Not long after that awkward conversation , I received a call from the state and they told me what they found. I was vindicated on a few concerns and the facility will have to address and fix them. This conversation was much smoother and my anxieties were calmed by the investigator. The state DID substantiate my concerns and complaints. I again encourage anyone on the fence about whether or not to speak up, just do it. If no one knows about an issue, how can anything change for the better? I may never truly know if any of the issues were fixed but the once bitter taste in my mouth is slowly going away. **It's a nice facility that has some bigger issues that need to be addressed. Considering the hospitals they are loosely affiliated with I expect Crystal Cove to do better.** The type of care a patient receives will always reflect on the facility regardless of why they are there. Short term care or long term care should be treated the same.

I was grossly disappointed with Care Choices' Hospice service. To start with, their contract and…read moremarketing materials are materially misleading, and internally inconsistent, in terms of the care that they actually provided. Their representative rushed me through the sign-up process without any clear explanation of the services that they were going to provide. Specifically, their communication to the parties involved (Assisted Living, the attending physician, and me) was vague, ambiguous, inconsistent, and confusing at a time when job descriptions should be crystal clear. It is Care Choices' responsibility to coordinate care and especially to make sure that "their" patient is being properly attended to. Hospice services are not just to provide a hospital-style bed and a wheelchair. As a direct result of Care Choices' lack of clarity, the Hospice service was waiting for the attending physician to treat, the attending physician was waiting for Hospice to treat, and the family was waiting for the patient to receive necessary care on a prompt basis. As it was, the patient's care was overly delayed, which caused the patient medical distress and the family members completely unnecessary emotional distress.

We had two signs up saying family will wash his laundry and a laundry basket in his closet. For his clothes to disappear and could never be found. Know the food smelled and looked disgusting. If I didn't bring him food he didn't eat. And they even lost a couple of his meals I bought him. The CNA's even felt comfortable enough to sneak into his snacks. And not to say the would only change his diaper 3 times a day. His clothes would be soaked with urine. Know the rehab part was good. They listen to concerns and tried to figure things out with us. And to say this is one of the top rated nursing centers.

The staff checked in on my relative regularly and expressed care. I also got to check out their GIFT SHOP on site which was pretty cool and very reasonably priced. They have a lot of nice things in there. They sell kimonos, cute mint and pill boxes, hand towels with cute designs, glass and plastic hummingbird decorations, and much more.
