My mother only suffers from forgetfulness; otherwise, she is in good health. My mother has been staying here for over a year; I visit occasionally and bring along some pastries or desserts I've made. The nurses see this but don't say anything--they are very nice. However, lately, whenever I visit, I encounter a Vietnamese woman who works in my mother's room; she isn't a nurse but assists the patients. Nowadays, every time I come here to visit my mother, it feels like visiting a prisoner in a detention center. Ms. L. constantly grills me about what food I've brought and what ingredients are in it; she doesn't just ask upon our first meeting--she scrutinizes every time I was here with every single item I bring for my mother. Last week, I brought my mother a small piece of chicken pie (3x3 inches) and three-quarters of a cup of Jell-O--a very small portion, certainly not a full lunch. As soon as she saw me, she began questioning me relentlessly: "What is that food? Did you put chocolate in the Jell-O?" "What kind of pie is it?" She is neither a nurse nor a nutrition expert. She questioned me as if interrogating a prisoner. That wasn't the end of it; after I finished feeding my mother, she came back and asked, "Does your mother eat a lot or a little?" I replied, "Why do you ask that?" She said, "If your mother eats a lot, I'll have her lunch cut off." I felt like she was flaunting her power over my mother and me; I felt we were being abused. It was just a small 3x3-inch piece of cake and three-quarters of a cup of Jell-O--my mother only ate half of it--yet she threatened to cut off her lunch. Who was she in this rehab facility? How much authority did she have to throw her weight around like that? This is America, not Communist Vietnam. At 12:05 PM, I saw a nurse hand Ms. L. a small carton of milk (I guessed it was a nutritional supplement drink like Ensure)for my Mom. I was even more surprised when I saw that, instead of placing the carton of milk on the table--which was completely clear and uncluttered--she stashed it away in the drawer of my mom bedside cabinet !!! Everyone at We Care Skilled Nursing are very nice and welcoming. The facility is clean and organized. The PT, OT therapists, CNAs, and the nurses are compassionate and attentive with their patients. Finding a skilled nursing facility for a loved one is one of the most emotional and stressful experiences a family can face. After my mother-in-law had a fall, she spent a few days at Washington Hospital, and we needed to find a rehab facility quickly for her recovery. We toured three different places, but the moment Ana Cruz started showing us around Wecare, I felt an immediate sense of comfort. Ana was incredibly knowledgeable, patient, and genuinely caring. She didn't just give us a quick tour -- she took the time to explain everything thoughtfully and introduced us to many staff members along the way. By the end of the visit, I knew this was the right place for my mother-in-law. After she moved in, we had an introduction meeting with three key department heads -- Nursing, Physical/Occupational Therapy, and Social Services. They explained the care plan in detail and helped set clear expectations from the beginning. That meeting was extremely helpful and reassuring. I especially appreciated it because when my own mom stayed at a different skilled nursing facility in the past, we never received that kind of organized communication or support. Throughout my mother-in-law's stay, every CNA, nurse, and staff member treated her with kindness, respect, and warmth. They were always supportive and friendly, and it gave our family so much peace of mind knowing she was surrounded by people who truly cared. Ana also went far beyond her role in admissions. Anytime we had questions or needed help, she never hesitated to reach out to other departments for us. Her dedication and compassion made a difficult time so much easier. Very unprofessional staff - transferred multiple times, I was requesting for medical records for a common patient. The staff puts the call on hold or just hangs up. Called the sister and had her call the facility. I already introduced myself as the new provider after patient was admitted to our smaller nursing home. Even informed We Care HH after hours on day of admission - to ensure continuity of care. Escalated to owner Dr. Jain. It is interesting that people nowadays do not have courtesy or professionalism. read more

Patients are crowded into rooms with very little area for their things. They get a small three drawer nightstand and that's it. One side of the bed is up against the curtain to your neighbor and on the other side you barely have room to get out of your bed. No place to move around. Place is extremely noisy and seems unorganized. And the scary thing is people can just walk from the street right past the desk and go inside no security check at all. I wanted to take a moment to write a review because I know it helps to ease the mind of possible new comers to the Facilitie who may be going through a tough time with medical conditions, Let me start off by saying that my mom is 85 years old and is in stage five of kidney disease, She is at the facility to be able to receive physical therapy to keep her moving as much as possible and from day one Anna the facility manager has been very kind, very helpful and has brought peace to not just my mind but my father as well who is 89 and having to watch my mom go through the things she's going through at this point in her life. Anna is very quick to respond to any questions or concerns we may have and always does it with a smile, and the nursing staff at the facility have been wonderful, super kind, and very present, which is important, the physical therapist that work with my mom are wonderful as well very encouraging and helpful. The cleaning staff and maintenance staff are very on top of things, my mom has been there for six weeks up-to-date not sure how much longer her stay will be but my self, my father and our family have peace of mind, knowing that she is very well taken care of at this facility which at the end of the day is such a huge deal, so a BIG thank you to Anna as well as all the staff members!! read more

We had our Grandma stay at Wecare per the recommendation of the case manager at Washington and Dr. Jain himself. I am new to Yelp and I felt the need to write this review. The staff was not attentive to our grandma at all. We tried several times to talk with the older Filipino lady who runs the changes of rooms but never got a hold of her. There were several people at the time of her stay that were yelling and the staff did not do anything about it. We asked to speak to a social worker and was told there is not one there. Therapy didn't do anything with my Grandma we were lucky to get a couple days a week of some. I know at times she didn't want to do it but they should have kept trying. I heard some of the nurses and staff speaking negatively about management and felt that was unprofessional. I assume WeCare has a home health as well which we were feeling pressured to use but declined. My grandma was there a few months ago and granted maybe things changed. I don't feel the reviews on here are honest and they are made by staff bc it was not a good experience. The food was not good either. There were a couple of nice CNAs but it took a while for them to answer call lights and I was told they were understaffed. All in all we would not put our family back in this place. It is heartbreaking to know she declined more being there and has not had a recovery like we were promised. read more

My mom was just admitted to this skilled facility and the whole process of transferring her there was so easy and smooth. The staff there, specially Ana, their admitting director was so helpful and accommodating. She guided me on what to do before admission and during her stay there. The rehab staff are so good and very patient with all their patients. The facility is clean and safe. I feel comfortable knowing that my mom is safe and well taking care of there. The front office staff are very welcoming and helpful. This is a truly great place for your rehab. My wife went there after a knee replacement surgery and the care was extremely professional and top notch! They have everything you need from excellent physiotherapy, onsite doctors and nurses as well as all equipment one may need for recovery. On top of that, the place is very clean and professionally equipped. The Director of Admissions, Ana Cruz is extremely helpful and very professional. She even helped my wife find a very good PCP (as her previous PCP had been very non-responsive over several years). I have been very pleasantly surprised by this facility. My brother is here. He went in 2 weeks ago unable to even stand due to a sudden health issue. This week we were able to visit physical therapy with him and he is now walking with a walker. Our family likes that they offer patio visits. The only cons I have heard are 1: my brother doesn't like the food. ( granted his taster is a bit off) and 2: you have to make an appointment to go visit. In this day and age though I don't think that is so crazy. Again I am very pleased. read more

Employees must be padding this rating... To piggy back off of my brother's post, Zac, we are about to remove our father from life support. The sepsis, which was contracted due to this facility not properly caring for his surgical wound spread to his brain... this is not a personal opinion, medical professionals at the hospital have confirmed. He is now having seizures. Dad was rushed to the ER from parkside 7 days ago and they had the audacity to bill us for those 7 days claiming they were holding a bed for him. I have since made it very clear they will not be charging us as they played an active role in the speed of progression of death and there were never agreements that he would return to their 'care'. While there, we had to ask and remind someone to feed him or they would let him sleep through meals, he lost nearly 20lbs in 2.5 weeks. His oncologist explained that he would run a low grade fever as a symptom of his medication and he was required to stay in a private room due to being imunocompromised. Parkside disregarded that direction, tested him for covid 3 times WITH ZERO SYMPTOMS, knowing the slight fever was a side effect and transferred him to a shared room where he then contacted pneumonia. We also had to remind them ourselves at least 3 times each. Labor day weekend they tried to refuse him care. The list of misconduct could go on and I will be contacting the health department as well has having already sought legal counsel. Everyone, this happened with us visiting through the window 4 to 5 times a day, countless phone calls on top of the visits. Do not let your loved ones go here. read more

My 91 year old mom was admitted here a couple of months ago from a knee surgery. I admire Anna's reception day 1, that she made us feel welcome. She provided us necessary information about the rehab program for my mom. We are blessed to have all the hard working staff who dedicate their time and efforts helping patients in need of care in this facility. My 81 year old mom was admitted to this facility after her hip surgery. She stayed in this facility for a period of 5 weeks. Her experience with this facility was extremely good. Nursing care, Physical therapists, Admin every one was so nice and took care of her very well. One of the facilities in Fremont that is COVID free and we would strongly recommend this facility for anyone considering rehab needs. Would be good if the facility can arrange Indian food especially for old age Indian patients. read more

The facility is very well maintained and extremely cares about creating a safe and covid-free environment for the patients. All the staff are very caring and helpful. My brother-in-law spent 2 weeks at this facility. The personnel were unfailingly kind and helpful. The intake staff, nurses and CNA's cleaned and shaved my brother-in-law daily, which was a treat for him as he had been on his own for a while and not able to do for himself. There is a cheerful atmosphere and the staff seems to really care about the residents. The facility is clean and the rooms are clean. I would recommend it if you require skilled nursing, short term. And my husband wanted to add that the food looked good too!. read more

If I could give this AWFUL place 0 stars I would... PLEASE PLEASE PLEASE DO NOT SEND YOUR ENEMIES HERE LET ALONE YOUR LOVED ONES!!!! I am shocked that they have any decent reviews. 1. This place is VERY old and dirty!!!! 2. They gave my mom OLD DIRTY socks to use! 3. They did not have my moms medications onsite but they still accepted her and she was in excruciating pain after knee replacement surgery!!! They should have never accepted her into their facilities. 4. They did not have a physical therapist onsite so they expected my mom to lay down in her bed for over 14 hours. Clearly this place has zero experience in giving care to someone who just had knee replacement surgery. She needs to be active and doing her PT at least 3 times a day. Laying in bed for 14 hours is NOT conducive to her recovery. DO NOT SEND ANYONE HERE!!!!!!! read more

This place is located on Parkmont Drive in Fremont. It's basically right across the street from Washington Hospital in Fremont. My grandmother is currently there, and she's not in a good state of mind, basically being pretty rude to their staff although it's not her fault, it's her mind. They have to endure a lot here, and their jobs aren't easy. I certainly wouldn't be able to do that kind of work. So far, when I've gone there they have seemed very friendly, and the physical therapist is a young guy, he's super nice and patient. read more

I am 54, the youngest person in this facility during the 2 weeks I was here. I had knee replacement surgery so I was admitted at my own request for PT OT to get enough strength and range of motion in my knee to be able to go home and safely be independent. The challenge was I have 20 concrete steps just to get up to my apt. Then inside I need to be able to perform activities of daily living safely such as showering, getting on and off the toilet, etc, and not to lose balance and fall. I live alone, which is why I chose to go to a rehab facility after surgery since I have no help at home. Anna, the admitting coordinator, called me at Washington Hospital. I appreciate her kindness and her work to match me with a roommate who is there for rehab only like myself. The majority here are ill with various conditions and not being ill nor near the average age here, I certainly appreciated her efforts.  Their van brought me over. They are literally right across the street from Washington Hospital. I was roomed with a very nice considerate woman who was on the mends from fracturing her pelvis and in for rehab only like myself. We got along great.  The staff here are all very nice and accommodating. John has a great sense of humor,  Flo a CNA is very nice and accommodating. They all root you on as you progress in rehab. I had a great PT, Pooja, and OT, Revati. They knew my goals and provided activities and exercises to help strengthen my muscles in my arms and legs. With that came balance which prevents falls. Since it was a knee replacement surgery and I have these massive stairs at home, I practiced stairs there while carrying full bags of items like I would grocery bags. I also did the exercises provided by the hospital staff on the weekends since the rehab program here is Mon through Fri. Some of the meals wasn't half bad actually. Any facility you go to is not going to have homemade quality or restaurant quality food. They have the menu posted by the activities room and alternative choices if you'd rather have something else instead, but you need to let your CNA know a couple hours before the meal of the change. You can ask for a copy of the menus. Or you can have family or friends bring meals to you. I also brought snacks like tangerines and crackers for in between meals. This facility doesn't have the nursing home smell like most do, and is very clean. They clean everywhere thoroughly daily and provide fresh linens daily. This is a big plus especially for me who wasn't sick and didn't want to get sick. My overall experience here was a great one. The staff are easy to find and listen to you. You need to be your own advocate no matter which facility you go to. Always be kind and respectful to the staff and they're happier to help you. As they say, do to others what you want done to you. Don't expect them to read your mind. They have other patients to tend to as well so you need to kindly ask them if you need something. Don't be demanding. They have a difficult job. I learned to tune out people who are ill here and focus on my recovery and goals, and I surpassed mine with a lot of work and determination on my part as well.. So thank you Park Central for giving me a pleasant experience and supporting me as I progressed very well in my rehab here! read more

My grandmother stayed at this nursing home for the past year. On multiple occasions she was taken to the hospital and only was taken to the hospital when she was at her last stance. This nursing home DOES NOT care about their patients. The only people I truly found helpful were the receptionists. My grandmother was taken to again a few days ago and the nursing home called me and asked me how she was doing and were she was. Like WTF. You should be the ones that contact me, updating me on my grandmother. Cutting to the chase. My grandmother passed away at the hospital, with her family by her side. PLEASE. WHATEVER YOU DO. Our mom came to the point of needing a "skilled Nursing Facility" which many of us might call a Nursing home. In general, these gave us the Heeby Jeebies, and we did not really want to have to put her in one, but it was necessary. We did not love this choice at first, but really, these people were so kind to all of us. We were very happy with her stay, which stretched into 3 weeks. The therapists were all very attentive. Our mom has dementia, so really, she was quite difficult. They tried to find food she would eat, they worked to keep her from falling, and on her final day, the nurse was proud to show me how she got her to take her medicine ( a challenging task) So, we were happy and thankful. Park Central is one of the cleanest facilities I've ever been in. Parking is hard to find, but it's worth walking to. The staff are very kind and some even go the extra mile just to make sure my uncle felt comfortable. This is a wonderful care center. Everyone here are very profession. My father lives here for several years. Nurses & care providers have been helping him with patience and kindness. They are not only understand patients need but also the family need too. They always show welcome attitude to us when we come for visiting. Another thing that I like most is they always sanitize the facility. It makes my father and family feels safe to stay. In fact, I cannot help to say that "team work" is one of the major factor make this a great place. Admin, social worker, physical therapists, speech therapists, nurses, custodians, kitchen staff, and volunteer programs (before COVID), etc. They all work well. read more

My mother recently spent about three months recovering from a major medical crisis after knee replacement. She received excellent care. The physical therapy department is top notch. She had significant mobility challenges and they worked with her diligently to regain the mobility she had prior to surgery. They did not have space available when she first needed skilled nursing and we had her at another facility. We were so relived when we could move her to Park Central. The difference in the level of care was night and day. Their COVID protocols were safe and appropriate, and strictly enforced. It gave me a lot of peace of mind since she also has fragile respiratory health. One night she had a medical emergency and they quickly responded to her needs and had EMS transport her to the hospital right across the street. They kept her exact same room for her during the three days she was gone. The same did not happen a the other place when she had a similar incident. Staff is friendly, professional, and compassionate. As with any large organization, there are a few who seem to enjoy their jobs less than others, but those were not frequent occurrences. The director of the facility makes regular rounds, visiting in depth with his clients. My mother felt people really cared about her and her recovery. If she ever needs skilled nursing again, this is definitely a place I would feel comfortable using and recommending to others. read more

This facility is one of the best in Fremont! They have great customer service and they also try to take care of their patients at the best of their abilities. Park Central has great activities for the patients there and the people who are rehabilitated there can keep themselves busy. It is also very clean and tidy, especially the bathrooms for the visitors to use. Employees at Park Central are polite to patients and clients.
