5 years later, I still think about a particular incident at Anaheim Crest. When the staff moved my grandpa's bed, he had a look of shock, there was no delicate care in caring for him as a human being. Beyond that, when I had a question about his health, I asked a nurse and she said not to ask her because she doesn't look like the doctor like she literally said "don't ask me, do I look like the doctor?" I'm sorry but if you care about your relative or family member, please don't bring them here. I might be biased but I am rightfully biased as I had a first hand experience and it was me who drove my grandma and myself here to this hell hole to go visit my dying grandpa and I still wish today they (Anaheim Crest) could have done a better job. Please do better. Your lack of empathy, and how you take care of your patients still does traumatize me to this day. read more

This place is awful. It smells like poop & pee. You have to hold your breath when you enter.My sister was there for therapy. She was there for over a week before the started therapy. On the day before her surgery two nurses came in to give her a shower. Only to rub her skin raw and tangle up her long by wadding it up.She was so mistreated. It says that they get a TV but my sister was there for two weeks and never had a TV that was promised. The food is the worst. I wound never even put my dog there for treatment. I tried talking to the staff and they didn't care.If you love your family member NEVER place them at this home.AWFUL PLACE. SHOULD BE CLOSED VERY DIRTY & SMELLY.LOUD and NOISY.ALL NIGHT LONG NO SLEEP. read more

This place was awful. The facility itself is old and dated & the hallways wreaked of poop. My mother was put in a room and forgotten about and by the time anyone checked on her, her oxygen levels were so low she had to be sent back to emergency. I visited every day and no one ever seemed to know what was going on and if she was receiving the treatments and therapies she was supposed to. I asked for 5 days for her to have a showered after spending two weeks in the hospital without & i overheard the staff complaining about it in the hallways because it was "off-cycle". They were supposed to administer breathing treatments daily and didn't and when I was there and asked for it they came and set it up but never returned to turn the machines off. Send your parents here only if you want them to die alone in their stinky rooms. read more

My mother had taken a fall and broke a hip, the hospital and insurance transferred her to this location. It was late on a Saturday evening, the ambulance brought my mom in and I met them there. The front door is locked after a certain hour so I had to walk around to the back. I walked in was not greeted by anyone at the nurses station and was advised I could go wait by my mom's room while they admitted her? Well after my initial shock and my mother having to be there for several months. The staff grew o me, my mother loved this place and her overall stay was pleasant. I observed the kindest nurses, who went out of their way to sit with and talk to some long term patients who may not have family or friends who visit them. I just want to say the therapy team here at Anaheim Crest nursing has been fantastic in particular​ Seijin and Richard. These two have encouraged me to improve each time they work. They make me work harder. They make me believe that I can do things and they respect the things that I do. I request in need. They cater to the strengthening of the upper and lower body so everything is together and working is one. If it wasn't for them, I wouldn't be where I am today. I also need to mention the CNAs but Joanna and Juan and Kevin and Lucille these people have really helped me along. Also. They really care about their jobs. I just want to express how thankful I am to have found this facility for my mom during the COVID pandemic. She has been here for almost three years now, and I continue to be impressed by the cleanliness of the place--it never has that unpleasant smell I've noticed in other nursing homes. The staff are incredibly kind, attentive, and genuinely care about both the residents and their families. I've even asked my mom if she'd like to move to an assisted living facility, but she's happy here and has no desire to go anywhere else. That peace of mind is everything to me. read more

They offer speech therapy, PT & OT, as requested by my doctor. Their level of experise is fair but the speech therapist ,Paul, goes abive & beyond & is a caring person. My nurse, Madison, is someone else who goes above & beyond, administers medications on a timely basis. read more

These two CNA's at Awesome. Not only does Jairo and Johana have each others back, they know and take good care of each of their patients. Everyday they come to work they have smiles on their face no matter the day will be. Jairo and Johana go above and beyond. Us patients don't even have to ask them, because they know us so well they know what to get us before we even have a chance to ask them. Very poor care due to employee laziness. They ignore call bells for at least 30 minutes in an attempt to "train" you to ask for less. I'm very sorry I picked this place but tbh, I had no other choice at the time of my parent's discharge. Typical nursing home MD who is basically there in name only. There is also no garden area so that residents can take a stroll. read more

This place is amazing. They took care of my girlfriend Tonya who had a knee replacement surgery. The staff and Physical therapy team, nurses Cna's were awesome. My mom stayed at Anaheim Crest for a week after being discharged from the hospital for pneumonia. She was supposed to have IV antibiotics for three days and then go home but the nurses kept saying they couldn't discharge her without a doctor's order. Each time I asked to speak to a doctor, I was told the doctor wasn't in and they'd leave a msg for the dr. to call me, yet I never got a call back. The weekend arrived and still no call, I was then told there was no doctor on the weekend to discharge her so they'd have to keep her until Monday. All along they kept saying that she had insurance coverage through Tuesday and not to worry because they would discharge her when her insurance as up. During my mom's stay, she experienced hallucinations, I can only assume from either being sick or from the medications they were giving her. Despite asking three times for her to be evaluated, no one ever looked at her. The entire ordeal was extremely frustrating. My mom was stuck there alone doing absolutely nothing for days. I feel sorry for the patients who are stuck there long term. read more

What a wonderful experience was made from a tough situation for our sister with Special Needs who arrived at Anaheim Crest with a pressure wound and then Bilateral Pneumonia. We are so fortunate that Anaheim Crest was her home for close to 9 months. The Health Care Workers and facility staff took excellent care of her. Our family would highly recommend this Skilled Nursing Facility to any one needing extended care. Please visit them and make your own opinion, it's super clean, updated paint and flooring, very clean, right next door to a Hospital which is very important for your family member suffering with any ailments that would need a hospital visit. We have 6 siblings that visited her many different day6, different times, randomly and each and every visit left us with a great appreciation for where she was at. read more

Someone very close to me spent 17 days at Anaheim Crest. I spent several hours on each of those days. I was impressed with every aspect. There was activity everywhere related to helping the patients. The meals were good quality and on time. The nurses were angels. I heard one nurse who was unjustly accused of something twice. Each time that nurse apologized when that was not necessary. Nurses are prompt to answer requests, and always kind to us. Meds were administered on time, several times a day. Rehab was patient and kind with my loved one. The case manager was always available and always gave me quick and adequate information. I am grateful for the treatment we received an Anaheim Crest. read more

THIS PLACE IS TERRIBLE! From the moment my grandmother was transferred here. She was forgotten about, pushed her call button, waiting over 45 minutes for someone to come. This place needs to be shut down. They also billed transportation incorrectly, Mia the case manager said If i gave her info, she would have notes of it... THAT DIDNT HAPPEN THE LAST 4 TIMES I GAVE HER INFO! She is SO rude and talks over you... every time I ask a question. there is NEVER a doctor available to talk too either. I stay at Anaheim crest from beginning June till the second of Sept. I had great experiances and some very bad experiences. I want to talk about the great experiances first. I had surgery on left foot baby toe removed was bad wound,but They have a wound nurse that was short of a miracle worker her name is CM she knows her job. She had an Assistant that was amazing also Annie these 2 woman were my heros. The LVN'Swere amazing there were 2 LVN'S were best ever they were Angel also Catherine And Debbie . The CNA'S were amazing 3 really touched my Heart Jay ,Joanna, Guilda. Beautiful people. The administrater Christine was very sweet. The case managrr Jessica went Above and beyond for you. Savanna front office great activities good ok now for the bad and it got bad quick with rest of the peoplethe kitchen worst ever dont pay attention to food allergies so watch it and ignored dislikes and make sure you like rice chicken all you get tough breakfast very bad lacks any nutritional value . The Social worker is worst ive ever experianced how shes still there is a sign to me of bad management. Worst place ever, took my 91 year old grandmother there and was basically tortured all night making her get up multiple times and undress when it was clearly not needed she called us crying early in morning so we went and tooker out immediately!! Their nurses have the worst communication there and are so rude and dont care that your taking care of a human being and a hurting, fragile human being! We have submitted our case and it will investigated, for possible negligence and sexual harrassment. Do NOT bring your loved ones here! read more

My grand mother in law was transferred here from a hospital in LA. They wer in contact with the hospital discharge people which made the transition more easier. They even let us see her even after hours (coz she came after visiting hours)... each document that needed to be signed was explained as well.... Staff here are very courteous, helpful and always have a smile... which is very heart warming specially during this difficult time. Makes u feel at ease that ur love one will be taken cared of. Social worker, DON, PT, Nutritionist, Activity Planner took d time to meet with us and explain what their plan was and how she was responding to the treatment... discussed about them discharging her to a assisted living.. thank u all... read more

I have all great things to say about this facility. This is my 1st time ever in a nursing facility and the whole staff from the nursing, adminstrative, Drs, and physical and occupational therapist.
