shows. It is evident that there are a lot of changes taking place here (literally and figuratively). The property has been maintained pretty well, considering. All of the staff from the LVNs, nurses, therapists (OT, ST, PT) and so on have been extremely helpful and provide the type/quality of care to match. My family member is always treated with respect and dignity, no complaints expressed. There are daily activities (often enriching), a gym, and the food menu is quite extensive. The meals have been described as both "nutritious and tasty." The dinner menu offers a month of varied meals; without repeating a meal! They even asked if there are items my family member doesn't particularly like. Some additional things I've observed, the staff is able to communicate clearly and collaborate effectively. The staff has been attentive and responsive; exceptional round-the-clock care, truly. There's a large, private parking lot, so parking isn't an issue here. Additionally, there are policies and procedures to protect residents' possessions. They take a thorough inventory of personal belongings and provide plenty of storage space for the belongings. Things haven't disappeared here. A BIG shout out to Tine and Wendy, so kind and caring. They truly are exceptionally hard working, so empathetic, and patient. Escondido Post Acute Rehab offers a high-quality skilled nursing program. One of the best SNFs around! The administrator, Daniel D. is empathic and eager/open to feedback. Thank you for being open, ensuring quality care, managing staff, complying with regulations, all things! I am happy my family member is here during all of these great changes, wish we had discovered this rehab five facilities sooner!

I had read the reviews before being transported…read morehere so already I was skeptical, but my case manager said there wasn't another option based on age (I'm in my 20s) and I'm sure my insurance. Good things: PT Team was great! Was able to get me walking again after only a week of being there! Sarah and Melanie were always so patient with me and listened to what I needed. Marsia my nurse was the best thing about this place! She advocated for me and pushed for me to go back to the hospital when I was having complications from my medications and the doctors didn't want to do Jack! Okay things: Most of the CNAs were nice. The younger ones tend to be the nice while the older ones can get impatient and short with you. But don't rely on a quick response, 30 minutes to respond to a call light! Bad things: No one checks on you! You have to request to have your sheets changed. Doctors only show up once a week when they want to to check on patients and you can't request any medical needs like labs till they are there! And even then they are likely not to do anything! If Pat comes into ur room when you have a call light on don't even bother asking her for anything, she won't do Jack and and say you can do it yourself even when you literally can't. Even if you get sick right in front of her she can't be bothered to help. Make sure you take your shower when you're told to otherwise you'll be thrown under the bus! To top it all off food is crap and give you like 15 minutes to eat and if you're not done then they will just leave your tray till the next meal time. Rooms are also very small and no real privacy! If you have other options choose another place, but if this place is your only option good luck to you. This place needs a lot of improvement!

I was told by Jenny that Records Department Manger had only been employed a week this location and wasn't there today. I requested for Administration to give me a return call. At 4:49 pm on 2/2/2026 I didn't receive a return call. 6 hours later so I called again to no avail. On 2/3/2026, I called again at 1pm requesting to speak to Records Manager. At this time Jenny told me Records Manager was in meeting all day and couldn't return my call. This is ridiculous and so unprofessional. The family is desperately requesting records for their loved one. This is requarding a precious like that was lost and facility is displaying NO concern or consideration for the family. Is this the type of facility you would trust your loved one at?? Think twice!!!

I came in on a gurney and walked out two weeks later on my way to a normal life. The staff is incredible. They work you like it's Boot Camp, but one day of rehab is like a week on the outside. They said I was two months ahead of schedule and on top of that the food is pretty good fresh with a nice variety of greens and protein. The nurses and the nurses aids are all top drawer if you're unlucky enough to end up in a rehab facility it will change your luck if you end up here I highly recommend Palomar health rehab rehabilitation out of five stars.     Very disappointed in the care at this facility. My mom was there for 22 days recovering from a…read morestroke. I wouldn't recommend this facility for a patient that requires significant help or has mild cognitive impairment. Post stroke she was wheel chair bound and required assistance for everything. In summary: Dr. B was excellent. Thoughtful, problem solving mindset and treated patient and family with respect, and some humor when appropriate. Rehab Team: Competent and caring. Speech. OT AND PT good therapists. Management: Poor. All patients receive 2 consecutive days with no therapy. That is 2 days isolated in your hospital room with no activities. This is truly detrimental to the healing process. In my experience this exasperated anxiety and feelings of isolation, reversing progress made during the week, disturbing sleep cycles and recovery. It's very important if you are considering this location for a loved one that you reflect on this. Your loved one will be sitting in a hospital bed for two consecutive days with no interaction except a nurse popping in to check on them briefly. Of course family can visit and we did, however it is not a substitute for professional engagement. This workaround that administrators have finagled to ensure patients hit the certain hours required while limiting weekend staffing is absolutely horrible and very transparent in their motives. Nursing Staff: Very inconsistent and overall not good. Didn't appear in alignment with goals of therapy. I didn't feel safe most of the time with the quality of care my mom was receiving. Not all the time, there were a few standouts that were excellent, but overall very lacking. I share this info. so if you're considering this facility you know what to expect. And with hopes that administrators will make changes for future patients. When searching for a rehab facility I admit I was influenced by the the gold star ratings from outside organizations, but after my loved one's experience, I realize this is more a reflection of savvy administrators working to get accolades from governing bodies vs. true gold star care.
