Worst Place; Don't let your family member go here in the Memory Unit in Station 2. Understaffed, poor food, no communication, they hang up on you and don't want to talk. My family member has unexplained injuries and no one is talking. A big COVER UP by this place. I have seen a few memory care units and this is one of the worst in California. I worry about my family member everyday. They don't give enough food and the food they do give is way below what's needed. My family member is persistently Thirsty and Hungry because of the poor food menu and lack of staff. I worry about the welfare and lack of care. This place is worse then prison. Don't let your family member stay at this substandard environment and put their life at risk. This for-profit company is threatening the life of all people who live there. Don't risk your family member to this place. They should be investigated by the authorities. UNBELIEVABLE, This is a typical reply from this FOR-PROFIT place. They are so afraid of being looked into, that even at this late hour when I couldn't get anyone on the phone; And the RN hung up on me when I asked for details related to my family member's welfare.....BEHOLD the administrator magically responds to my post here. This dirty place knows how to play the game and keep collecting on these poor residents and creating HARDSHIP on their family such myself. I have tried to contact everyone that I possibly could for help including Social Services, Food Manager, other Management, CNA's, Nurses and even the cleaning lady who pushes the mop cart around just walks around in circles here while this place remains dirty to patients. Floors, walls, trash overflowing, beds not made, dirty linen and no clean sheet or blankets. My family member's bed is always just a mattress with bedding on it. There is no chairs to sit on in this place. Although, there are a few plastic chairs that will dump the patient out of the chair if they sit on it. This place is a joke, really. This is a FALSE presentation of things by management while patients and family suffer daily in silence with no where to turn. Patients are losing their life's here and this company is only about PROFIT. There is such dirty conditions here, I don't know why the Health Department can't inspect and see the truth. Patients are left abandoned on their own when they need help while the Doctors over medicate and drug patients; not for their health and well-being but for behavior management and control. Our family is very upset over the lack of care and poor treatment at this facility. This FOR PROFIT company which owns several facilities in California gets away with it at the expense of patient suffering. We have heard daily cries for HELP from our family member who is THIRSTY and not given enough water or fluids to drink. And, HUNGRY because of lack of food being given. For god's sake, even the homeless get more attention and food at a homeless shelter. My family member has paid taxes his whole life and supported others to only be treated worse then the dogs at a dog shelter are treated. I'm not over stating how bad it is here at this facility. Our family members health condition has has become grave in only 9 months being there and we are very concerned over this LIFE EMERGENCY that has given us grief beyond the medical condition. We pray for authorities to investigate this sad facility and protect the life's of those patients who reside there now. There is nothing words from management can do to change the worsening condition of our family member as long as they are at this facility. And, as long as their at this facility their LIFE IS IN DANGER. read more

This place is so horrible. If u could give negative stars i would!!! If u love ur familey dont bring them here!!! It looks nice on the outside. The lobby looks better now that they finished construction but the rooms and the hall ways are horrible!!! Dont let the "charming cozy" outsode fool you at all. The beds are all broken and dirty as hell. The bathrooms stink. It always smells like shit and piss everywhere. They never have enough blankets and pillows for the patients. The food is horrible!!! But I do like that they have a snack girl go to everyone's room to offer them something. But sometimes that's the only thing the patients will eat all day since "the tray went missing" I don't understand how an LVN can be a ADON? If i want to talk to the ADON/DON they better be an RN or else i don't want to talk to them at all... So there is this "ADON" named jose (LVN).... He don't do shit He is always sitting at the station playing games on his phone. And when I asked him a question about my family member, he gave me this blank look on his face, says "I don't know. Go ask her." Pointed to another nurse and took off. I asked the nurse who he was and that's when I found out this lazy guy was not only the ADON of that unit but also the Unit MANAGER! He is not only rude but also incompetent. Then the staff... the staff is not only UNDERPAID but also SEVERELY UNDERSTAFFED AND OVER WORKED! The staff all know it too... And when they have a new employee come in for training u can hear the staff either be rude/mean/or taking out there frustration on to them to the point where people quit on the spot or "go on break" but they never come back. I was talking to one of the CNA's there and they said they looking for a new job since the administrator is refusing to give out yearly raises and she has been due for her raise for over 3 months now. And the way this CNA works she deserves far better then what she is making right now. They need to take care of what little loyal staff the have. Well this place is SEVERELY UNDER FUNDED SINCE They don't have enough supplies, staff, can't give out raises, and cut nurses to save money and give 1 person the workload of 3. I dont understand how each station has and "ADON" that don't do shit but hang out at the station, but if a nurse calls off why don't they get on the floor and help out? Or help answer phones, and call lights? Why let the patients suffer while there fat lazy ass sit and do nothing? Not everyone of the staff is rude. There are very few good workers there but they are too good for this place. I am trying to get my family out of that hellish place right away!!!! read more

My elderly mother was brought here under the pretense of being admitted to another post acute, and without patient's/POA permission. The doctor at the initial acute care hospital, slandered a previous skilled nursing facility (which did everything necessary to assure patient safety and well-being) to cover negligence on this hospital's part (patient prematurely discharged in less than qualified condition) and pulled a bait and switch - my suspicions as to why are obvious and I will let the reader draw their own conclusions. Through the HMO, I never received an official document authorizing Terrasina to receive the patient, the discharging hospital doctor becoming terminally unavailable for comment, and the reality and concern of this, overlooked and down played. At Terrasina, the quality of care was mixed, you had some rock stars, in reception, admitting, secretarial/nursing station, nursing, nursing assistance, physical therapy and case management, but I saw a common theme, the staff afraid of their subordinate, and with some, ethics compromised, care lessened, and others just down right ill industrious. Too bad, the place is being renovated and I can see the outcome being stimulating for both staff and patient. (However industrial toxins concerned me) I hope new medical equipment, beds and leadership are in the future, happy staff, happier patient. The post acute physician was a phantom, never introducing him or herself, lacking professional courtesy by not returning phone calls, making Terrasina resemble a "business" rather than a house of healing, leaving the patient's advocate out of touch (delaying otherwise prudent decisions), and transference to another facility, a hardship. I never saw the food, so I can't comment. There was a safety issue/abuse (night shift) that was quickly resolved so no formal complaint issued. For the sake of those who truly are dedicated to post acute care, I salute you, and hope your above and beyond, endless passion pulls up others who resemble minimalists at best, and unfortunately cast a rather large shadow on the committed staff. The place has potential, I hope as a whole, this is recognized, along with a more open and honest approach and much needed transparency. Cheers. read more

If I could give this place negative stars, I would. The management aka the DON "Pam" & Administrator "Gary" in this place are a joke, and the DSD Michelle is in her own world, you cant say anything to the woman. My father was a patient here for a few years. I had to get him the hell out of this place. The management DOES NOT care for their residents nor their workers. There was a specific resident that my father was telling me about. He told me this resident would cuss him out and many others. Also threatening to have someone k*** him and others. My dad said that The resident even went to his room and spit in his face! DISGUSTING! My dad is there to get better, not be threatened or spit at! Who does that!!!! He was so scared he would NOT eat for days or sleep because he kept his eyes open. I had to beg him to please eat and rest. It hurt seeing my dad this way. He told me he does not want to fall asleep and someone hurt him. I talked to one of the charge nurses and she told me that resident threatens the staff too! She also told me that Gary and Pam are well aware. Why is the DON & administrator okay with this? I eventually talked to both the administrator and DON & they both told me "we are aware, I am sorry we will have a talk with him." Michelle the DSD told me "Oh he is my friend, I will talk to him." That is the Weirdest thing ive heard. Regardless, They did not keep their word. Well, from what I heard, they do NOT care, its about the money. This is a sad place. I got close to certain charge nurses. They told me that Gary said if they are not happy, leave. He wants them to quit so they cannot get unemployment. Regardless, my father is in a new nursing home, and he is getting the care he deserves. There is no patient threatening him or spitting in his face! Please do yourself a favor and do not pick this place. Also before I end this, state is always here. They cover up their mess before state comes. This place has turned to a Psych facility. All the doors locked, god forbid a fire broke out. This is supposed to be a nursing home. Not a prison nor another Patton. I really hope The owners get new management here. read more

When did Terricna Post Acute care turn into a Homeless Shelter!?!? Yesterday, I went to go visit one of my family members, who currently resides there. I walk through the main entrance and found at least 5 people at approximatley 6:40am laying all over the place. Like, WoW.... Two had patients blanks and sheets laid out on the floor like a bed, one was sleeping on a desk and the other lined up chairs together to make a bed, like wow!!! As soon as I passed by one of the ladies asked me for money, so she can go buy food... After I went to go visit my loved one. I went outside to smoke, one of them asked me for a cigarette, I gave them one. I was very curious, why!?!? They're hanging out in the main lobby. I asked... They told me, that they are not patients there, that they're currently homeless and don't have a stable place to stay, so they spend the night there, where they get free roof, free showers and meals. I was like wow... I guess Terracina has opened there facility to help homeless people. Unfortunately, Not to be mean, due to they dont have security people checking random people in and out... Plus, they are letting anybody come on site. I don't feel that my family member is safe there anymore. Don't get me wrong, the staff there always helpful, but i feel there is not enough to keep watch. I'm going to transfer my family member to another facility, I dont wanna take a chance for an accident to happen. I rate this place one star, due to I wish I was notified that the facility had opened their doors as Homeless Shelter, plus one of the gentlemen had a beer in his hand and drinking on site and I dont want my family member gettong mixed with that type of crowd. read more

am writting this review of TPA and i want people to really k ow what is going on in tbis place. Tbos is my story about my dad. There comes a time when you find yoyr family member isnt ready to go home after a hospital stay.The hardest decision i had to make was to put my father in a skilled nursing facility. I would of preferred to take him home but the idea of me taking care of him and his pride would let him. So i decidef to bring him to Terracina Post Acute. I walked in expecting the worsed but what i found was a group of nursing and other staff members more like family. There were several department heads that came togheder to meet my father from the start and to welcome him and to make sure that his stay here will be a good experience. Physical therapy motivated him to get better and get on his feet. Respiratory cheked on him to make sure his breathing okays and he doesnt need anything. The doctor met with him the first day and reasured him that all his needs will be met and he will be going home soon. My dad was not a respiratory patient . But the department head Pat met with him to discuss how he is doing and told him that if he needs anything to ask her and continued to check on him on the daily basis up untill he was discharged. Nursing staff very attentive and made sure my dad was comfortable. The call light was answered in a timely manner . Now my dad is home and doing really well. The nursing staff made him feel like familiy and not just a patient and thats want i want everyone to know to know about Terracina . read more

ok first of all NONE of the pictures on the website are real only the picture of the building! Great staff well some but horrible management from ALL departments. Had my husbands mom there for 3 weeks we tried to take her out the next day of her stay but we had trouble with the insurance but anyways! st1 was her st. unit leaders from 1 and 3 are unprofessional. The admissions girls and all that work in the lobby look like they work in the strip club what kind of nursing home is this?????? had problems with the activities lady she was really rude to me her staff was trying to help me and she was rude even to her staff what kind of profession is that???? you can tell the staff aren't happy there all I hear were complains I don't blame them but its a shame to have all this patients hearing that plus us the family we want a nice, clean, positive environment for our love ones! please post real pictures of the nursing home and of the staff don't try to lie to all the families that are trying to put there love ones in a safe place! god bless!!!! read more

My name is Frank, I am an 80 year old retired army veteran. I have experienced a lot of things in my 80 years, many amazing highs and challenging lows. Very recently I found myself facing one of the most difficult situations of my life helping my adult child navigate an extreme health crisis. I visited several skilled nursing facilities in the area i was less than impressed by, when i walked into Madison Grove in Redlands and met Gracie the admissions coordinator, I knew I was in the right place. She had a very special way of addressing all of my concerns and setting my mind at ease, she is very warm and has a magnetic personality and you can tell she knows her stuff. It has been a few days since this interaction and nothing I have witnessed in this facility has done anything but solidify that I made the right choice. Thank you Gracie, and thank you to the rest of the staff at Madison Grove. It's comforting to know that my daughter is in a place where all of her needs will be addressed with genuine empathy and professionalism. read more

I asked about my family member from a cna , she told me I need to ask LVN, the lvn asked Gary and came back and told me he said "tell her to get in line , like the rest of them." I PROMISE you there are hella issues. I've heard from cnas, lvns. I am SO GLAD , I took my dad out. Thankfully I didn't put my mom at this horrible plave . Clean place, great energy. I encountered 2 handsome, diligent, well spoken young men who gave me hope. Hope for the future, hope that this current generation of young people aren't lost and hopeless. Hope that hardwork ethic, integrity, character, accountability, selflessness, honor, and ​taking joy in service of others will still exist in the years to come. I didn't catch their names but 1 was tall, handsome, adorably sweet, with long hair, large and strong in stature and soft in temperament. I believe he was a stock boy of some kind. Although he seems capable of far more. The other was average height, athletically built, handsome, confident, charismatic, with big energy and wisdom beyond his years. I believe he said he directs activities. This facility is a great place, made exponentially better by the presence of these 2 young men (and many others I'm sure) Please, let these gentlemen know how impressive and valuable they are. Respiratory Dept is THE BEST - Jenn, you and your team have been absolutely phenomenal, and have made so many things, so much easier on me. As you may already know, I was previously at UCLA Medical Center on life support. The care I received was the best and I could not have asked for anything more. They saved my life. I was very concerned about being transferred to another facility where I may not receive the same level of care. I can tell you, hands down, you are just as good as UCLA. You have been so attentive, caring, kind, and have been right on top of every bit of my care. I have never had to worry about what my status was, or what the next plan was. You have informed me of everything through this entire process, and everything you have said, has happened, in the time frame you stated. I can't thank you enough, you are amazing at what you do and I am so grateful for you, and all your help. read more

Worst nursing facility I have ever seen! Always short staffed , patients living in their own filth. Social workers there are completely useless! They only are there to take your $$$$ and let your love ones die. Julie B. social worker director needs to be fired immediately and not let near any other patient ever again! . I was impressed with the care that my father received at Madison Grove. I appreciated all the nurses, staff, doctors, and other health professionals that treated my dad with compassion. He was not able to do anything for himself and he was treated with dignity and kindness. I'm especially grateful for Julie that went out of her way to make sure she understood my family's needs and what she could do to make my dad comfortable. Every time we came to visit, the nurses and health workers greeted us with a smile and always asked if there was anything they could get us. They knew that it wasn't just my father going through the most difficult time in his life. It was a comfort to know that when we were not there with him, that he would be in good hands. read more

If I could give this place zero stars I would. This place is inhuman. My brother in law was sent here with severe brain injury. He was treated horribly, the place was dirty, hot and the staff was very rude. I wish I could say at least one nurse was nice. All the nurses here seem to be rejects from the nursing industry..no heart, no compassion towards their patients. My husband had to literally beg for the doctors to release him back to hospital emergency room. Please do your research this place is horrible. I feel so sorry for the patients that do not have a voice, they are literally left to die. I hope this could help someone. We were told all Post Acute Nursing homes are all the same.. My words are inadequate in expressing my gratitude for the compassionate care I received while going through seven months of rehab. The Physical Therapy department used their knowledge, skill and steadfast patience to give me back my life. Meca has formed a wonderful physical team & Joseph has an equally skilled Occupational Therapy Team. From what I witnessed, if the patient does not improve as expected it is because the patient is refusing therapy. If the patient is dirty, they are refusing showers. Sorry to say, too many cases of that, most of their families seemed unaware. As many other reviews stated the nursing staff is a blessing. Thanks to these folks I am home and getting stronger every day. read more

My husband was just admitted to this facility last night. Here we go again. Depression sets in. same Ole thing...again. then this woman comes in and introduces herself as Adriana. She explains how things work here. Who will be coming in to treat my husband. As she's speaking, different professionals were coming in checking on my husband and introducing themselves. So much better than the Kindred facility in Perris where we almost gave up all hope. So far, these people seem to be on it. Lots of care and concern here. It helps my heart. It's clean.
