Very unhappy at the unprofessionalism between the different dentists and their associates. I'm told different things by different people and then when I'm trying to get their story straight I am charged to talk to the dentist for five minutes to find out what's going on in my case. I just received a letter after not being back after almost 5 months insinuating that I didn't have the money to pay for my treatment, needless to say I left a discouraging message saying never to contact me again because I'm taking my business elsewhere. Do not come here unless you want to be seen numerous times thinking certain charges are included but end up get charged for it after its done, like to be told different things, and then when you're trying to get everything straightened out be charged again for it. If I could give them no stars, I would do that. My 85 year old mother went to them and it has been a nightmare. They not only are too big for her face, but even more important it hurts to eat with them, They even come out, which I did not think implants were supposed to do!! She has called to complain and they do not return her phone calls. When she has finally got in there to have them adjust them, it is never fixed. This business refused to honor their warranty on a dental implant that costs thousands of dollars. My general dentist recommended a dental implant so I went to David and Associates for an implant. It fell apart within the warranty time frame and they found reasons to not replace the crown. The first reason was that I did not get my regular dental care with them. I refused to leave my dentist of 15 years just because they did the implant. They wanted me to sign paperwork to this effect. when I refused to the dentist said they would still honor the warranty. When I brought this up with the business office they said they could not honor the warranty because someone had drilled on the tooth. This is ridiculous. My dentist would have zero reason to drill on a fake tooth. They seem very competent and friendly when offering service. I do not blame the actual dental staff. Lady I brought up to Jacksonville has had problems with her teeth for at least 7 years she paid over $35,000 for her teeth and they were never right she went back 8 times to get them corrected and after all these visits they told her on what was suppose to be the last visit that they would call her in two weeks for her to come up and pick them up it took them 2 months to call her and all they said was it was appointment problems as to why they didn't call her so I'm sure that their 24 hour policy just doesn't work and before that they also lost her teeth and had to start all over again with the impressions to make a new set from the start when she went in to get fitted they also put someone elses teeth into her mouth to try them I would nevergo to them for any reason read more

My husband and I were so happy at first for this dentist. As my husband travels all over the US working and is rarely home the D5 they offer would be so nice. After meeting with the Dentist directly it looked like finally he would be able to get his teeth fixed. We were told by the Dentist that insurance would be worked out with his office staff after the meeting and how they have helped people by even waving costs. Well I can say that was all lip service for sure. Dr. David and associates is a joke. To give someone hope and then nothing. The staff doesnt help with insurance. At the end of our visit we Were told to contact our own insurance company and they would recommend someone for us to try and get a loan with. Yep that's right they keep it all inside. So after months of trying to reach someone in the office and never being able to I finally reached someone who in a not nice way stated that they don't accept insurance for this at all. Her name was Lisa and when I informed her of what the Dentist said about insurance her answer was well I don't know why they would say that. She let me know they will not take out insurance if we wanted we could pay for it and try to get our insurance to reimburse us but they won't have anything to do with insurance. So much for all the commercials this company puts out about helping people. Thats not the case. Just a little about us. I work for Amazon and carry great insurance. My husband who needed this procedure becuase he is always gone and can't make appts is a sandblasts and painter. He works on out US Naval ships making sure they stay seaworthy for the service members. I am proud of him. In no way should this dentist have given us hope that he would help when its clearly the policy to not do that. read more

I would have to agree with the implant issues with this dentist. I had implants put in 2018 and it took until 2020 to get them correct and not breaking. They definitely use folks insurance and money very casually as if it is their disposable income, and not producing an effective good product. In the end you can't be nice when they mess up, and call your insurance company and make a formal complaint. The insurance company takes these complaints serious, and will make them reimburse your money for faulty work. Then make a complaint to the Florida Dentistry Board, there is a lot folks don't know about when going to a dentist. Look at the Florida Statute which governs dentist as well, there are things that the Dentist should do and not his/her dental assistant, then they charge your insurance as if the Dentist completed the work. Some items that the dental assistant do, by law they should not being doing, but folks don't know. In the end it is all about money and these dentist, doctors, any service industry don't give a damn about you nor your health. Your teeth are very important and at the end of the day is detrimental to your overall health. I encourage folks to be vigilant and take control of you, don't put your trust in folks just because they maybe the professional, you have been with you all of your life and know you. Utilize what is available to you to fight against what is costly bad service, and/or product. I do think this dental offices uses the cheapest materials out there and charge as if they are using top quality, and it maybe also that their in-house lab is not trained enough or educated in the products they are offering. This dental office keeps all the money in-house, this makes for bad business, as they are very deficient on the end product and that is clear on the reviews regarding the implants. I would not suggest anyone to get implants there, only general dentistry work. read more

We moved here in the military and my teenage daughter was referred off NAS Jax to get dental work for an implant. We took her to David and Assoc. and they measured everything and had a couple of appointments before the implant procedure. On the day of the implant appointment, they sent my wife to get medication from a local pharmacy so my daughter would have them for after the surgery at home. When she came back, they said they could not do the implant and that there was not enough space to do the procedure. How do you not know you could not do the procedure after going there twice and they x-rayed and measured everything? KEEP IN MIND THEY PUT MY DAUGHTER UNDER USING ANESTHESIA EVEN THOUGH THERE WAS NOT A PROCEDURE. I was afraid they did something to her while she was asleep. When I heard that, I went directly there to confront all involved. They all ran like damn wimps. They failed to answer any questions. We saw Dr. David hiding behind a door. WE NEVER CAME BACK. DO NOT GO TO THIS OFFICE FOR ANYTHING. They are incompetent do not trust them. read more

Not a good experience. I got orthodontic treatment to straighten my teeth and correct my overbite. During the initial consult it was explained that both the conditions were correctable by braces. About 18 months into my treatment my overbite was not corrected at all. When I asked about my overbite I was told that it would require teeth to be pulled and the orthodontist did not like to pull teeth from adults. I spent all that time in braces and still have an overbite. I would not recommend this business to anyone. read more

I have been a patient at David & Associates for three years. About 8 months ago I was told I needed a dental implant. It could be done there for four thousand dollars. I checked around and found a very competent oral surgeon to do it for less. However, I would have to go back to David & Associates for the crown for the implant. It took two tries and several molds to get a crown. After the crown was inserted, I had to sign a paper that said I agreed to accept the crown. When my oral surgeon saw the crown, he was not happy. The new crown had thrown off my bite. I went back to David & Associates to have my teeth cleaned. The dental assistant did a great job cleaning my teeth. Her name is Marlene. She made a lot of x-rays. Found a cavity in one of my bottom front teeth. Also, a filling had fallen out of one of my bottom front teeth. An appointment was made to see my dentist there. Bad, bad news. I need a root canal in one of my bottom front teeth. I need two more implants. I need three more crowns. I need. I need. I need. Where has this dentist been for the last three years? Why have I not had any x-rays? Why has he not seen that my teeth needed attention? My mouth has been examined by him many times. HIs comment to me was, "I hate giving bad news to nice people". Really!. Then, why did he tell me that one of the financial people would get with me and walk out of the room. Finally, I asked one of his assistants, Margaret, if I could leave. She went to him, came back, and said I could leave. I went to see the treatment coordinator. When I was shown the proposal, I thought I would lose it. The total came to approximately $8,523 and that was not the actual amount because I would need to go back to my oral surgeon to do the implant surgery on two teeth. Now you can understand why I am so upset over the fact that I was not given proper dental care for the last three years. I am 73 years old. I'm on social security and have a small part time job. Now I have to go into considerable debt. All of this was not necessary if I had been treated correctly by this dental office. If you want to go to a dental office that is very disorganized and gives you poor dental care, this is the place to go. Dr. David you need to have meetings with your dentists that would include addressing good protocol, proper dental care, good manners, and organization to see that the patient's needs are met. Seemed like a very. unusual office. Didn't get a good vibe. This was years ago so maybe things have changed but never went back to find out.at the time, they advertised specials on teeth whitening but when I showed up for appointment,receptionist said the offer was no longer available even though the ad was in that mornings paper with no expiration date. The regular price was higher than other offices so just had it done elsewhere. Maybe things have improved now but sure was a waste of time and money from taking time off work to go. read more

I am horrified at the commercial I heard this morning considering it was explaining how my husband was treated here. He had major surgery that we paid $40,000 cash because we heard they were "the best". During the begining of his 9 hour horrific surgery, he woke up. They only shot him with novicane after while they removed teeth, shaved his bones and implanted rods. I was sickened when they came out with him and he had tears rolling down his face. To add insult to serious injury, he didn't get his permanents for roughly 6 months. I would never recommend this place to anyone. I am disgusted that we paid an astronomical amount of money for something that was malpractice at best. read more

I literally made a yelp account just so I can review this place and hopefully prevent someone else from going through what I did. I have never had such a horrible medical experience in my life. I had a routine dental cleaning (I go to the dentist once a year) here two weeks ago, I now have gum infection and swollen lymph nodes as a result. At first my technician seemed very nice but as soon as she got into my mouth it's like she turned into a different person. She started being extremely judgmental and condescending about the way my mouth looked, I will agree I don't floss enough and had a good amount of plaque built up but by no means was my mouth gross or "that bad". I noticed immediately she was cleaning my mouth very roughly, unlike any other cleaning I have had, there was instant pain. I could feel her digging into my gums, it is my understanding usually when they have to go in your gums they offer some sort of numbing or something to ease pain. I was in visible pain wincing and groaning, she never once asked if i needed a break or if I was ok. She made me keep a mirror on my lap and periodically made me hold it up to my mouth as she condescendingly told me how horribly I took care of my teeth and poked her tools into my gums. She basically made me watch myself bleed. My mouth was full of blood throughout the cleaning and most of it I had to swallow myself because she didn't use the suction enough, sometimes the suction tube wasn't even in my mouth. The final time she made me hold up the mirror she made me watch as she stuck one of her tools up under the gum of one of my front teeth and it started bleeding profusely, I started crying because at this point my mouth was full of blood and the pain was excruciating. She asked me why I was crying I told her I really don't like to watch myself bleed and it hurt really bad, to which she replied something along the lines of "it's gotta get bad before it gets good". No apology no asking if I needed a break. She told me I had really bad gingivitis and was on the verge of gum disease and my teeth falling out. I have since seen another doctor who doesn't think I have gingivitis at all! She used scare tactics the entire time to shame me I suppose into taking better care of my teeth. I left crying and in pain, mouth bleeding. I couldn't eat solid foods for the rest of the day. I was never told to use any sort of antibacterial rinse to prevent infection from the amount of bleeding, my entire mouth was practically an open wound at this point. Now I have an infection in my gums, I've never had any pain in my mouth ever in my life. My mother called to complain on my behalf as I was in hysterics and considerable pain when I got home. Whoever answered the call immediately started defending the tech, trivializing what I went through and eventually hung up on us. We called back and asked to speak to Joel David himself and he never got back to us. This place needs to be shut down. read more

There are some good things about this practice, however, the rating is primarily related to customer service. If you need to call for any reason, expect to be on hold for at least 20 minutes. At times, a person will pick up the phone and just hang it up. While this might be a mistake, it's happened to me so many times now, I'm beginning to wonder. I've received reasonable care from this practice, however, the techs there have pressured me to accept treatments and equipment that I just wasn't interested in. They are tolerant of those with dental anxieties but don't have any processes that support anxiety reduction. read more

I had the D 5 procedure over six months ago . The bottom bridge was placed so far the back in my mouth it caused a very severe overbite .The office was having me come in over many visits to get this corrected .The temp . bridge they put in had broken completely in half . This was the second time this happened leaving a half mouth of teeth and a sharp edge which had cut my mouth in several places . I called numerous time and left messages a couple of days passed and they called me . I explained what happened " Again " and they advised me I would have to wait till next Tuesday because they were only open one day a week now because of the virus . On Tueday April 7 2020 I drove two Hours to get to an appt. at the Mandarin Office . When I arrived I met with a dental tech "Pauline" and a "dentist Gayta , the spelling on the dentist may not be correct .? I oploligize for that !!. They were both obviously angry to be there , and they told me the only thing they would do is try to grind down the sharp edges on my half bottom bridge which I spent 35000 .00 dollars to have put in .!! I asked them to please take out the half bridge and put in the original D 5 bottom bridge they had . The stated , "We Cant Find it " I stated, no problem I will wait . I knew they could have found it if they wanted , And I know Dr David would have found it !!!! Long story short I left feeling angry and cheated ** If Dentist Gayta and Dental tech Pauline did Not want to be there that day they should have stayed home instead of taking their anger and out on the patients . THIS IS THE MOST INCOMPETENT DENTAL OFFICE THAT I HAVE EVER BEEN TO. If you are looking for a new dentist I highly recommend you choose any other dental office other than David and Associates. I went in to have a crown replaced. The office manager gave a quote of $262.00 to replace the crown. She however never bothered to call the insurance company to confirm this quote. On my second visit the assistant was so terrible I was given a $50.00 gift card for a choice of restaurants due to what I went through. That should tell you something about her abilities. When the new crown was put on the dentist said to bite down on it and when I did it broke in half. They ordered a new crown and I finally got a new crown along with a bill for $774.00. So the office managers initial quote went from $262.00 to $774.00 due to her not bothering to call the insurance. This was the worse dental experience I have had in my 64 years. read more

I am retired military and I visited this Dentist for their D5 ultra procedure and even meet with Dr. David about everything. I was completely interested until they told me they don't bill the insurance company for that procedure, but they could get me all the credit i needed to pay for it which was very upsetting. I explained that I wanted the insurance billed and I would pay what was remaining and they said NO they would not do that. I have since changed dental insurance companies to MetLife, but before doing so I went and got a copy of the billing codes that would be used for the D5 procedure and matched they with all the different companies to see which one would cover all the billing codes, and of course MetLife did. I have since within the last two months Nov and Dec 2019 and asked if the would give me a pretreatment estimate so I could send it to the insurance company (at their request) to get an idea of how much my out of pocket expense would be and have been to NO they want do that I have to pay for the procedure then file with the insurance. This does not make any since why they will not give me an pretreatment estimate unless their are cost they are not telling people. I feel they should be investigated and find out why they want take insurance for this procedure. read more

I have had several bad experiences with David & Associates, all of which have been with the front office staff and insurance/billing department. The entire time my son was in braces the first time around I spent more time on the phone correcting billing charges on his account for things that were included in the treatment, constant scheduling screw ups- I can't even count the amount of times I would take him in for a long appointment for the cleaning & adjustment only to be told he was never scheduled for the cleaning, I had to track down his spring loaded retainer that was sitting in one of the ladies drawers- after 3 month she still had apparently not found the time to call and let me know it was in....I could go on and on. They ALWAYS over book so you can anticipate a long wait and the ladies in the front office are not only rude & curt but none of them are the brightest crayons in the box. My entire family loves the dentists and their staff, its sad that something so easily fixed is forcing his patience to seek dental care else where. My son is now on his second round of bottom braces & at his request we went back because he is so comfortable with the Orthodontist & staff. After much convincing by one of the hygienist, I too decided to give them a try again but that was a huge mistake. I went in for my "new" client appointment (since I changed my dentist over a year before!), needless to say I didn't make it past the front desk! After being in the waiting room for almost 2 HOURS and listening to how care free and funny the ladies at the front seemed to think it was I not only choose to take my business else where but I also pulled my son's records & have moved him to a much more professional dental practice for his remaining ortho treatment! Dr. David- if you really took pride in your patients & their experience in your office, you would pay more attention to how your office staff treats them! My suggestion- wipe out your office manager & surrounding staff and start all over!!!! read more

BEWARE!!! I spent two entire years (4/2015-3/2017) trying to complete a procedure I was told would take 7 months. I am very unhappy with the final product. The implant denture made for me and forced on me to accept, is too big & unuseable. I can not eat anything without it popping loose.No food grinding ability due to bad match. I was told I would not need any adhesive with this product and I would be able to eat anything. That was a bold faced lie. This denture they call permanent, has to be removed every evening and has to be adjusted every month. This procedure cost me 5 figures and I am stuck using the temporary denture more than the ones called permanent. Worst of all, after the surgery I kept getting infections at the implant & I was repeatedly told by the surgeon (DR. Noorthrop) that it had nothing to do with his work & he had no idea why it was swelling all the way up to my eye. I dealt with this infection for months until my fitting date arrived and fortunately it was swollen, so the fitting Dr.( Dr. Mike) finally did x-rays & decided the surgery had to be done again. The surgeon then bitched about the order & tried to make me believe it was Dr. Mikes fault the swelling was happening. Again weeks of healing. You never see the same people twice so everytime you go in you have to explain your entire procedure to new people. I finally was able to be fitted with my final denture-I rejected the first one because the teeth were to small in the front & I asked that just the four in front be a little larger, the back teeth matched and fit well. Dr. Mike sent me away for two week (always the time between visits as the Dr. does not live in town and only comes in every 2 weeks)and the new set which I was stuck with didn't look right or allow me to eat, he had all the teeth changed to large teeth that do not fit. when I asked for an explanation I was told they only had 2 size teeth (male & female) and they can't be used together. This of course, was not explained beforehand. Now I am stuck with teeth I can not use. During this 2 year period my health has deteriorated due to stress, anxiety & inability to eat correctly. Take heed people, David & Associates are in it for the money only-NOT to improve your health. I finally gave up and asked for refund on price of denture so I could go elsewhere & when their own technician said the bite was off & denture unusable, they gave me a an extremely low 4 figure refund that will not cover anything & they kept the credit I had on file. Heed the warning-He advertises excessively because he doesn't have return customers. I wish I read this before I ever stepped in the office. BEWARE!!! read more

Horrible Experience! It is hard to believe that David & Associates has a 5 star rating....but it is their website. I arrived at David & Associates at 2:45 for my 3:00 first time consultation. The office is small and confined, located in a strip mall storefront. It is dingy with old and dirty carpeting. The decor resembles a nick knack thrift store. Certainly did not look like the office depicted it the TV commercials. I was ushered to the dental chair where I sat for about 25 minutes waiting for the dentist. He came in, said his name so fast that I did not understand it. He looked at the x-rays, looked in my mouth and mumbled that "something was going on" with 2 other teeth besides the one I came in for, then left. This evaluation took less than one minute. After another 25 minute wait I was taken to a "treatment specialist" who looked like a high school homecoming queen dressed like she was going to a night club. This "treatment specialist" produced a proposed bill totaling over $17000 for the implant procedure. This is the price they came up with after a ONE minute evaluation. Nothing about the implant procedure was explained. The only thing that was marginally explained was financing and insurance coverage. I doubt if the treatment specialist knew anything about dental implants. It was clear that David & Associates foster this patient assembly line concept to maximize revenue and consequently treat patients like cash cows. read more

Administrative support doesn't know what they are doing...from the front desk all the way to the CFO...constant battle to get financials and records straightened out. Dr David doesn't seem to know what he's doing either...I had a post put in for an implant and he could figure out how to get the cap screw removed (his commercials make it look like he's done thousands of these). If you want shoddy unfinished dental work...then this is definitely your place. I received a root canal which was fine but when the permanent tooth got stuck, instead of them removing it by cutting it out and just remaking the tooth, I was sent home to see it tooth would loosen on its own after they tugged and pulled on it several times. Finally my third appointment they finally decided to just cut the tooth out only to find the root was now broken. They also wanted me to believe they it was broken before the root canal and not the constant tugging and pulling that happened during the 3 follow up appointments to attempt to removed the tooth. I'm certain after all the X-rays taken before and during procedure a broken root would have been noticed. After that happened I was ushered into a finance office to tell me how much it would cost for an implant. Now I'm left with a loose tooth that's not cemented. So you don't even finish the initial job, caused damage, and want me to pay more money to fix your botched work GTFOH! read more

To the point. Dr Forrest is the most arrogant person that I have ever had the displeasure of meeting. He would be a fine used car sellsman but as soon as the sell is done and you have a problem with the work he becomes self righteous and refuses to consider that maybe he missed something. He was greatly offended that I had gotten a second and third opinion about the work he had done.Tooth extracting and deep cleaning. So offended that he brought me a mirror and asked me to show him the tartar he missed on the cleaning he preformed. He then left never to be seen again by me. He sent a message by a wonderful hygenist of the treatment plan he would recommend but didn't have another moment of time to explain this his self. This is what you can expect from David And Associates. If all goes well good for you but if it doesn't dont look for sympathetic ears there. Just go find another dentist. Very disappointed in the customer service. I was advised by Phyllis when she scheduled an appointment that one extraction would cost 85.00. I have insurance and she scheduled the appointment. I arrived and waited over an hour to be called. Once I reached the desk with the amount on my printout by Phyllis, I was advised I needed to pay nearly 500.00. It was awful and the African American lady said I can make a payment arrangement acting ghetto. I then left and went to the Carson Group and became there patient. I was able to get 2 teeth extracted for 87.00. I am not a patient of the Carson Group. Second bad experience was today with the Ortho department. Phyllis advised me to make my payments in the office. I paid over 500.00 and paid for brackets to be replaced and Phyllis had amnesia. I called and left Phyllis a message to explain what happened and she never returned the call. I am finished with Dr. Ouellette. He was rude and will never do anything else in my mouth or my daughter's mouth. Too unprofessional for me. Dr. Joel was nice when I received service from him. The billing and treatment starts off correctly then extra charges and fees are billed outside of what was discussed. read more
