I am a patient of David and Associates, I have tried for 2 months to get in to see the Doctor about a cracked tooth. The front desk told me Sherry would be calling me back. She has not in two months. I am army retired, went to David and associates in 2018. I signed documents after my procedure for 2 implant, and 2 veneers. It stated your work is quranteed for 5 years. Number 7 veneers failed so I came in to have it put back on. They examined mr, cut the tooth so low, the veneer could not be put back on. The only thing they could do is put a crown on it. They tried to charge me $1689 for a crown instead of providing the quarantee because I hadn't been back in two years. Why would I pay them when the VA provides free dental care and only after I received my implants began paying for them. The bottom line is you are probably still going there because of the Commercial, but military personnel should avoid them since you receive dental care now if retired with 100% disability. They had a review on TV for a single implant for $2499. I called, and they asked if I had insurance, and I replied I yes. She then told me the advertised price was just for people who don't have insurance though the ad didn't state this. I said what difference does it make. Then I asked what the cost would be with insurance to which she replied $3500. So basically if you have insurance the cost to you goes up $1000 for the exact same service. They just pocket the extra thousand dollars and you get no benefit from your insurance at all. Shysters. read more

Like the others, I wish I could give this zero or even less stars!! I can relate to every negative review here!!! $28k, 18 months, a million miles traveled back and forth from Tampa to Jax, not to mention a million hours of my time later, and I still have broken D5 implants. Same thing .... every time..... every 1-7 days after receiving my "finals"..... bottom right breaks, the upper left breaks...... EVERY TIME!!!!! I have had so many "finals" put in my mouth that I have lost count at this point. The set I just got two weeks ago.... already broke. So, I've had at least 25 sets made and not one has lasted longer than 4 weeks. I'm not sure what to do... I need to get off this hamster wheel of letting them do the same thing over and over again with no better results.... but I am out 28k and can't stand the thought of having to pay someone else to fix their mistakes!! Has anyone else who has had these same problems been able to resolve your issues? If so, please advise how you did it!! read more

David & Assoc. is extremely lacking in Professionalism. Don't go in there thinking that you can make any recommendations on how you'd like your treatment to go. They become intimidated to meet someone with any knowledge about whats going to happen in THEIR mouth. Attitudes were kinda ahhhh. Horrible with returning phone calls, you'll wait for a week minimum. I was put on hold several times without even a greeting. Just a ring and a click. After quoting me a price and making a couple recommendations on what I could try, I was scheduled, only to go to my appt. to be told that, "they discussed your case this morning and we don't think that we'll be able to satisfy you. We won't be working on your case." I was heated. You called me up here for THIS, when I had a standing appt. with another dentist directly after this appt. that I was unable to keep D2 David & Assoc. basically canceling my appt. She then asked me to leave out of the side door. Probably trying to reduce the possibility of a scene. I specifically told her that I was going to leave a horrible rating on Yelp. It seems that there are already a few. I have no idea where I read those good reviews about them. But it wasn't here! read more

I went here for years, getting routine x-rays whenever they were needed. One time when I got an x-ray the nurse said she saw a gray area for one tooth but said it was probably nothing and moved on. Dentist said nothing about it so I assumed I was ok. Come a few months later, that same tooth was evaluated and needed to have a root canal done ASAP. Yet they had seen something on the X-rays before and could have prevented it. This dentistry treats people like they're bags of money. My father went to get an estimate for implants and the lady acted very coldly, the attitude of take it or leave it. I have had problems with Dr David and Associates since the day of surgery. I have expressed all of my concerns to my patient coordinator and she handled everything. She obviously couldn't change what happened but definitely gave the sympathy that was needed... I have had several problems with lack of communication within the staff and a very rude office manager Crystal. If you don't see things her way you are argumentative. I asked to speak to Dr David to express my experience as I have spent a lot of money here that comes with a 5 year warranty. I have already reduced the amount of interaction I have with the staff to make sure I don't get incorrect information. After waiting 30 minutes I asked if there was an update to be told I tried to tell you he will not see you today. I said you were going to tell him that I wanted to speak with him. Update on D5 implants.It's now going on 18 months since my D5 surgery and things are not any better.Since my last review I had my lower D5 break several times.18 months in I would not recommend D5 with David and associates.It is definitely a flawed product that very expensive for a average guy.I spent the money because I was led to believe that my dental problems would be over.Far from the truth.The only thing good to say is that they look GREAT!The other thing disappointing is that when you have problems they treat you like the problem.Management will not take a call so you have to leave a message then sit for hours waiting on call that you may not get. read more

They claim in their advertisements that they have the latest equipment and technology. I spent $27,000 there and after finishing with them I went to an attorney to sue them. I was advised it was really hard to find an attorney that would sue a dentist. I assume it was not profitable enough. As for the latest technology and equipment: I was getting a root canal when the dentist broke off a titanium bit in my tooth. I was then sent to another dentist in a totally different practice to have the drill removed, for which the dentist billed David and Associates. I tried to see Dr. David about my process, which included multiple caps, and was advised that I could not see him by the office manager. After my dental work was complete, I was giving a seminar in another city 2 weeks later when one of the front caps just fell off while I was talking. The worst experience of my life. While I'm sitting in chair waiting to get a tooth pulled, person from office comes back asking for my credit card for payment. Injection for anesthetic was the most painful I've ever had. I was about to come out of the chair and was literally in tears. I've never, ever, had it hurt like that. It was going to be after hours when numbing wore off, so I asked for a prescription for pain med, in case the after pain was as bad. Dentist said Tylenol would do.. He was wrong. The pain was unbearable and no way to get anything. Office was closed.. I had planned to get extensive work done, but I will not be returning here. I wouldn't recommend them for anything.. If I could leave no stars I would. I went in because a cap that they put in came off. The appointment then turned into X-rays, exam, deep cleaning and a determination I needed an implant. I met with a Periodontist a week later who said I did not need a deep cleaning. I was not told until I showed up to get the implant that I was going to get cadaver parts. During that appointment, in the same office, another Dentist told me I didn't really need the procedure at all. After receiving this information I decided to decline anymore services. I was paying in advance for these dental procedures and they were holding thousands of dollars of my money. I missed one appointment before the implant fiasco and they charged me $275. I have requested a refund and they have been weeks without issuing it. I keep calling for an update and they keep delaying. Today I was told my refund would be $400+ less with no explanation of the reason. This place is not what it used to be. They just seem to want your money. read more

Absolutley phenomenal! I had both arches done 3 weeks ago.I am a healthcare professional with 36 years of exp. working closely with doctors and nurses. Suffice it to say I am well aquainted with true quality care and sadly, also many who should not be in this line of business at all. Dr. Joel David dds IS the REAL DEAL. I researched several other well known "cosmetic and restorative" dental providers and was surprised to find most had alot in common. They were only interested my dental health as it related to the elite prices I was quoted and my ability to pay. I was priced out of the market. When I found D5Teeth, I WRONGLY assumed that affordable meant cheap. I stand CORRECTED. Dr. David at D5 Teeth has been the best experience I could have ever imagined. My case was a 9 of 10 in difficulty and a true pro runs toward a challange, not away from it. Dr.David took my case on personally. His expert hands were directly involved to insure perfection. The results are STUNNING !! In one week I have a healthy, realistic, custom smile that I have not had for over 25 years. All at a price that was possible due to the utilization of cutting edge tech allowing the fabrication process to be done at a fraction of conventional techniques. All done "in house". Horrible after care, concern for their patients & communication. Had my teeth removed & implants put in a year ago. 1st, the sutures were suppose to be dissolvable sutures but were not. Most ,but not all, were removed on my 1st f/u appt. Was supposed to have someone contact me for the next appointment. DID NOT HAPPEN. Was not contacted again until my 1st year maintenance checkup. (The sutures that were not removed have been covered over by my skin) tried to explain it to the person who contacted me & was basically called a liar and i was just due for the 1st yearly maintenance. Ok, made the appointment, over a month in advance, drive 2 & 1/2 hours to get to the office to find out my appointment was cancelled by them for video taping. NO COMMUNICATION FROM THE OFFICE PRIOR. Rescheduled, another month in advance, apparently only do implant maintenance on Thursday &Friday??? Again drove 2 & 1/2 hours to find again, I'm cancelled for in-service. With the current price of gas this is bull. NO COMMUNICATION FROM THEM PRIOR AGAIN. They know people are stuck cause by giving a discount, if anyone else work on the teeth the warranty in void. Man came in the waiting room while i was there. Said hid implant teeth were so jagged they were cutting the inside of his mouth. I too have a jagged back tooth. Have had a lot of mouth discomfort since this procedure. Also told them my mouth was hurting. To no concern from the staff. Again, aftercare has mich to be desired. It is horrible. read more

I had the worse experience I have ever had, Dr. Noorthock was very rude, cruel, and downright mean. It started off bad the minute Dr. Noorthock walked in the darkroom, yes darkroom, why a darkroom?? The worse part was, Dr. Noothock didn't numb my gums before he stuck the needle in my gums twice. The minute he touched my gums with the needle I told him it hard, he didn't stop. It was SO PAINFUL I was shaking so bad my feet were jumping up and down at the same time I was yelling " that hurts, that hurts" Dr. Noorthock DIDN"T CARE I WAS IN SO MUCH PAIN. He then pulled my tooth out, in less than 5 seconds, he got up out of his chair and told me, " You can go now " I will get Answers. read more

I have always been afraid of going to the dentist. When I chipped my tooth I went to David & Associates to get it fixed. Everyone who I met was very kind and so I decided that I would go to them to get the rest of my dental work done. Each time I went my service got worse and worse. I needed two crowns put in and two cavities filled. I got one filled and they made impressions for my crowns. I got two temporary ones. At this point in time I was no longer living in Jacksonville and was anticipating only having to come in to get my crowns put in. However, that was not the case. The first set of crowns did not fit and they had to make all new impressions. So I had to wait longer. I went back again and one fit but yet again another didn't. I was told that my cavity would be filled before I got my permanent crown sat. They didn't do that either. I went in, they sat my crown, and then again told me that the new one didn't fit. I was asked if I wanted novocaine... Naturally I said no because I didn't understand what they were about to do. (Mind you, previous times I got impressions done for a new crown I was numb). The assistant began shoving a cord into my tooth down between my gums. She was struggling to get it in and at this point my gums were beginning to bleed a lot. Finally after wincing in pain and pulling away they gave me novocaine. Dr. David came in, gave me the novocaine, and then immediately started drilling down my gums. He did not wait for the numbing to start and again I was wincing in pain. Dr. David and the assistant then started holding me down and I had four hands in my mouth, pushing my tongue down and holding the suction down my throat. I was gagging and wincing but they continued on. I sat in the chair crying with four hands in my mouth, signaling to them that I was in pain and nothing was done. After that appointment, I needed to come back to get my new crown sat. Again, I am not living in Jacksonville and they are all very aware of that. I drove up from West Palm Beach to get the new crown sat. I called ahead of time to tell them that I did not want them filling my cavity. I just wanted the crown sat so this whole nightmare would be over. 15 minutes before my appointment, when I was already ony way there, I got a phone call telling me that they didn't actually have my crown ready. I had driven 4 hours to get back to Jacksonville for this appointment. I was extremely frustrated and told the girl on the phone that I was not coming back and they could forget about rescheduling my appointment. About 30 minutes after that I got another phone call telling me that they had made a mistake and they actually did have my crown. They had "mixed me up" with another Marisa that had an appointment that day. But I don't believe that for a second. Either they were lying in the first place because they didn't want to put in my crown after the last time and then got mad when I would not give them my business, or they are just that disorganized that they couldn't tell the difference between patients. As if they only looked at first names before calling a patient and canceling appointments. If I could give this place 0 stars I would. I asked for a refund because I had already paid them for something that I never had done and have yet to hear back. I am just out of college and don't have thousands of dollars to waste like that. All I received was an "apology" letter from Dr. David. It was honestly less than enough. I am still expecting a refund and if I don't get it I have already been contacting my lawyer and we will sue for malpractice amongst other things. DO NOT GO HERE. DO NOT. They are disorganized and incompetent. I have been to a lot of dentists and have never been treated so horribly. Don't let them swindle you out of your money. I'm sure there is more but I can't even remember all that happened. So utterly disappointed. read more

I consider David & Associates to be to dentistry what ambulance chasers are to the legal profession or McDojos are to Martial Arts.I went to these guys because a veneer popped off of my front tooth and I couldn't get an appointment with my dentist in time for a party I was attending. I arrived and was told that I would have to have X rays. I told them that I did not need them, and that I had some done recently (I don't like to be exposed to more radiation than is absolutely necessary for obvious health reasons) . They informed me that they will not see a new patient without X rays being taken in the office. I conceded, had a bunch of unnecessary X rays taken of my mouth and was taken back to an exam room. The doctor saw me promptly, looked at my tooth and said that he would not replace the veneer, but could place a crown on it (a much more invasive procedure that would require grinding what remained of my tooth into a stump and gluing a false tooth to it). Placing a crown also requires two visits, as opposed to one for the veneer. I told him my situation, and that my dentist would place a veneer on the tooth in one visit, and he said that there was nothing that he could do for me unless I wanted to crown it.I declined and went to the front desk to pay (cash, no insurance).I was billed over $90 for some X rays that I didn't need and a consultation with zero work done. I eventually went to my dentist, who gladly placed a veneer on the tooth. These guys are all about a dime and not receptive to their patients' wishes or concerns. I should have known that they are only concerned with money by the amount of advertisements that they produce. I guess that they would be OK to use for a simple cleaning if you happened to get a discount coupon in the mail, but steer clear if you have any concerns or recommendations on how you want them to provide the service that you are paying them for. I wouldn't return to them for anything. The positive reviews here look suspicious also. One has two reviews, both dental offices, both positive and in two different states, and the other is a one time reviewer. This is a bad sign. read more

I went to the dentist to replace old veneers. They removed the old and put on temporary veneers until the lab completed the new veneers. I went into the office, they removed my temporary veneers, and then asked me to sign a waiver that I agreed that the new veneers were ok otherwise they would. Not proceed. My old teeth were exposed and they coerced me to sign it under this state of duress. They could have had me look at the new veneers with the temporaries still intact but I believe the way they made me sign it was not right and I am wondering if everyone else had the same experience in signing their agreements under duress???? read more

They were able to see me quickly, which I appreciated, but when I was seen, I had a hard time getting the dentist to listen to me. He kept telling me to try things I had already tried, which he would have known had he been a better listener. My daughter had been the highest bidder on a David and Associates voucher for a fundraiser that she gave me for Mother's Day. She knew I needed some dental work and couldn't afford it. My husband and I had both suffered from some major health problems over the past few years that I am just recovering from and they resulted in high medical bills. On top of it all, we lost a job with health insurance. When my daughter went in to pick up the voucher from David and Associates, she asked how long I had to use it and was told the voucher had no date on it purposefully and would be honored. Today, I went in to schedule an appointment with my voucher and they wouldn't honor it. They said it expired even though there is no date on it and I explained the circumstances. My son had been a patient of Dr. David's at one time, so I had experience there and was extremely disappointed. I know there are a lot of worse problems in the world, but I didn't expect this from David and Associates. read more

I wish I had never stepped foot inside this business. My wife was to the point of tears every time she had to go in, because there was always some kind of issue with getting her denture. We went through this process for several years, and their latest answer was "let's just switch you to the Teeth In A Day treatment, and you'll only have to give us another $11k, after the $6k credit from the work that was never completed. And when I asked for a refund of the $6k, they said it wasn't a "true dollar amount" and could not refund the money that I paid, for work that was never completed. I will never again go to them for anything, nor will I recommend them to any family or friends. read more

This place is a sham. Husband had 12 teeth pulled and screws placed in his bone for transplants with no IV sedation or nitrous oxide sedation. I pill for anxiety relaxation. Who does that? They farm them in and farm them out. No follow up phone call the next day to see how he was doing. My dog was treated better at the vet for having one tooth pulled. I've never taken the time to look at yelp for a lot of things......Will never make an important decision without checking going forward. In January of 2019, I had all of my teeth pulled, and had implants and full dentures inserted at the cost of $33,000. David and Associates contracted to do the work and maintain the dentures providing that any warranty would be void if anyone else did any work to the teeth. After locking me into a position that requires that only they can provide dental services, they have routinely failed to keep all but one of my appointments, calling days before the appointment date to cancel and reschedule. I schedule my life activities around my appointments. To consistently fail to keep an appointment and constantly having to reschedule indicates poor management and places undue hardship on me. It seems that they've gotten the big money, they don't care about me as a customer, but they don't want me to go to someone who will provide quality care. I have asked that they refer me to someone who can maintain my teeth but they refuse to do so, yet they can't do it when they commit to. read more

Honestly, I don't like writing reviews for places unless Ive had a serious issue. After having braces for 2 years previously and the process taking longer than expected, I thought I was done with this place. I needed a new retainer and called to get a quote, and was given the wrong price which I wasn't aware of until I came in to get a new one. $400 and 2 retainer molds later, it still isn't correct. Instead of fixing the second mold the ortho tech looked at me like I was crazy and said the retainer fit correct even though it didn't. Basically 1/10 recommend unless you're a 1 and come type of person. read more

They pulled her upper teeth and put in metal things - they cannot get the teeth right - she has had so many appointments and they always put in teeth that are too big for her mouth. Dr. David won't even come in to see her anymore. The office manager is nice, but what can she do. This is a scam. I do not recommend them for anything. THIS IS ONE OF THE MOST INCOMPETENT MEDICAL OFFICES THAT WE HAVE EVER BEEN TO. We had a consultation about my daughter's braces. They told us that one of her baby teeth would need to be removed before we start treatment. So, we scheduled to have that tooth removed. When we arrived at that appointment, we learned that they were planning to pull three teeth, and would have done so if we had not overhead them talking about it. After we sorted that out they went ahead and pulled the tooth. When we came for the next appointment, they informed us that they would not be able to put braces on, because the tooth that was pulled in their office, was not supposed to be removed. They then suggested that we replace that tooth with a $5,000 implant! read more
