Santa Rosa Post Acute Skilled Nursing Facility was a wonderful surprise for Me. I didn't know what to expect, but my brother received tremendous care at the facility. Every staff member was kind and helpful. No matter who I asked a question of, If they didn't Know the answer, they would stop what they were doing and find someone to help me. Jim was at the facility for nearly 3 months, and I was able to visit him nearly every day. Never did I ever see anyone treat him with anything other than great care and compassion. On the morning that Jim transitioned, When I got the call to see him for the last time, His nurse was waiting for me in the lobby. She was in tears as we walked down to his room. The care here isn't great. Do not bring your loved ones here. You have to wait in pain for two hours to get a pain pill. The place smells like urine. The room a family member is in has sticky floor. The nurses here seem like they are only here to make money while doing the absolute least possible. I truly don't know how this place is in business. Physical and occupational therapy is good and they provide you with no beds pads. Only something like a towel. Which is pretty unsanitary. read more

Wow. From the moment I walked in, I got a bad vibe. The woman behind the desk did not speak. When I asked for directions to a patient's room, she pointed. During the weekend, the reception area remained unattended while patients walked behind the desk and looked at patient's records. Weird place. read more

It's a dungeon. I was taken there for skilled nursing care and lasted one night. Upon entering, I was assaulted by a sickening, putrid odor. The lobby looks like the picture, but behind it is a living nightmare. It is like a state-maintained mental institution. The place is kept dark even in daytime. The beds and fixtures are dirty and in disrepair. The call buttons are either shut off or ignored. The man in the bed next to mine had to scream for nearly an hour to get a nurse to change his soiled diaper. Poor guy was just lying there in his feces, calling for help. No plastic urinals are issued. I was told to urinate in my diaper and eventually someone would come and change me. The staff is uniformly rude and indifferent and most of them speak no English, at least not any that is understandable. I called 911 the next morning and demanded to return to the hospital, vowing I would never go back to this gulag. If you care about yourself or a loved one, then please don't put them here in Shawshank. Like me, most would prefer death to a stay in this disgusting hell hole. read more

I wish I could give this place ZERO stars. What a nightmare! Beautiful entranceway with smiling people, but behind the scenes it is horrific. I was transferred here by ambulance and left on a bed. I was never given lunch, no dinner either. No medication for my multiple broken bones. After many many hours of yelling for help, someone finally came to assist me to the bathroom. The safety bar around the toilet was broken and smeared with stool. The bathroom was really filthy on top of that. It didn't improve from there, unfortunately. Do not allow anyone to go there. We we're finally able to transfer mom to a bed here after a long wait at a facility in Marin. Ricci in Admissions is so gracious and warm and answered so many calls when I repeatedly asked if a bed was avail yet. To my surprise last week he said a resounding "YES we have a bed available!" Granted it is only the second week, but so far I haven't met ANYONE who isn't friendly, professional and readily available when we have questions. I am so over the moon happy mom was moved from Marin. Santa Rosa Post Acute is better than the other facility was in many ways; the quality of care, hard working and friendly staff, and especially the FOOD! read more

I waited two years to write this review because I am so disgusted with Santa Rosa Post Acute. I would do whatever I can to avoid having a loved one placed here. To me, it's literally hell on earth. I can't begin to detail everything that went wrong here. It would take pages and pages. The care from day one was poor. The day she entered was the day they lost her wheelchair. Therefore, she didn't get out of bed for 9 months. She needed a specialized wheelchair and was not ambulatory at all. They did not replace. They instead ordered through Medi-Cal did, and it took 9 months, but it should have been replaced by the facility because they lost it. She further deteriorated because she never sat up losing strength. They also lost her teeth and she couldn't chew for 9 months. They didn't replace until I called the ombudman. Clothes were constantly displaced, and at times I saw other residents wearing them. I found one patient myself on the street in my loved one's night gown, demented and lost. I gently persuaded her back to the facility and they hadn't noticed this patient had wandered off. I saw staff yell and berate patients with dementia. I had the head nurse say to my face, "That bitch needs to shut up" when referring to a distressed patient. The doctor here is AWFUL, DEMEANING and gave very poor care, not even ordering the most basic of tests (like A1C for a diabetic) until I pushed and pushed. The doctor gave the worst care I have ever seen. Basically, she was ignored and they waited for her to die. She was just a body in a bed until a new body came in. Quality of life? Evaluating health? non-existent. I couldn't even get an email or a phone number to talk to the doctor. I was medical POA. They wouldn't even brush her teeth, I would see my loved one's mouth and teeth crusted in food particles over until I complained and complained. She died and not one person, not one, not a nurse, a caregiver, a front desk clerk, a social director, no one expressed condolences. I called ahead of time to tell them she died and we would be in to clean out her room. When we got there, there was a big commotion becasue "no one told us you were coming". No one assisted, stopped by to saw they were sorry-NOTHING. They never cared for her. This place is atrocious. If for some reason you are here, make the ombudman (who are WONDERFUL) your best friend. Call the state if there is a serious allegation of wrongdoing. Label all your stuff, especially phones and chargers. Chargers are CONSTANTLY stolen from loved ones. Over our time here there was an employee or two who cared. I remember one CNA who was kind and sweet and loving. One nurse who was very honest about the situation in the facility when I asked questions. She didn't lie and told me the very unflattering truth behind the stuff that was happening. This place is a complete joke. They do as little as possible with rehabilitation. Its not the CNAs or the nurses .Its the management. All they care about is their precious EXIT PLAN! They run up the bill with medicare as high as they can and then they want to get rid of you. They did nothing to help my mom. read more

I personally worked at this facility for 7 months. The one, yes I said one good thing that I can honestly say is that they have an outstanding wound care program. In my personal opinion the management team lacks professionalism, are very clickish and are NOT always honest with the staff. I suggest doing your research, taking a tour, and most definitely look at the CMS (medi-care) ratings. read more

This place is a joke. The staff is a joke. The times ive been here the front staff are ALL on their phones. The communication is horrible and the place is disgusting. They dont answer the phone because everyone is on their cell phones. If you can, avoid this place for your family member. The 5 star reviews have to be fake theres no way this facility with unattentive, rude, incompentent workers could ever receive 5 stars let alone any stars. read more

Found the facility to be very dirty and clean. Quality of care. Some of the staff really does care but it seems the upper management is just there to collect a paycheck. God forbid that you need any documentation from the front office or you want to talk to management they all seem to be unable to locate the food is terrible read more

There are 100's of reasons this place gets zero stars! The marginal 2.25 stars SR POST ACUTE is receiving here on YELP is ONLY because of a very select few nurses (THANK YOU ANGELA!) that actually care about their patients & take pride in nursing & the van driver "TAYO" (spelling?) who is also amazing! Otherwise, starting with the admissions department (COMPLETE LIARS) general services staff (MORE LIES & REFUSE TO BACK UP THE PROMISES THEY MAKE), the social services department (especially INCOMPETENT), theft of patient property... and a "head honcho" named KEVIN BILLS who is chronically "in a meeting"!!! BEWARE & AVOID putting your loved ones here for a single minute especially our Veterans who served this country with honor. SRPA should be ashamed of themselves.
